Small decreases in serum Ca(2+) and more prolonged increases in serum phosphate (P(i)) stimulate the parathyroid (PT) to secrete parathyroid hormone (PTH), and 1,25(OH)(2)D(3) decreases PTH synthesis and secretion. A prolonged decrease in serum Ca(2+) and 1,25(OH)(2)D(3), or increase in serum P(i), such as in patients with chronic renal failure, leads to the appropriate secondary increase in serum PTH. This secondary hyperparathyroidism involves increases in PTH gene expression, synthesis, and secretion, and if chronic, to proliferation of the PT cells. Low serum Ca(2+) leads to an increase in…

Bisphosphonates (BP) are pyrophosphate analogs that include very potent inhibitors of bone resorption. BPs act directly on the osteoclast, suppressing isoprenylation by inhibiting farnesyl diphosphate synthase in the cholesterol pathway, which causes osteoclast inactivation. BPs should therefore reduce the bone loss produced by any cause, including hyperparathyroidism and hypercalcemia of malignancy (MIH), caused by parathyroid hormone (PTH) and PTH-related protein (PTHrP), respectively. BPs at higher doses than used in osteoporosis are indeed the treatment of choice for malignancy-induced hyp…

Renal osteodystrophy is an important concomitant disease in chronic renal failure. Secondary hyperparathyroidism cause high turnover bone. Hypocalcemia and phosphate retention stimulate the parathyroid and to proliferation of the parathyroid cells. In these days vitamin D deficiency, resistance to vitamin D, abnormality of sensitivity to calcium, direct effect of phosphorus , more severe form of parathyroid hyperplasia, abnormality of gene and increased skeletal resistance to PTH are pointed as cause of parathyroid hyperfunction.

Histopathological and pathophysiological investigations including the genetic approach have been contributing to management of renal hyperparathyroidism (HPT). In renal failure, parathyroid glands initially proliferate diffusely and polyclonally, and then are transformed to monoclonal nodular hyperplasia with aggressive growth potential and diminished expression of both the vitamin D receptor and calcium-sensing receptor. When more than one parathyroid gland progresses to nodular hyperplasia, HPT is refractory to medical treatment. To prevent advanced renal HPT, progression to nodular hyperpla…

Primary hyperparathyroidism is a systemic disease, more and more frequently recognized-concerning 1 to 3% of the population. Statistically appears in 1 of 1000 adults, with significant advantage of women. In Poland every year about 30 new cases are noticed and incidence increases with an age. In spite of significant advance of the knowledge, it still makes a lot of diagnostic troubles. It appears to be non-specific illness, characterised by just one symptom, mainly urolithiasis, sometimes chronic ulcer disease, chronic pancreatitis, arterial hypertension, disorders of the movement or psychic d…

OBJECTIVE: To define the role of surgery for organic primary hyperparathyroidism and to evaluate the diagnostic and operative methods applied.
DESIGN: Retrospective, non-randomized, non-blinded evaluation. Case series.
SETTING: Tertiary referral center. Department of Surgery.
SUBJECTS AND METHODS: The report documents experience in surgical management at a single institution of 312 patients with primary (and 498 patients with secondary) hyperparathyroidism in a period from 1974 to 2000. Patient symptoms, clinical features, diagnostic and localization methods, operative management, and posto…

Primary hyperparathyroidism (PHPT) is considered a cause of secondary osteoporosis as a consequence of its known catabolic effect promoting osteoclast activity and bone resorption. However, recent in vitro and in vivo studies have shown that parathyroid hormone (PTH) may also have an anabolic effect on the mammalian skeleton. These two paradoxical effects of parathyroid hormone are discussed in the light of recent results of basic research, and of bone densitometric and histomorphometric data collected in patients affected by PHPT. Review of the literature leads to the conclusion that in PHPT …

Hyperparathyroidism (HPT), resulting from the excess of endogenous parathyroid hormone is cited as one of diseases which cause secondary osteoporosis. HPT consists of primary (1 degree) and secondary (2 degrees) HPT, resulting mainly from chronic renal failure (CRF). HPT is easily distingishable from primary osteoporosis by biochemical measurements. Parathyroidectomy (PTX) is the only option available for the radical cure of 1 degree HPT and more than 10% increase in bone mass occurs after PTX. On the other hand, dietary phosphorus restriction, phosphorus binders, active vitamin D3 metabolites…

The history, physical and radiologic findings, treatment and pathology in five unusual cases of hyperparathyroidism is presented. The hyperparathyroidism was caused by a large (113 grams) mediastinal adenoma in the first patient, who is alive 25 years after surgery. A parathyroid carcinoma with compression of the esophagus was documented in the second patient. This patient is alive and normocalcemic 23 years after surgical treatment. A third patient with hyperplasia returned with hypercalcemia 20 years postsurgery requiring reoperation. A fourth patient with advanced bone findings was found to…

UNLABELLED: Familial primary hyperparathyroidism (PHPT) is usually encountered in the context of multiple endocrine neoplasia (MEN) syndromes. Few families have been reported in the literature where PHPT was the only abnormality. However, in these families no long-term follow-up data were reported and no genetic linkage studies were performed.
OBJECTIVE: We investigated a large family with a familial primary hyperparathyroidism for biochemical and genetic markers of multiple endocrine neoplasia syndromes.
DESIGN: A family screening study.
PATIENTS: Thirty-seven family members participated i…

Over the last 20 years, widespread assay of serum calcium in patients with diverse and often minor symptomatology and its introduction into routine check-up lists in perfectly healthy subjects has led to the identification of asymptomatic hyperparathyroidism defined as apparently isolated hypercalcalcaemia. In most subjects, the diagnosis is easily confirmed by assaying intact parathormone 1-84. The real problem is management of this condition, particularly in elderly subjects. Due to the lack of any truely effective medical treatment, the only alternative is between abstention and surgical ex…

The number of glands and the exact location of each normal and pathological parathyroid identified in 416 parathyroidectomies for primary and secondary hyperparathyroidism have been recorded during the last 25 years (1966-1990). Nineteen per cent of all glands were found in ectopic locations. Half of these were distant from the thyroid lobes, most frequently posteriorly alongside the oesophagus, or in the upper anterior mediastinum encapsulated in the thymic remnant. Their discovery required, in some cases, extensive dissections, but a sternotomy was necessary in two cases only. Parathyroid gl…

We report on 14 patients suffering from persistent (n = 13) and truly recurrent (n = 1) primary hyperparathyroidism. Reoperations were successful in 13 cases. Cervical reexploration was only required for 5 patients who had previous operations by unexperienced surgeons in other clinics. In 5 out of 7 patients operated by us, the causes of the persistent primary hyperparathyroidism were 4 adenomas and one hyperplastic gland in the mediastinum found by median sternotomy. The remaining cases were a rare ectopic location of the inferior glands and an inadequate resection in four-gland-hyperplasia. …

Historically, primary hyperparathyroidism during pregnancy was associated with significant risk of maternal morbidity and fetal death. Maternal hypercalcemia results in fetal hypercalcemia, leading to suppression of fetal parathyroid gland function. Neonatal hypocalcemia with tetany is a common occurrence after birth when maternal calcium flow is interrupted. From 1930 to 1990, 109 cases of women with primary hyperparathyroidism associated with pregnancy have been reported, 39 of whom were treated surgically before delivery. Although fetal mortality rates for medically treated women have impro…
