Animal studies have shown that toluene can cause hearing loss and can exacerbate the effects of noise by inhibiting the middle ear acoustic reflex. In this investigation, carried out in Long-Evans rats, the tensor tympani tendon was cutoff and the stapedius muscle was electrocoagulated in one or both middle ears. Rat hearing was evaluated by measuring cubic distortion otoacoustic emissions (2f1-f2; f1 = 8000 Hz; f2 = 9600 Hz; f1/f2 = 1.2) prior to, during, and after activation of the olivocochlear (OC) reflex. A band noise centered at 4 kHz was used as suppressor noise. It was delivered contra

PURPOSE OF REVIEW: Although cough is one of the defining symptoms of asthma, wheeze, chest tightness and breathlessness have generally received more attention. The impact of coughing for patients may be more important than currently appreciated and has been rated more troublesome than wheeze, chest tightness or disturbance of sleep.
RECENT FINDINGS: Studies objectively measuring cough in asthma reveal that cough frequencies vary from those similar to healthy volunteers to cough counts comparable to patients presenting with chronic cough, but the potential triggers for cough in asthma are poor

INTRODUCTION: The aim of this study was to determine the role ofintra-wall nervous and neurohormonal system in the control of airway transport of sodium and chloride ions, as well as to identify regulating mechanisms having an effect on the permanent electric potential of airway tissue, named the transepithelial electric potential (PD) and on reversible changes of this potential (dPD). Using amiloride, a sodium ion blocker, and bumetanide, a blocker of the chloride ion co-transport system, the importance oftransepithelial sodium and chloride ion transport for support of the cough reflex was de

Sacral reflex testing is a very specific one. Coming from sensory receptors afferent neuronal pudendal passway enters the spinal cord S2 to S4 via spinal dorsal roots and transmits the bioelectrical activity to a complex interneuronal circuitry. Information is processed at this level and dispached into spinal motoneurones for innervating the pelviperineal muscles according to their specific radicular origins. Selective staged sacral reflexes contribute to locate the site of compression. Sacral reflex configuration was based on clinical and electrophysiological findings which is actually confir

OBJECTIVES: Activation of the trigeminovascular system and sensitisation of brainstem trigeminal nuclei are thought to play an important role in migraine. The aim of this study was to investigate the blink reflex and its habituation in patients with "chronic migraine".
METHODS: We studied 35 patients suffering from "chronic migraine" (IHS classification criteria) outside and during a spontaneous attack, and 35 control subjects. An EMG device with a specific habituation test program was used to elicit and record blink reflex responses and to randomly repeat stimulations at different time inter

The aim of the study was to analyse the diagnostic value of blink reflex (BR) in motor neuron disease (MND). We studied 25 patients with MND including amyotrophic lateral sclerosis (ALS) in 18 patients, primary muscular atrophy (PMA) in 4 patients, and primary lateral sclerosis (PLS) in 3 patients and 12 healthy volunteers. The blink reflex was obtained in typical way. In ALS group, statistically significant increase in latency and decreased amplitude of R2 responses were found compared to the control group (p < 0.05), whereas BR in patients with PLS and PMA was within normal limits. The prese

A 72-year-old man was referred for further assessment of a chronic cough. He noticed an association between the episodes of coughing and palpitations. Electrocardiography (ECG) revealed normal sinus rhythm and sporadic unifocal ventricular premature contractions (VPCs). Each cough was preceded by a premature beat. Continuous wave Doppler echocardiography revealed a VPC-induced transient increase in the pulmonary artery blood flow. He was successfully treated for VPCs with oral disopyramide, resulting in subsidence of both the coughing and palpitations. We suspect that the VPC-induced hemodynam

OBJECTIVE: To investigate the underlying pathophysiology of primary lateral sclerosis (PLS) regarding possible changes in blink reflex (BR) circuit.
METHODS: BR was studied in 10 patients in whom diagnosis of PLS was established by comparing them to two control groups: ten patients with amyotrophic lateral sclerosis (ALS) and 30 healthy volunteers. All patients were followed up annually for a 5 year period. A typical evoked potential study of the BR was employed.
RESULTS: Significantly lower values of R(2) and R(2)' amplitude were found both in the PLS and ALS groups compared with the health

Concepts related to the pathophysiology of reflex sympathetic dystrophy syndrome (RSDS) are changing. Although sympathetic influences are still viewed as the most likely mechanism underlying the development and/or perpetuation of RSDS, these influences are no longer ascribed to an increase in sympathetic tone. Rather, the most likely mechanism may be increased sensitivity to catecholamines due to sympathetic denervation with an increase in the number and/or sensitivity of peripheral axonal adrenoceptors. Several other pathophysiological mechanisms have been suggested, including neurogenic infl

This case report, with videotape, focuses on reflex epileptic seizures of a girl who started to have seizures, provoked by tactile stimuli, at the age of twelve months. They were described as myoclonic jerks predominantly in the upper part of the body and the arms. During video/EEG monitoring at the age of fifteen months, seizures provoked by the same tactile stimuli were recorded. For clinical and electroencephalographic reasons, these are best described as absence seizures (persistence of minimal myoclonic jerking). To our knowledge, such seizures at this age have not been documented before.

