Pulse oximetry may produce skin damage after the administration of photosensitizing chemotherapeutic drugs. Surgery must often be performed in near darkness during photodynamic therapy. Limiting the duration of pulse oximetry and rotating sites allowed successful use of pulse oximetry in a long anesthetic during which verteporphin was administered.

PURPOSE: We describe pharyngeal oximetry with the laryngeal mask airway in two patients with low perfusion states where finger oximetry failed.
CLINICAL FEATURES: One patient was a 47-yr-old man with septic shock and the other a 64-yr-old man with multiorgan failure. In both patients, a # 4 laryngeal mask airway with a pediatric pulse oximeter probe was inserted behind the tracheal tube. To study the peripheral blood flow in patients with panic attacks during "Stress" conditions, nine patients with history of panic attacks were subjected to mental stress of arithmetic calculation and room-air voluntary hyperventilation. Finger Pulse Volume (FPV) was significantly altered during the period following mental activity in untreated patients when compared to controls and patients under treatment. There was also an increase in FPV in untreated patients at rest and during recovery after mental stress when compared to the treated patients.

We report our experience with 199 patients requiring admission with a diagnosis of croup over an 18 month period. The value of using pulse oximetry to monitor these children was critically examined. Twenty-nine patients with clinically significant stridor were monitored for an average of 12 hours. The degree of systolic hypotension causing failure and recovery were tested simultaneously with three oximeters (CSI 504US, Nellcor N-200, and Ohmeda 3740) in nine normal male volunteers. Perfusion of the right hand was slowly reduced and restored by 1) elevation of the hand plus systemic hypotension with nitroprusside if needed (EL); 2) clamp compression of the brachial artery (CL); 3) brachial cuff inflation (CU); and 4) intraarterial norepinephrine (NE). With EL, pulse pressure was normal whereas right radial arterial systolic pressure (SP) was 25.3 +/- 12.4 mmHg at failure and 34.1 +/- 13.

Five patients with severe dysthyroid optic neuropathy were treated with intravenous methylprednisolone (1 g daily for 3 consecutive days). Before administration, visual acuity of the more severely affected eyes of each patient was counting fingers at 5 feet, 8/200, 20/400, 20/200, and 20/80.
