OBJECTIVES/HYPOTHESIS: Direct suspension laryngoscopic biopsy performed under general anesthesia is the conventional management for obtaining pathological diagnosis for neoplasms of the larynx, oropharynx, and hypopharynx. Since the development of distal chip laryngoscopy and digital imaging systems, transnasal flexible laryngoscopy tissue sampling has gained popularity as an office-based procedure. Additional assessment with narrow-band imaging (NBI) can help to increase the diagnostic yield. The aim of the study was to evaluate the accuracy, sensitivity, and specificity of a novel diagnostic

BACKGROUND: Difficult airway intubation is an emergency condition both at the emergency department (ED) and in out-of-hospital situations. Retrograde intubation (RI) is another option for difficult airway management. There are limited data regarding the successful rate of RI compared with direct laryngoscopy (DL) intubation, the commonly used method in the ED.
METHODS: This study was a randomized, controlled trial. Participants were randomly assigned to either the RI or the DL technique to attempt intubation on a difficult airway mannequin (Cormack and Lehane grades 3-4). First, all participa

BACKGROUND: The McGRATH MAC Video laryngoscope (Covidien Japan) is a relatively new device for tracheal intubation, which provides an excellent glottis view. We here report the clinical experience of the McGRATH in 100 patients who received general anesthesia.
METHODS: Tracheal intubation with the McGRATH was performed in 100 patients requiring oro/naso tracheal intubation. The view of glottic opening was scored according to the classification of Cormack-Lehane. The time required to intubate and the number of intubation attempts were recorded.
RESULTS: Cormack-Lehane classification 1 or 2 wa

Intubation of patients with a supraglottic mass causing obstruction of the glottis remains a difficult problem for the experienced anesthesiologist. Awake fiberscopic endotracheal intubation is the recommended approach in such cases; however, use of a video laryngoscope for awake intubation can be an alternative to a fiberscope. Here we present two cases of awake intubation using a King Vision™ video laryngoscope in patients with a supraglottic mass, and a literature review on use of video laryngoscopes for awake intubation. After topical anesthesia and sedation with opioids, the patients were…

OBJECTIVE: To compare the image quality provided by rigid laryngoscopes versus flexible distal-chip laryngoscopes when documenting the same laryngeal pathology.
METHODS: This paper reports a prospective single-blind study. Ten early stage glottic cancer cases were selected. Photographs of the pathologies were taken using both rigid and flexible distal-chip laryngoscopes (a total of 20 photographs). Nineteen clinicians were asked to review the laryngoscopic photographs; the clinicians were provided with a worksheet, which included questions regarding the clinical description, photograph qualit…

BACKGROUND: Softer endotracheal (ET) tubes are more difficult to navigate in the oropharynx than the stiffer polyvinyl chloride (PVC) tubes during nasotracheal intubation (NTI). Cuff inflation has been used to guide PVC tubes into the laryngeal inlet during blind NTI, but it has not been tested when performing NTI under direct laryngoscopic guidance. We assessed the role of cuff inflation in improving oropharyngeal navigation of 3 ET tubes of varying stiffness during direct laryngoscope-guided NTI. Simultaneously, we also assessed and compared the nasotracheal navigability and incidence of nas

BACKGROUND: Injury to the vagus nerve or one of its branches during carotid endarterectomy (CEA) can result in vocal fold paralysis (VFP), but the exact mechanisms and site of injury responsible for VFP after CEA are unclear. The aim of this study was to identify the site of nerve injury in patients with VFP after CEA using magnifying laryngoscopy.
METHODS: We performed 96 consecutive CEA procedures in 87 patients over 5 years. After 56 CEA procedures, we examined vocal fold movements with a flexible nasolaryngoscope and detected VFP in 5 of 40 cases (9 %). At 6-8 weeks after CEA, these five

We studied the efficacy of the Coopdech videolaryngoscope Portable VLP-100, by comparing it with a Macintosh laryngoscope, and another video … e trachea using any device. Our results indicate that, in patients with difficult airways, the videolaryngoscopes (VLP-100 and AWS) would provide higher success rates of tracheal intubation than the Macintosh laryngoscope, but the VLP-100 may be inferior to the AWS.

STUDY OBJECTIVE: Glottic visualization with video is superior to direct laryngoscopy in controlled operating room studies. However, glottic exposure with video laryngoscopy has not been evaluated in the emergency department (ED) setting, where blood, secretions, poor patient positioning, and physiologic derangement can complicate laryngoscopy. We measure the difference in glottic visualization with video versus direct laryngoscopy.
METHODS: We prospectively studied a convenience sample of tracheal intubations at 2 academic EDs. We performed laryngoscopy with the Karl Storz Video Macintosh Lar

OBJECTIVES: Video laryngoscopy has been shown to improve glottic exposure when compared to direct laryngoscopy in operating room studies. However, its utility in the hands of emergency physicians (EPs) remains undefined. A simulated difficult airway was used to determine if intubation by EPs using a video Macintosh system resulted in an improved glottic view, was easier, was faster, or was more successful than conventional direct laryngoscopy.
METHODS: Emergency medicine (EM) residents and attending physicians at two academic institutions performed endotracheal intubation in one normal and tw

Extremely low birth weight (ELBW) infants frequently require endotracheal intubation for assisted ventilation or as a route for administration of drugs or exogenous surfactant. In adults and less premature infants, the risks of this intubation can be greatly reduced using video laryngoscopy, but current products are too large and incorrectly shaped to visualize an ELBW infant's airway anatomy. We design and prototype a video laryngoscope using a miniature camera set in a curved acrylic blade with a 3×6-mm cross section at the tip. The blade provides a mechanical structure for stabilizing the t

The McGrath video-laryngoscope was used in a patient with immobilized cervical spine and suspected difficult airway after a high level fall with traumatic brain injury and suspected cervical spine trauma. The airway was successfully secured in the preclinical setting using an oral endotracheal tube with full view of the glottis. Because video-laryngoscopy offers potential advantages compared to established airway management techniques, further studies are required to evaluate the significance of video-laryngoscopy in prehospital emergency medicine.

Patients with Pierre Robin sequence are a classic model for patients with a difficult airway. In these patients tracheal intubation may be facilitated using the lateral approach. In a 3-day-old newborn with Pierre Robin sequence, scheduled for anaesthesia to take a mould of the cleft palate, we used a video-intubation laryngoscope to give a video-display of the lateral approach intubation technique to the junior colleagues. The video-laryngoscopic view allowed detailed demonstration of the intubation procedure to all persons present. In addition, video-transmission of the laryngoscope picture

In the practice of medicine, tracheal intubation is a critical procedure, yet medical education often lacks quantitative instructional methods, relying instead on intuitive guidance from experts. This study used motion capture and eye tracking to analyze differences in movement and gaze between novices and experts during tracheal intubation. It also compared the use of direct and video laryngoscopes. Results showed that with a direct laryngoscope, experts directed their gaze farther than novices during the laryngoscope insertion phase, with a significant difference in eye-to-gaze distance [exp…

BACKGROUND AND OBJECTIVE: Vocal cord leukoplakia (VCL) is a common precancerous lesion of the larynx associated with a high postoperative recurrence rate. Current prognostic assessments primarily rely on subjective clinical features, and quantitative prediction models based on deep information from laryngoscopic images remain scarce. This study aims to extract quantitative image features from laryngoscopic images to develop and validate a predictive model for 2-year postoperative recurrence of VCL and to identify key prognostic features.
METHODS: A total of 241 patients with VCL from two cent
