Optimizing the vault of collagen copolymer phakic intraocular lenses in eyes with keratoconus and myopia: comparison of 2 methods.

PURPOSE: To compare 2 methods for optimizing the final central vault of a collagen copolymer posterior chamber phakic intraocular lens (pIOL) (Visian Implantable Collamer Lens) in eyes with keratoconus and myopia. SETTING: Private practice, Beverly Hills, California, USA. DESIGN: Comparative interventional study. METHODS: The length of the pIOL to implant in keratoconic eyes with myopia was selected based on the white-to-white (WTW) distance or the sulcus-to-sulcus (STS) distance using the pIOL manufacturer's protocol. The final central vault distance was compared a minimum of 3 months post

Congenital glycosylation disorder: a novel presentation of coexisting anterior and posterior segment pathology and its implications in pediatric cataract management.

We report a case exhibiting the coexistence of anterior and posterior segment pathology in the same eye secondary to a congenital disorder of glycosylation resulting from a DPAGT1 gene mutation. This case details a novel gene mutation in a male infant found to have bilateral congenital cataracts, removed at 6 and 7 weeks of life, only to uncover bilateral retinal and optic atrophy. Our report highlights issues of surgical timing for syndrome-related pediatric cataracts, given the risks related to secondary glaucoma versus deprivation amblyopia, in an infant born with both cataracts and vision-

Traumatic Hyporeflective Micro-Elevations and Irregularity of the Ellipsoid Zone in Blunt Ocular Trauma.

PURPOSE: To investigate the prevalence, clinical course, and prognostic significance of hyporeflective micro-elevations and irregularity of the ellipsoid zone (MEEZ) in blunt ocular trauma. METHODS: Patients with blunt ocular trauma underwent standard ophthalmological examination and optical coherence tomography angiography. MEEZ was defined as small hyporeflective foci at the level of the ellipsoid and interdigitation zone, with disappearance of the thin hyporeflective band between these layers. The prevalence and spatial distribution of MEEZ, choriocapillaris perfusion, and associated findi

Comparison of concentration-time profiles of levobunolol and timolol in anterior and posterior ocular tissues of albino rabbits.

The potential effects of anti-glaucoma drugs, such as levobunolol and timolol, on blood flow in the posterior segment of the eye are of great interest in terms of changes in optic nerve head perfusion and prevention of visual field loss. These effects are related to the rate and extent of their absorption into the site of action. In this study, the concentrations of timolol and levobunolol in the aqueous humor, iris-ciliary body, vitreous humor, choroid-retina, and optic nerve were compared following instillation of a single drop of 0.5% ophthalmic solutions into albino rabbit eyes. Intraocular endoscopy: A review.

Optimal visualization is one of the most challenging aspects of performing vitreoretinal surgery. In situations where conventional microscopic techniques provide poor posterior visualization, the adjunctive skill set of endoscopic visualization may be needed. This allows for by-passing the opaque anterior segment media and getting access to the posterior segment pathology. Endoscopic vitrectomy is a useful and unique adjunct to microincision vitreoretinal surgery. The optical set-up of endoscopy allows for clinical approaches that are impossible with regular microscope viewing systems. These i

Quantitative ophthalmic posterior segment optical coherence tomography angiography and systemic conditions.

Optical coherence tomography angiography (OCTA) noninvasively and quantitatively images the microvasculature of the eye's posterior segment in 3 dimensions. OCTA has known utility in the diagnosis and management of certain ocular conditions and is now being used to assess systemic conditions, including respiratory, cardiovascular, renal, obstetric, neurologic, rheumatologic, and genetic conditions, among others. OCTA may improve our understanding of the pathophysiology of systemic conditions and identify biomarkers useful in their diagnosis, prognosis, and management. We detail known associati

Correction of irregular and induced regular corneal astigmatism with toric IOL after posterior segment surgery: a case series.

BACKGROUND: Toric intraocular lens (IOL) implantation can be an effective method for correcting corneal astigmatism in patients with vitreoretinal diseases and cataract. Our purpose is to report the outcome of toric IOL implantation in two cases - a patient with scleral-buckle-induced regular corneal astigmatism and a patient with keratoconus following pars plana vitrectomy. As far as we are aware, there are no reported cases of toric IOL implantation in a vitrectomized eye with keratoconus nor of toric IOL implantation in patients with scleral-buckle-induced regular corneal astigmatism. Oculomotor nerve palsy secondary to aberrant posterior cerebral artery.

The authors report a rare case of an oculomotor nerve palsy attributed to mass effect from an aberrant posterior cerebral artery. The patient presented with complete ptosis, mydriasis and ophthalmoplegia of the affected eye. MRI demonstrated distortion of the oculomotor nerve as it traversed inferior to the non-aneurysmal P1 segment of an aberrant posterior cerebral artery. The oculomotor nerve palsy resolved spontaneously.

Optimising the surgical outcome in a case of post-traumatic cataract using ultrasound biomicroscopy.

A 12-year-old boy was brought with the chief complaint of diminution of vision in his left eye for the past 1 month. History revealed a blunt trauma to his left eye with a wooden stick 6 weeks ago.Visual acuity was 20/20 and hand movement close to face in right and left eye, respectively, without any relative afferent pupillarydefect. Slit-lamp examination showed a total cataractous lens with signs suggestive of suspected posterior capsular (PC) defect. The PC defect was screened initially using B-scan ultrasound of the posterior segment, followed by confirmation of the same using ultrasound
