Until advances in retinal imaging over recent decades, detection of retinopathy of prematurity requiring treatment depended largely on indirect ophthalmoscopy by ophthalmologists with experience in examination of low birth weight, often fragile infants. Recent studies have added to our understanding of the pros and cons of using this technology to provide timely care for the increasing number of infants at risk for ROP blindness worldwide.

We examined four patients with the suspected diagnosis of optic disc drusen by scanning laser ophthalmoscopy (SLO) and fundus photography. In all four cases the autofluorescence that distinguishes optic disc drusen was observed with the SLO, but not with the fundus camera. Three of the four examined patients, children or adolescents, were spared further diagnostic procedures to clarify the underlying cause of the swollen optic disc.

Evaluation of the eye fundus after an extracapsular cataract extraction with posterior chamber lens implantation was performed in 50 patients 1 month after surgery and in 50 another patients 2 years after surgery. In the earlier period a very good inspection was possible in 88 p.c. of cases, 2 years after cataract extraction in 78 p.c. The usefulness of bilateral ophthalmoscopy was emphasized.

Several studies have validated the role of telemedicine as a new powerful screening and diagnostic tool for retinal disorders, such as diabetic retinopathy and retinopathy of prematurity. With regard to retinopathy of prematurity, bedside examination with binocular indirect ophthalmoscopy has been the gold standard technique for screening, yet with several limitations. Herein, we review the current evidence that supports the role of telemedicine for the screening of infants with retinopathy of prematurity.

A 25 years old male patient presented with long stature with disproportionately long limbs and digits (arachnodactyly). Examination of cardiovascular system showed mitral regurgitation and dilatation of aortic root. Slit lamp examination showed bilateral superotemporal dislocation of lens. Ophthalmoscopy revealed waxy pallor of optic disc, arteriolar attenuation and bone-spicule like pigments in the retina. The diagnosis of retinitis pigmentosa was confirmed by electroretinogram.

We report here a rare case of a combination of Oguchi's disease and congenital retinoschisis. A 36-year-old male patient presented with a decrease in vision in both eyes and night blindness. Indirect ophthalmoscopy revealed bilateral macular stellate striations and golden-gray discoloration of the retina outside the vascular arcades. This discoloration turned to a normal retina after complete adaptation to darkness (Mizuo-Nakamura phenomenon).

The anatomy of the chiasmal region and the visual symptoms due to pituitary tumors will be presented from the viewpoint of an ophthalmologist. The importance of perimetry in every patient with otherwise unexplained loss of vision is stressed. Other tests (swinging flash light test, colour vision, screening ophthalmoscopy) are mentioned as well. An ophthalmologist should participate in the diagnosis as well as in the follow-up after treatment of any case with pituitary tumors.

A central nervous system dysfunction of nontraumatic etiology was initially suspected in three cases of shaken baby syndrome. Blood contaminating the cerebrospinal fluid was attributed to a traumatic lumbar puncture. Failure to detect retinal hemorrhages contributed to the misdiagnosis. Emergency physicians must consider the diagnosis of shaken baby syndrome in a critically ill infant with bloody cerebrospinal fluid. Ophthalmoscopy should be done routinely in these patients.

A case of 37-year-old man with tuberous sclerosis is presented. Classic triad (Vogt) which consists of mental handicap, epilepsy and adenoma sebaceum around a nose and cheeks was observed. Ophthalmoscopy showed bilateral retinal astrocytomas with mulberrylike appearance. During 6 years follow-up small progression of ocular changes (findings) was noticed. Two cases of orbital myositis simulating infectious cellulitis were reported, in which the diagnosis was made by CT scans, biopsy and laboratory findings of thyroid function. In both cases, ophthalmoscopy revealed partial occlusion of the central retinal vein and choroidal folds due to posterior scleritis. Histopathology of a biopsy specimen revealed infiltration of lymphocytes, eosinophils and plasma cells and perivasculitis which suggested an allergic origin of this disease.
