An important domain of artificial intelligence is deep learning, which comprises computed vision tasks used for recognizing complex patterns in orthopaedic imaging, thus automating the identification of pathology. Purported benefits include an expedited clinical workflow; improved performance and consistency in diagnostic tasks; decreased time allocation burden; augmentation of diagnostic performance, decreased inter-reader discrepancies in measurements and diagnosis as a function of reducing subjectivity in the setting of differences in imaging quality, resolution, penetrance, or orientation;

Is "killing the biceps" during rotator cuff repair a capital crime or a lawful act? One of the most passionately debated topics in shoulder surgery is what to do with the biceps during rotator cuff repair: save it, tenotomize it, or perform tenodesis. Results of repair are not very successful, and given that repair of massive rotator cuff tears shows a 40% to 57% failure rate, there is renewed interest in sparing the biceps tendon as a humeral head depressor-or so that it may be used as a local graft for revision rotator cuff repair. The literature regarding tenodesis versus biceps sparing dur

For irreparable posterior superior rotator cuff tears (IPSRCTs), techniques other than reverse shoulder arthroplasty and tendon transfer are still under research; among these, reusing the long head of the biceps (LHB) is noteworthy. Various techniques of LHB reuse have been reported and can be divided into 3 biomechanical categories: restoration of the force chain of the rotator cuff, augmentation of the superior capsule, and combined force-chain restoration and superior capsule augmentation, among which in situ biceps tenodesis may be the simplest. LHB reuse for IPSRCTs is always combined wit

The tension-free repair of retracted supraspinatus tears with the open muscle and tendon advancement technique first described by Debeyre in 1965 gave satisfactory clinical results. The purpose of this anatomical study was to test the feasibility of an arthroscopic supraspinatus advancement technique. A total of 10 cadaveric shoulders were operated. We assessed the feasibility, measured the slide distance, and recorded the position of the suprascapular nerve (SSN) for each shoulder. Reattachment of the tendon to its native footprint was achieved in all cases. The mean slide distance was 38.8 m

BACKGROUND: Post-operative shoulder stiffness (POSS) is one of the most frequent complications after arthroscopic rotator cuff repair (ARCR). Factors specifying clinical prediction models for the occurrence of POSS should rely on the literature and expert assessment. Our objective was to map prognostic factors for the occurrence of POSS in patients after an ARCR.
METHODS: Longitudinal studies of ARCR reporting prognostic factors for the occurrence of POSS with an endpoint of at least 6 months were included. We systematically searched Embase, Medline, and Scopus for articles published between

BACKGROUND: Despite advances in surgical techniques, the use of maximal repair to treat large or massive rotator cuff tears results in a high retear rate postoperatively. Currently, no randomized controlled trials have compared the outcomes of maximal repair with interposition dermal allograft bridging reconstruction.
HYPOTHESIS: We hypothesized that large or massive rotator cuff tendon tears reconstructed using bridging dermal allograft would have better clinical outcomes 2 years postoperatively, as measured using the Western Ontario Rotator Cuff (WORC) index, than would those receiving the

PURPOSE: The optimal technique for arthroscopic rotator cuff repair is still controversial. The aim of this study was to compare modified arthroscopic double-pulley suture-bridge (DPSB) technique with medial knot tying to those without tying, considering clinical and radiological outcomes.
METHODS: This study included 292 patients with large full-thickness rotator cuff tears treated with modified DPSB technique. The patients were divided into 158 cases with medial knot tying (knot-tying group) and 134 without tying (knotless group). At follow-up, clinical outcome was assessed by the Constant

BACKGROUND: The partial articular supraspinatus tendon avulsion (PASTA) lesion repair remains a topic of debate. We have performed in situ repair of PASTA lesions using a potentially viable threading lasso fixation technique. This retrospective case series aimed to evaluate the clinical outcomes of PASTA lesion repair using threading lasso fixation. To the best of our knowledge, this is the first study to review this technique and its outcomes in terms of pain and upper extremity function.
METHODS: Twenty-five patients with PASTA lesions who were treated with threading lasso fixation were rev

