Treatment of brachial artery injuries in children, particularly those resulting from supracondylar humeral fractures, is controversial when distal pulses are absent yet the hand remains warm and pink. This article presents a retrospective study of eight children, ages 3 to 13, who underwent brachial arterial exploration because of absent distal pulses following arm trauma. Absent pulses indicate diminished blood flow, and in all eight cases brachial artery obstruction or severance was confirmed at surgery. In four of the children, who presented with cold, devascularized hands as a result of po

PURPOSE: Humeral shaft fractures may be managed conservatively or operatively; however, increasing non-union and complications after non-operative treatment have renewed interest in intramedullary fixation. Innovative intramedullary nails with distal internally locking, requiring neither fluoroscopy nor external guidance, aim to reduce surgical time, simplify the procedure, and minimise the risk of iatrogenic radial nerve injury. This study evaluates the clinical and radiological outcomes of a humeral nail utilising distal internally locking.
MATERIAL AND METHOD: A retrospective review includ

BACKGROUD: Peripheral nerve injuries are a common complication of pediatric supracondylar humeral fractures. While most resolve spontaneously, the timeline for recovery remains unpredictable. This study investigated whether earlier correction of fracture alignment-regardless of timing of definitive surgical fixation-shortens the duration of neurologic recovery.
METHODS: We retrospectively reviewed pediatric patients (< 18 years) treated surgically for supracondylar humeral fractures between October 2008 and February 2025. Patients presenting with preoperative neurologic deficits were included

BACKGROUD: The Proximal Humeral Internal Locking System (PHILOS) plate, designed for the treatment of proximal humeral fractures, demonstrates versatility when used in an inverted (upside-down) configuration. We aimed to retrospectively compare the clinical and radiographic outcomes of patients who underwent surgical treatment for extra-articular diaphyseal distal humeral fractures using either an extra-articular distal humerus plate (EADHP) or a modified PHILOS locking compression plate.
METHODS: This study included 58 patients: 39 in the EADHP group (group A) and 19 in the PHILOS plate grou

Restoring elbow joint motion is paramount for upper extremity optimum function. In end-stage elbow disease and stiffness, total elbow arthroplasty is the recommended option for older patients; however, for younger, highly demanding patients, interposition arthroplasty (IPA) is the management option of choice. We report a case of an 16-year-old female patient who presented after she had an open-grade IIIA, type AO 13C2.2 distal humerus fracture, which was managed initially by debridement and a cross-elbow external fixation. The decision was made to manage the fracture by open reduction and inte

This article presents a new method for the measurement of humeral torsion with the use of ultrasonography and discusses clinical applications of the method such as the evaluation of cubitus varus deformity after a supracondylar elbow fracture. Torsion angle increased 14 degrees from the time the patients were newborns to when they were adults. No significant difference was found between the right and left sides in healthy subjects. When humeral shaft or supracondylar fractures are treated, or when rotational deformities of the humerus are corrected, this method of measurement is very practical

UNLABELLED: An 89-year-old man presented with right elbow pain after a low-energy fall. At age 2 years, he sustained an untreated distal humeral fracture dislocation, which resulted in progressive remodeling and formation of a functional pseudoarticulation. Despite permanent deformity, he maintained stable functional motion for decades and performed heavy manual labor. Imaging revealed a displaced distal humeral fracture through the chronically remodeled elbow. Open reduction and internal fixation with a variable-angle locking plate was performed, while the pseudoarticulation was intentionally

BACKGROUND: In the setting of periprosthetic humeral fractures, the humeral stem of the implant represents a substantial challenge to the optimal method of proximal fixation. This study aimed to compare the initial biomechanical stability provided by cerclage cables with a locking plate insert versus bicortical locking screws (i.e., the gold standard for fixation) in fresh cadaveric humeri.
METHODS: After calculating the sample size, we utilized 10 sets of cadaveric specimens and created a 5-mm osteotomy gap 120 mm distal to the tip of the greater tuberosity, simulating a Wright and Cofield t

