Management of male neurologic patients with infertility.

Many aspects of fertility rely on intact neurologic function and thus neurologic diseases can result in infertility. While research into general female fertility and alterations in male semen quality is limited, we have an abundance of knowledge regarding ejaculatory dysfunction following nerve injury. Normal ejaculation is the result of coordinated reflex activity involving both the sympathetic and somatic nervous systems. Nerve injury can result in retrograde ejaculation, and anejaculation. With retrograde ejaculation, the ejaculate is propelled into the bladder instead of out through the ur

[Varicocele and male infertility: AFU 2006 guidelines].

The routine assessment of an infertile man with varicocele must comprise complete clinical interview including the patient's medical and reproductive history, physical examination and at least two sperm counts. Imaging examinations are not indicated to characterize the varicocele, except when physical examination is inconclusive. However; scrotal ultrasound can be useful in infertile men to detect concomitant diseases, especially testicular tumours. Treatment of varicocele must be proposed when all of the following conditions are present: 1) the varicocele is palpable; 2) the couple's infertil

CFTR mutations and polymorphisms in male infertility.

Apart from cystic fibrosis, mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene are also involved in congenital bilateral absence of the vas deferens (CBAVD). A mutation is identified in about 80% of the CFTR genes derived from CBAVD patients; the genetic defect in the remainder is yet unknown. In contrast to CF patients, when CFTR is involved, at least one of the mutant CFTR genes of CBAVD patients harbors a mild mutation. A polyvariant mutant CFTR gene is the most frequent CBAVD causing mutant CFTR gene. Here, combinations of particular alleles at several polymor

Male infertility: risk factors in Mongolian men.

AIM: To determine the most common risk factors of male infertility in Mongolian men attending an infertility clinic. METHODS: A prospective, case-control study was conducted in which 430 men were enrolled. All the men had sought their first infertility evaluation between 1998-2002 in the State Research Center on Maternal Child Health, Ulaanbaatar, Mongolia. They were divided into two groups depending on the results of their semen analysis: 191 with abnormal semen and 239 with normal semen profile. Univariate and multivariate analyses were performed to determine any association between risk fa

Prostatitis and male infertility: evidence and links.

Prostatitis and infertility are common disorders in men and many researchers have investigated a possible link between the two disorders. After almost 3 decades of research the answer to this question remains unknown, although ample evidence exists to support a relationship between male infertility and prostatitis. This paper discusses this evidence and then derives a rational treatment approach to the man with infertility and prostatitis.

Chlamydia trachomatis and male infertility in Tunisia.

OBJECTIVE: Chlamydia trachomatis is a common sexually transmitted micro-organism. The impact of chlamydial infection on semen parameters and male fertility is controversial. Our purpose was to determine the prevalence of C. trachomatis in the male partners of infertile couples in Tunisia and to assess the relationship between chlamydial infection markers and male infertility. METHODS: Chlamydial DNA in urethral and in semen specimens was determined by using the Cobas Amplicor polymerase chain reaction (PCR) assay and chlamydial immunoglobulin G (IgG) antibodies were measured by micro-immunofl

[Male infertility. Significance of life and occupation].

A retrospective study was carried out which focused on the impact of lifestyle and exposure to different external factors on sperm quality. A questionnaire was sent to 312 men who had previously attended our laboratory for semen analysis. Their answers were correlated to sperm quality. Men exposed to a relatively high degree of X-rays were found to have significantly reduced sperm count. A previous symptomatic genital infection did not significantly influence the sperm quality, but an earlier sexually transmitted disease in the female partner was associated with a reduced rate of pregnancy. New areas of research in male infertility.

Recent research in male reproduction holds much promise for future clinical application. Research on the relationship between sperm, semen, and the immune system may provide novel approaches to treating immune-related infertility. Investigations of sperm motility have shed new light on these complex mechanisms and may lead to rational approaches to the improvement of sperm function. New assays for secretory products unique to the testis show potential as markers for specific testicular cellular functions. In-vitro fertilization promises to become a viable treatment option for couples with male

Male factors in infertility--a preliminary report.

