The management of superficial candidiasis.

Superficial Candida infection includes several common conditions, most often related to some underlying local or systemic predisposition. Appropriate identification of the pathogen is important in the management of candidiasis as the result of differences in susceptibility among species and strains of Candida to different antifungal drugs. Treatment options are reviewed for oropharyngeal candidiasis, vaginal candidiasis, cutaneous candidiasis, paronychia and onychomycosis, and chronic mucocutaneous candidiasis. Because of the importance of predisposing conditions for candidiasis, adjunctive me…

Possibly the first case of onychomycosis by Fusarium lactis: Case presentation and literature review of onychomycosis by Fusarium species.

Fusarium species (spp.) is frequently found in soil and plant residues and on plant bodies in all climatic zones worldwide. Although there have been few reports of onychomycosis caused by Fusarium spp., it is characterized by drug sensitivity and other characteristics. Here, we report what may be the first case of onychomycosis caused by Fusarium lactis. We analyzed the mycology and characterized previously reported cases of onychomycosis caused by Fusarium spp. A 73-year-old otherwise healthy woman presented with discoloration and thickening of her right thumbnail with paronychia. Direct micr…

Retronychia of the toenails: a review with emphasis on pathogenesis, new diagnostic and management trends.

Retronychia is a condition, first described in 1999, characterized by the embedding of the proximal nail plate into the proximal nail fold a … , which could be challenging because retronychia can easily mimic other nail disorders with chronic paronychia. Since the first description, significant advances have been made regarding diagnostic criteria including ultrasonography. We performed a systematic review of the literature on retronychia from inception to April 2018 with an emphasis on the pathoge …

Skin Manifestations of Targeted Antineoplastic Therapy.

The management of oncology patients has changed significantly over recent years, with the development of new targeted anticancer therapies. Cutaneous adverse effects are among the most frequently observed toxicities with many targeted agents; their intensity can be dose-limiting or lead to the discontinuation of therapy. Tyrosine kinase inhibitors can cause maculopapular rash and hand-foot reaction, whereas papulopustular rash, paronychia, regulatory changes in hair, and dryness are caused by epidermal growth factor receptor inhibitors. SMO inhibitors, vismodegib and sonidegib, may result in m…

Nail involvement in autoimmune bullous disorders.

Autoimmune bullous disorders frequently cause nail abnormalities, particularly paronychia and onychomadesis. In pemphigus vulgaris (PV) nail abnormalities can even precede skin findings. Nail lesions often relapse just before generalized disease exacerbation or recurrence. Severe nail changes are often associated with extensive and severe disease. Fingernails are more commonly affected. A report in the literature associates hemorrhagic nail abnormalities with poor prognosis in patients with PV. Nail scarring and pterygium are a rare complication of bullous pemphigoid. Nail loss has been occasi…

Acute felon as a complication of systemic paclitaxel therapy: case report and review of the literature.

Breast cancer now affects 1 in 8 American women and the taxane agent paclitaxel (Taxol Bristol-Myers Squibb) is a major tool in the treatment of many such patients. Hand surgeons are therefore likely to encounter upper extremity complications related to the use of taxane therapy. We present an unusual case of a felon developing in a breast cancer patient on paclitaxel therapy with no antecedent history of trauma. Whereas onycholysis and subungual hemorrhage are reported complications of taxane therapy (Fig. 1), an acute felon with or without associated paronychia is an unusual and more aggress…

[Candida in dermatology].

Regarding Candida in dermatology, two pathogenetic pathways must be taken into account: 1. on infection of the skin 2. immunological reactions with skin alterations as a result of Candida infection or colonization in the mouth and/or intestine. Case reports describe typical situations of napkin dermatitis, intertriginous candidosis, the intrauterine Candida infection of the foetus, Candida granuloma, Candida folliculitis and Candida paronychia. In the second part results of investigations of patients suffering from psoriasis, atopic dermatitis and urticaria are presented. There were no differe…

[Zinc deficiency in artificial nutrition].

A 35-year-old patient was parenterally fed for a total of 170 days because of a twice reoperated gastric ulcer followed by a gastric-antrum small intestinal fistula. This lead to a complete acute zinc deficiency syndrome with pronounced acrodermatitis enteropathica-like skin changes in the face and the genital region, paronychia, flat blistering on the hands and feet, loss of hair, fingernail ridging (Beau's) and serious physical changes. A dramatic improvement and healing of all these skin changes, the antrum and small intestinal fistula as well as physical disorders occurred after intravenou…

A case of subungual squamous cell carcinoma treated with local resection and adjunctive ALA-PDT.