Background and Objectives: Superior capsular reconstruction (SCR) with the use of a fascia lata autograft or a dermal allograft is an established treatment in treating irreparable rotator cuff (RC) tears. The long head of the biceps tendon (LHBT) has been recently proposed as an alternative graft for SCR. The purpose of this study was to present the surgical techniques and clinical studies utilizing the LHBT for SCR. Material and Methods: Medline, Scopus, and the Cochrane library were searched for relevant studies up to December 2020. The primary outcomes were pain intensity improvement and th

BACKGROUND: The microscopic pattern of inflammatory mediators associated with rotator cuff pathology is well documented; however, little is known regarding the contemporary presence of macroscopic inflammatory joint involvement. Our aim was to investigate shoulder synovitis in a large group of patients with different sized rotator cuff tears (RCTs) and to correlate the degree of macroscopic inflammatory changes of the glenohumeral joint with RCT severity.
MATERIALS AND METHODS: A total of 296 consecutive patients (169 F, 127 M; mean age ± standard deviation: 60.75 ± 7.91) submitted to arthros

Shoulder rotator cuff repair results in significantly improved outcomes compared with conservative treatment, but some repairs result in retear and, worst of all, enlarged retears (i.e., tears larger after surgery than primarily). Elevated serum total cholesterol and low-density lipoprotein levels and fatty infiltration of the infraspinatus are significantly related to symptomatic failed rotator cuff repair. Hypertension could also be a risk factor. In such cases, nonoperative treatment, reverse shoulder prosthesis (in older patients), or alternative joint-preserving procedures (superior capsu

Given the high prevalence of rotator cuff tears and their socioeconomic impact, surgeons and researchers have tried to understand their etiology and pathomechanism for almost hundred years. Articles about tendon degeneration with increasing age dominate the literature, and numerous factors contributing to tendon degeneration have been identified. One of them, the lateral extension of the acromion, as quantitated using the acromion index or the critical shoulder angle, has become very popular in the last few years. Other big tendons in the human body, such as the distal biceps tendon, the Achil

Shoulder SLAP repair anchors have traditionally been placed through a posterolateral trans-rotator cuff "Port of Wilmington" portal. Alternative anterolateral portals have been proposed to place superior glenoid anchors that seemingly avoid traversing the rotator cuff via the rotator interval but put the suprascapular nerve at risk. A recent cadaveric study demonstrated that the traditional posterolateral portal safely traversed the infraspinatus muscle belly in all specimens, which was in contrast to the anterolateral portal that traversed the supraspinatus tendon in all specimens. On physical examination of the shoulder, and specifically the rotator cuff, the full and empty can tests associated with weakness are the tests of choice for supraspinatus tears. However, under the huge administrative burden and in the context of the primacy of imaging mandated by regulatory agencies and/or payor, medical insurance coverage for rotator cuff repairs requires other physical examination tests and many other criteria. Moreover, some propose to reduce or even eliminate regulatory requirements mandating documentation of a history and a physical examination. It could be prudent to le

For the treatment of large to massive rotator cuff repairs, not all partial repairs are equivalent. The technique of rotator cuff repair in large to massive rotator cuff tears matters. By using mobilization techniques to mobilize the rotator cuff to obtain anatomic or near anatomic coverage, double-row techniques to increase footprint coverage, and medial mobilization to decrease tension on the repair, exposure of the humeral head can be minimized, which can improve outcomes.

Rotator cuff injuries are among the most common musculoskeletal conditions that affect shoulder function and can ultimately impact quality of life. While physical therapy is essential in the care of rotator cuff injuries, the ideal dose of therapeutic exercises continues to be a significant clinical dilemma because of the generalized nature of rehabilitation protocols. This pilot study proposes a machine learning approach to personalize rehabilitation using surface electromyography (sEMG) data collected from eight healthy individuals by testing four key shoulder movements: scaption, internal r…