During the course of our studies into the control of fluid extravasation from the splenic vasculature, we found that intrasplenic inhibition of NO biosynthesis caused an increase in systemic blood pressure. The present experiments were designed to investigate the mechanisms underlying this novel observation. There was an increase in mean arterial pressure when the nonspecific NO inhibitor N(G)-monomethyl-L-arginine (L-NMMA) was infused via the splenic artery but not when the same dose was administered systemically. Conversely, blood pressure decreased after intrasplenic but not systemic admini

Reflex sympathetic dystrophy is a well-recognized syndrome in human patients following injury to an extremity. The syndrome may include hyperesthesia and autonomic changes. The autonomic changes are initial vasodilatation followed by vasoconstriction (e.g., edema followed by cyanosis, and cool skin); hyper- or hypohydrosis; atrophic changes in the skin, subcutis, and muscles; and osteoporosis. Early treatment with a short course of steroids and infiltration of the painful site with lidocaine may alleviate symptoms. If that fails, sympathetic ganglionic block with lidocaine (and possibly steroi

OBJECTIVE: Reflex anoxic cerebral crises are due to depression of nerve function caused by a vagotonic state or vagal hypersensitivity. In this paper we propose to review the physiopathology, clinical features and diagnostic procedures of these crises.
DEVELOPMENT: There are three types of reflex anoxic cerebral crises: asphyxiating anoxic crises, ischemic anoxic crises and asphyxiating-ischemic anoxic crises. They are caused by sudden activation of the so-called syncopal reflex. They are characterized by diverse clinical manifestations and may often be mistaken for epileptic crises. They are

Reflex sympathetic dystrophy (RSD) syndrome has been recognized clinically for many years. It is most often initiated by trauma to a nerve, neural plexus, or soft tissue. Diagnostic criteria are the presence of regional pain and other sensory changes following a noxious event. The pain is associated with changes in skin colour, skin temperature, abnormal sweating, oedema, and sometimes motor abnormalities. The clinical course is commonly divided into three stages: first (acute or hyperaemic), second (dystrophic or ischaemic), and third (atrophic) stage. The diagnosis is primarily clinical, but

The generators of the audiogenic startle reflex (ASR) are located in the bulbopontine reticular formation. We studied the influence of acute vascular supratentorial lesions on ASR. Ten patients with hemiplegia due to hemispheric cerebral infarct were studied within 5 days of stroke onset. ASR and magnetic cortical stimulation were performed the same day. A muscle response to magnetic stimulation was not elicited over the plegic side in any patient. In four of seven patients, ASR was enhanced over the plegic side. We suggest that enhanced ASR is due to the loss of a predominantly inhibitory hem

A patient with post-hypoxic brainstem reflex myoclonus was examined. In spontaneous jerks electromyographic activity started in the sternocleidomastoid and spread down the spinal cord bilaterally. A similar jerk was elicited by somaesthetic or acoustic stimulation. The patient's blink reflex suggested bilateral pontine lesions at least. Thus, the pattern of muscle activation pointed to a brainstem origin of the myoclonus. Spinal conduction studies conducted during the jerks suggested that the myoclonus signal facilitated a path identical to the spinobulbospinal reflex demonstrated in animals.

OBJECTIVE: To demonstrate the feasibility of intraoperative monitoring of the bulbocavernosus reflex (BCR) as an indicator of the functional integrity of sacral nervous structures to aid in preventing their intraoperative injury.
METHODS: Intraoperative BCR was elicited by electrical stimulation of the dorsal penile/clitoral nerve in 119 patients anesthetized with propofol, fentanyl, and nitrous oxide, with short-acting relaxant. Thirty-eight patients underwent surgery without risk, whereas 81 underwent surgery with risk of damage to sacral structures. Different patterns of stimuli were appli

The ciliospinal reflex response is mainly mediated by second- and third-order sympathetic nerves to the dilatator muscle of the iris. As the pupillary response to various pharmacological agents indicates a sympathetic dysfunction in patients with cluster headache, the ciliospinal reflex was studied in 25 patients. Five of these patients with cluster headache exhibited a Horner-like syndrome (miosis, ptosis) on the symptomatic side. The pupillary responses to phenylephrine and tyramine showed that the Horner-like syndrome was due to postganglionic sympathetic nerve dysfunction. Their ciliospina