The early symptoms of subungual squamous cell carcinoma are often nonspecific, leading to frequent misdiagnosis as conditions such as paronychia or subungual warts. Treatment options for this malignancy remain relatively limited in clinical practice. This study presents the case of a 74-year-old male with subungual squamous cell carcinoma who achieved significant improvement following photodynamic therapy. The patient had experienced pain and exudation of the big toe for over six months. Initially diagnosed with paronychia, he was treated with oral clarithromycin tablets (0.5 g daily) for half…

Cutaneous and systemic manifestations of EGFR inhibitor therapy: A case of PRIDE syndrome.

EGFR inhibitors have revolutionized the treatment of advanced malignancies by targeting cancer cells specifically. The "PRIDE" complex encompasses a range of cutaneous adverse effects linked to EGFR inhibition therapy, including acneiform eruptions, xerosis, eczema, paronychia, and hair and nail changes. Cutaneous toxicities are among the most reported adverse events in this context. This report presents a case illustrating the dermatological side effects of EGFR inhibition therapy. The case involves a 73-year-old man with colon cancer undergoing (folinic acid, fluorouracil, irinotecan) + cetu…

Tackling Inflammatory and Infectious Nail Disorders in Children.

Nail disorders are common in the pediatric population and affect quality of life. We review inflammatory conditions (ie, nail psoriasis, nail lichen planus [NLP], trachyonychia, nail lichen striatus, nail atopic dermatitis [AD]) as well as infectious conditions (ie, nail disease due to hand, foot, and mouth disease [HFMD] or Kawasaki disease, onychomycosis, acute paronychia). Inflammatory and infectious nail conditions may involve the skin or occur in isolation. By highlighting the distinctive features and clinical presentations of these infectious and inflammatory nail disorders in children, …

Hand infections.

Many acute hand and upper extremity infections should be treated as surgical emergencies to avoid stiffness, contracture, pain, and amputation. Proper treatment requires understanding of anatomy and how this influences the behavior of certain infections, common offending organisms, antibiotic treatment, management of host factors, and surgical intervention. This article reviews the microbiology, antibiotic coverage, and surgical treatment of the most common infections in the hand: paronychia, felon, herpetic whitlow, flexor tenosynovitis, deep space infections, septic arthritis, bites from hum…

Drug-related nail disease.

Drug-induced nail abnormalities can present with a wide variety of clinical manifestations that are often dose-related and that disappear after drug withdrawal. Some nail changes are asymptomatic and only cause cosmetic problems, whereas others cause pain and discomfort and impair manual activities or deambulation. New side effects have been recently observed with different groups of anticancer agents. These include painful onycholysis and subungual abscesses caused by taxanes and anthracyclines (eg, doxorubicin) in addition to ingrown nails, paronychia, and pyogenic granuloma associated with …

Nonscarring inflammatory alopecia associated with the epidermal growth factor receptor inhibitor gefitinib.

Gefitinib (ZD1839, Iressa, AstraZeneca, Wilmington, Del) is a novel oral anticancer agent that acts by blocking the function of the epidermal growth factor receptor. Gefitinib and other drugs that block epidermal growth factor receptor function have been associated with a similar and interesting pattern of cutaneous adverse effects, including follicular acneiform eruptions, xerosis, desquamation, seborrheic dermatitis, chronic paronychia, and hair texture changes. These effects appear to reflect the significance of epidermal growth factor receptor signaling in the skin. Here we present a case …

Zinc-responsive dermatosis in a red wolf (canis rufus).

An 18-mo-old male red wolf (Canis rufus) presented with footpad hyperkeratosis, suppurative paronychia, distal limb pyoderma, and peripheral lymphadenopathy. Diet for the previous 11 mo consisted of a mixture of two commercially prepared dog foods with a mineral supplement containing primarily calcium. Culture of the draining tracts on the distal limbs yielded a mixed population of opportunistic bacteria. Histopathologic findings were consistent with a diagnosis of zinc deficiency. Medical therapy consisted of 15 mg/kg amoxicillin p.o. b.i.d. and 10 mg/kg zinc sulfate p.o. s.i.d. Calcium suppl…
