Antiseptic urinary catheterization and maintenance of the bladder catheter.

Catheter-related urinary tract infections are very common both in the community and in the acute hospital care setting, particularly in the critical care environment. In order to minimize this problem, it is essential to adopt a correct approach from insertion to withdrawal of the urinary catheter, and always perform proper antisepsis. There is too much information on antisepsis measures with no consistent results. The present article reviews the existing scientific evidence on the subject and establishes recommendations based on the evidence, in order to optimize outcomes. The entire process

Gas surrounding the urinary bladder in emphysematous cystitis.

We report a rare case of emphysematous cystitis in a 66-year-old woman with a history of diabetes mellitus. The predisposition of diabetes mellitus and infection of gas-forming bacteria is considered to precede the manifestation of emphysematous cystitis. The present recommended diagnosis test is computed tomography, which have definite value in the evaluation of gas accumulation in bladder wall, or na air-fluid level in bladder.

Bilateral Hydroureteronephrosis with a Hypertrophied, Trabeculated Urinary Bladder.

Bilateral hydroureteronephrosis involves the dilatation of the renal pelvis, calyces and ureter; it develops secondary to urinary tract obstruction and leads to a build-up of back pressure in the urinary tract, and it may lead to impairment of renal function and ultimately culminate in renal failure. Although clinically silent in most cases, it can be diagnosed as an incidental finding during evaluation of an unrelated cause. In a minority of patients, it presents with signs and symptoms. Renal calculus is the most common cause, but there are multiple non-calculus aetiologies, and they depend

Glass ampoule in urinary bladder as a foreign body.

A wide range of objects have been inserted into the urinary bladder and urethra. Most foreign bodies in the lower genitourinary tract are self-inserted via the urethra as the result of erotic impulses, psychometric problems, sexual curiosity, or sexual practice while intoxicated. Here we report the case of a 32-year-old female with alleged history of insertion of glass ampoule in the urethra by the husband in eroticism which was removed with open surgery. This is the first case of its kind as per the literature available.

Electric wire in the urinary bladder in an adolescent boy.

Self-inserted foreign bodies in the bladder is an uncommon entity in males due to the lengthy urethra. Here we present a case of insulated electric wire present in the urinary bladder of a 15-year-old male inserted for sexual arousal. Diagnosis was made by history and radiological investigation and it was removed by cystoscope with an uneventful recovery.

[Bladder augmentation and urinary diversion in children and adolescents].

BACKGROUND: In children and adolescents, the indication for continent urinary diversion or bladder augmentation is rare. Today, for most patients with a neurogenic bladder, conservative treatment (clean intermittent catheterization [CIC] and pharmacotherapy) is the method of choice, while for patients with bladder exstrophy-epispadias complex (BEEC), primary reconstruction is recommended. Only after failure of conservative treatment or primary reconstruction should bladder augmentation or urinary diversion be considered. Other rare indications include patients with malignant tumor involving th

Müllerianosis of the urinary bladder: a rare tumorlike lesion.

Müllerianosis was first described as a rare entity consisting of an admixture of cervical, tubaric, or endometrial epithelium within the lamina propria and muscularis propria of the urinary bladder. This lesion occurs mainly in the dome or posterior wall of the urinary bladder in women of fertile age. Its clinical presentation is characterized by hematuria, pelvic pain, and dysuria, nonspecific symptoms that are related to the responsiveness of müllerian glands to hormonal stimuli. The major interest of müllerianosis resides in its similarity, from clinical, cytologic, and histologic viewpoint…

A Rare Case of Primary osteosarcoma of urinary bladder.

Extraskeletal osteosarcoma is a malignant mesenchymal soft tissue tumor without attachment to the bone, and is able to produce osteoid or cartilaginous matrix. This entity accounts for 1% of all soft tissue sarcomas. Thus far, less than 35 cases of bladder osteosarcomas have been reported in the literature. These tumors are associated with very poor prognosis. We report a case of primary osteosarcoma of the urinary bladder presenting with intermittent hematuria, dysuria, and right flank pain. Contrast-enhanced computed tomography scan of the abdomen, pelvis, and chest revealed a bladder mass a…

[Foreign bodies in urinary bladder: a clinical case].

OBJECTIVES: A pencil in the urinary bladder is an unusual problem for urologists. We present a case in a 44-year-old female with an eyeliner pencil self-introduced into the urethra. MATERIALS AND METHODS: The patient was referred to us with a recent history of "ingestion of foreign body". The clinical presentation, radiologic data, and treatment were reported. RESULTS: The patient had a pencil placed sideways in the urinary bladder, with focal perforation of the left wall. A cystoscopy was performed and the pencil was removed at the same time by using grasping forceps and hydraulic overdiste…

Villous adenoma of the urinary bladder: a case report.

Villous adenomas of the urinary tract are rare, in contrast to urothelial neoplasms. Most reports were scattered individual cases. Only two case series of this entity have been published. The histopathology is identical to that of the much more common villous adenoma of the gastrointestinal tract. The authors reported a case of urinary bladder villous adenoma in a 41-year-old Thai patient who complained of hematuria for one day without any other symptom. Cystoscopic examination revealed a papillary growth at the bladder neck associated with marked degree of bullous edema and bilateral mild hyd…

Conservative management of spontaneous rupture of the urinary bladder.

We report a very rare case of intra-peritoneal spontaneous rupture of the urinary bladder which was successfully managed conservatively in a 77-year-old woman, who presented with lower abdominal pain, acute urinary retention and pelvic organ prolapse (POP). Computed tomography raised the suspicion of bladder rupture which was later confirmed by cystoscopy and cystogram. Following 8 weeks of urinary catheterisation, the bladder healed spontaneously. A subsequent sub-total abdominal hysterectomy and sacrocervicopexy were performed to treat the POP. The management of bladder rupture is discussed.

Large cell undifferentiated carcinoma of the urinary bladder.

We present the clinicopathological features of eight cases of large cell undifferentiated bladder carcinoma not otherwise specified (LCUBC). LCUBC was characterised by sheets of large polygonal or round cells with moderate to abundant cytoplasm and distinct cell borders. The LCUBC component varied from 90 to 100% of the tumour specimen with five cases showing pure LCUBC. The architectural pattern of the tumour varied from infiltrating tumour to solid expansile nests with focal (<5%) discohesive growth pattern in two cases. Immunohistochemical staining demonstrated that LCUBC cases were positiv

Micropapillary carcinoma of the urinary bladder: a case report.

BACKGROUND: Micropapillary carcinoma is a rare but unique histologic variant of urothelial carcinoma with an aggressive clinical course. In spite of well-defined clinicopathologic characteristics, the cytomorphologic features in urine specimens have been only rarely described. CASE: A 78-year-old man presented with gross hematuria, showing a large mass on cystoscopy. Urine cytology contained tight clusters of atypical cells, originally interpreted as high-grade urothelial carcinoma or adenocarcinoma. Final diagnosis was made by histology and immunohistochemical study. On retrospective cytolog
