OBJECTIVE: Established middle-ear cleft cholesteatoma is associated with keratinous debris, which is likely to be an ideal medium for saprophytic fungal colonisation. This prospective case study aimed to explore the incidence and nature of fungal elements in cholesteatoma keratin samples obtained during primary mastoid surgery.
METHODS: All cases of middle-ear cleft cholesteatoma treated with primary mastoid surgery at the El-Sahel Teaching Hospital over a seven-month period were included. Keratinous debris obtained from the mastoid antrum was subjected to mycological analysis at the Departme…

OBJECTIVE: To observe the long-term outcome of ossiculoplasty using autogenous mastoid cortical bone in chronic otitis media in-patients.
METHODS: Sixty-one ears of 57 in-patients with chronic otitis media, with or without cholesteatoma, underwent type III tympanoplasty using autogenous mastoid cortical bone as the prosthetic material. Twenty-one ears were treated by canal wall down mastoidectomy and 40 ears by canal wall up mastoidectomy. The follow-up period was 3 to 6 years (average 4.2 years). Pure tone averages for thresholds at 0.5, 1, 2 and 3 kHz were calculated using standard conventi…

We describe the case of a 22-year-old woman who presented with a slowly growing osseous lesion of the mastoid cortex. On computed tomography, the lesion was found to involve the mastoid cortex, with which it demonstrated similar attenuation. The indications for treatment in this case were the patient's sensation of a mass effect, the encroachment of the mass onto the external auditory meatus, and a cosmetic deformity. The tumor was removed in its entirety via a postauricular approach. Findings on histopathologic examination were consistent with a compact osteoma. Mastoid osteomas are rare, ben…

OBJECTIVES: This study aimed to: understand the effect that high intensity noise associated with drilling (during otological surgery) has on … oustic emission changes and cutting burrs compared with diamond burrs.
CONCLUSION: Drilling during mastoid surgery poses a threat to hearing in the contralateral ear due to noise and vibration conducted transcranially.

We present a review of patients from Eastern Europe who have recently immigrated to Ireland with complicated otological disease. We carried out a retrospective chart review of these patients. These are a complicated cohort of 7 patients, 5 (71.4%) of whom had previous ear surgery, none had old notes and there was often a need for interpreters (3 or 42.8%) and challenging surgery. Follow up was also problematic with many of the patients. In summary this is a case series to highlight a relatively new group of patients to emphasize the need for pre-operative CT scans, facial nerve monitoring and

OBJECTIVES: The present study was undertaken to analyze the relationship between the method used for manipulation of the levator veli palatini muscle and the area of the mastoid air cells in patients with cleft palate.
DESIGN: Retrospective study.
PATIENTS: The subjects were 50 patients seen for surgical treatment of cleft palate.
INTERVENTIONS: Palatoplasty was carried out using the mucosal flap method in 25 patients and the mucoperiosteal flap method in 25 patients. In the mucosal flap method, the levator veli palatini muscle was overlapped and sutured, followed by posterior movement of t…

BACKGROUND: The mastoid is a rare site of nasopharyngeal carcinoma recurrence after radiotherapy, and no relevant reports are currently in the literature.
METHODS: Two case reports are presented describing patients with a history of nasopharyngeal carcinoma who received primary radical radiotherapy. The first was treated by conventional radiotherapy, using 2-dimensional techniques, while the second was delivered by intensity-modulated radiation therapy (IMRT).
RESULTS: The patients presented with progressive mastoid recurrence after radiotherapy at 12 and 16 months, respectively. Clinical pr…

PURPOSE: To explore the outcomes of lateral attic wall (LAW) reconstruction using autogenous mastoid cortical bone in cases with poorly developed zygomatic root cells and/or a low tegmen or with significant anterior or lateral sigmoid sinus in tympanomastoid surgery.
MATERIAL AND METHODS: Thirty-five ears with chronic suppurative otitis media, all of them either with poorly developed zygomatic root cells and/or a low tegmen (26/35 ears), or with significant anterior or lateral sigmoid sinus (9/35 ears), were included. LAWs were removed temporarily to offer exposure of the attic; after removal…

We report here the case of myiasis of the mastoid cells in a 50-year old Saudi farmer. Eight larvae of suspected Calliphorid fly were extracted from his right mastoid at examination in the clinic. The larvae almost ate into his brain, using their powerful screw-shaped mouth parts. It is the first report of Calliphorid larvae affecting the mastoid cells from Saudi Arabia. The epidemiological and clinical implications of this finding are discussed below.

OBJECTIVES/HYPOTHESIS: To determine the incidence of otitis media (OME) with effusion on histologic examination in temporal bones with mastoid cavities reduced by the fenestration procedure for otosclerosis.
STUDY DESIGN: Temporal bone histologic study.
METHODS: Light-microscopic examination of serially sectioned temporal bones.
RESULTS: The incidence of otitis media with effusion in temporal bones with prior fenestration operation was not any more frequent than the control group of temporal bones with surgically unaltered mastoid cavity.
CONCLUSIONS: There is no increased incidence of oti…

Temporal bone encephalocele (TBE) has become less common as the incidence of chronic mastoid infection and surgery for this condition has decreased. Due to its declining incidence, the diagnosis of TBE may be delayed and result in the development of serious complications such as cerebrospinal fluid leak, meningitis, epidural or subdural abscess. Six cases of large (>1 cm) TBE of diverse etiology are described. Two patients had suffered previous temporal bone fractures, two had had prior mastoidectomy, and two patients had long-standing chronic mastoiditis. Two patients had undergone prior unsu…

It is generally agreed that patients subjected to an intact canal wall mastoidectomy to excise mastoid/epitympanic cholesteatoma should undergo a "second-look" operation to rule out residual cholesteatoma. The morbidity of this second operation can be significantly reduced with endoscopic mastoidoscopy, when compared to the traditional postauricular (open) incision approach. Details, benefits, and limitations of endoscopic mastoidoscopy are discussed.

This study, merging data from two different clinics, was undertaken to evaluate the efficacy of endolymphatic mastoid shunt surgery using the Denver Inner Ear Shunt operation for Meniere's disease. This surgery was performed by the senior authors (C.G.J., J.R.E.D., and M.E.G.) between May 1984 and October 1986, after we attended the Colorado Otologic Research Center (CORC) instructional course. Sixty-six and thirty-four cases, respectively, were performed. Follow-up questionnaires were sent to this total of 100 patients. Fifty-six patients returned completed questionnaires, which formed the ba…

Thirty-three patients with acute purulent otitis media and mastoiditis caused by Gram-negative bacilli are presented. The main features of the disease include: predilection for young male infants, a high rate of complications that include sepsis, mastoiditis and osteomyelitis of the base of the skull. Patients that are diagnosed early respond well to drainage and ventilation of the infected middle ear combined with in vitro effective antibacterial therapy. Patients that receive prior inappropriate antibacterial therapy tend to have prolonged courses and require mastoid surgery. It is suggested…