INTRODUCTION: Neglected distal humeral fractures in elderly patients often progress to painful malunion, advanced osteoarthritis, and profound loss of elbow function. When anatomical reconstruction is not feasible due to osteoporosis and chronic deformity, total elbow arthroplasty (TEA) serves as a reliable salvage option.
CASE PRESENTATION: We report a challenging case of a 68-year-old female who presented with left elbow pain and severe post-traumatic combined contracture 5 months after injury. She had initially been treated by a non-professional with above-elbow plaster immobilization in f

Monteggia fractures are characterized by a fracture of the ulna associated with radial head dislocation. T-condylar fractures of the humerus are intra-articular injuries that disrupt the distal humeral epiphysis. This report describes a rare case of a pediatric patient with an ipsilateral T-condylar humeral fracture and a Monteggia fracture-dislocation. A 10-year-old girl presented with elbow pain and deformity after falling while playing basketball. No open wounds or neurological deficits were observed, and the radial artery was palpable. The radiograph revealed a pediatric T-condylar humeral

BACKGROUND Surgical management of distal humerus fractures in hemodialysis patients with an ipsilateral arteriovenous shunt is challenging due to shunt injury and bleeding risks. Treatment selection requires careful patient evaluation and arteriovenous fistula (AVF) preservation. This report describes 2 such cases managed with open reduction and internal fixation using locking plates. CASE REPORT Case 1: A 62-year-old man with chronic renal failure sustained an AO type C1 distal humerus fracture in the shunt-bearing limb. Surgery was performed in the supine position without a tourniquet, using

INTRODUCTION: the "Inside-out" technique is one of the options for medial epicondyle fracture fixation of the humerus. It involves preparing the fixation from the internal portion of the fracture to the external portion. Then, the fracture is fixed with a cannulated screw from the external to the internal portion. This study aims to describe our experience with this technique, its outcomes, and its complications.
MATERIAL AND METHODS: this is a retrospective observational study describing a series of cases treated with the "Inside-out" technique for medial epicondyle fracture fixation from 20

BACKGROUND: O'Driscoll popularized the principle of linked column fixation for distal humerus fractures. Despite the linked column concept being widely accepted, there are few reported techniques to accomplish this goal. A novel device was designed based on the principles of linked columns. An interlocking beam is used to connect the medial and lateral plates, creating a unified fixed angle construct. Our primary objective was to report clinical outcomes across multiple institutions for a linking beam used in distal humerus fracture fixation.
METHODS: A retrospective series was collected from

Distal humerus fractures are a contemporary problem because the life expectancy, autonomy and functional demands of older patients continue to grow. This is combined with surgical advances in bone reconstruction, especially in fragile patients. A distal humerus fracture in an older adult is a serious injury with an uncertain prognosis. In fact, damage to the elbow joint in this complex anatomical area overwhelmed by low-quality bone occurs in patients who often have unfavorable characteristics (fragile skin, low physiological reserves, organ failure) combined with pharmaceutical treatments tha

PURPOSE: Inconsistent outcomes have been reported in several prior studies of elderly patients with distal humerus fractures treated with open reduction and internal fixation (ORIF). We evaluated the outcomes of ORIF using modern precontoured plates exclusively in a parallel orientation.
METHODS: A retrospective review was performed to identify the patients aged over 65 years who sustained an isolated distal humerus fracture between 2015 and 2019. We identified 22 patients who underwent distal humerus ORIF using parallel, precontoured locking plates. Electronic medical records were reviewed f

Lesions of the nerves are a relatively frequent complication after humeral shaft fractures, the nerve most commonly affected being the radial nerve. In the case of proximal fractures of the humerus, particularly dislocation fractures and luxated fractures, lesion of the axillary nerve and irritation of the plexus, are most frequent. In distal humeral fractures the ulnar nerve is most frequently first place and shows a serious complication. The therapy of the fracture must differ in most of these cases from conventional procedure. We believe that primary paresis of the radial nerve is a relativ

This study investigated the clinical and radiologic outcomes of lateral humeral condyle (LHC) fractures in children and evaluated the functional outcome of these injuries according to the type of treatment using the Quick DASH questionnaire. Data on consecutive children admitted to the Emergency Department for LHC fracture (01/11-12/18) were collected from their charts. Anterior-posterior and lateral radiographs of the injured elbow were used to classify each fracture according to Jakob's classification, and to detect any other concomitant bone lesions. Data on pain, stiffness, impact on daily