Findings in 325 consecutive primary seminal analysis done at the Infertility Treatment Centre of IPGMR, are discussed. The clinical profile of the infertile couples was recorded. Semen was examined for quantity, sperm density, motility and morphology. An attempt was also made to correlate past illnesses with the quality of semen. 65 patients (20%) showed azoospermia, while 29 (8.9%) showed sperm count of less than 10 million/ml. History of past illnesses which contribute to azoospermia/oligospermia was obtained in 46% of azoospermia cases compared to only 10% in subjects showing sperm count of

Use of methenamine hippurate in male infertility.

In sperm-freezing for infertility patients with low sperm counts, we found that semen containing high numbers of white blood cells, greater than 15 to 20 per high-power field, was both poor in quality and difficult to freeze. In examining these patients carefully, we found that many had smoldering prostatitis and some an overt history of prostatitis. We treated them with methenamine hippurate using various methods and found that by decreasing the white blood cell count in their semen, we could frequently improve the semen quality, particularly sperm motility, also achieve pregnancies in some c

Testicular biopsy in evaluation of male infertility.

Testicular biopsy findings in 100 infertile men were correlated with the clinical findings. Mild or moderately severe tubular lesions were seen in 57 cases and severe changes in 43. Clinical examination and semen analysis were no guide to the severity of the testicular lesion. Though patients with normal sized testes more commonly had mild tubular lesions, many were severe. Patients with small testes more often had severe lesions but some had only mild tubular changes. Biopsy findings in both aspermic and oligospermic patients ranged from normal to a complete loss of germinal tissue.Testicular

Iatrogenic male infertility: medical and surgical treatments that impair male fertility.

Many medical and surgical treatments result in impaired male fertility. Sometimes impairments are permanent, while other times they may be reversible. Clinicians who treat urologic and nonurologic problems, as well as those of us who treat male and female infertility should understand what treatments affect which aspects of reproduction and what options for management are available. Conditions for which treatment may impair fertility range from benign prostatic hyperplasia to cancer to behavioral health issues. This month's Views and Reviews summarizes these conditions, the mechanisms of ferti

Small Non-coding RNAs and Male Infertility.

Male germ cells carry a complex RNA payload, whose quantity and composition varies during the different stages of spermatogenesis. Small non-coding RNA comprise microRNAs (miRNA), piwi-interacting RNAs (piRNA), tRNA derived small RNAs (tsRNA), endogenous siRNAs, and rRNA-derived small RNAs. MicroRNAs regulate the self-renewal and differentiation of spermatogonial stem cells, as well as their progression through meiosis, while piRNA are thought to play a crucial role at pachytene stage of adult spermatogenesis, since defective piRNA processing has been found in azoospermic men with spermatogene

Tissue engineering studies in male infertility disorder.

Infertility is an important issue among couples worldwide which is caused by a variety of complex diseases. Male infertility is a problem in 7% of all men. In vitro spermatogenesis (IVS) is the experimental approach that has been developed for mimicking seminiferous tubules-like functional structures in vitro. Currently, various researchers are interested in finding and developing a microenvironmental condition or a bioartificial testis applied for fertility restoration via gamete production in vitro. The tissue engineering (TE) has developed new approaches to treat male fertility preservation

Where do obesity and male infertility collide?

The parallel rise in obesity and male infertility in modern societies necessitates the identification of susceptibility genes underlying these interconnected health issues. In our study, we conducted a comprehensive search in the OMIM database to identify genes commonly associated with male infertility and obesity. Subsequently, we performed an insilico analysis using the REVEL algorithm to detect pathogenic single nucleotide polymorphisms (SNPs) in the coding region of these candidate genes. To validate our findings in vivo, we conducted a comprehensive analysis of SNPs and gene expression of

TENT5D disruption causes oligoasthenoteratozoospermia and male infertility.

BACKGROUND: Oligoasthenoteratozoospermia (OAT) is one of the most complex aggregators of male gametic problems. However, the genetic etiology of OAT is still largely unknown. OBJECTIVES: To reveal the new genetic factors responsible for male infertility owning to OAT and reveal the outcomes of the affected patients from intracytoplasmic sperm injection (ICSI). MATERIALS AND METHODS: Two infertile men with typical OAT were recruited in 2018 and retrospected a cohort that included 47 patients with OAT from 2013 to 2021. Fifty healthy men with proven fertility served as control subjects. To ide
