A late-onset arrhythmogenic right ventricular cardiomyopathy unveiled by supraventricular tachycardia: Case report.

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a rare myocardial genetic disease that leads to heart failure and sudden cardiac death. The diagnosis of ARVC remains challenging given its variable presentation. A 66-year-old woman was admitted to our cardiac intensive care unit for the management of a left heart failure flare-up with an atrial flutter, which remains an atypical mode of the revelation of arrhythmogenic right ventricular cardiomyopathy (ARVC) retained based the association of genetic predisposition, the presence of repolarization abnormalities on electrocardiogram (EKG

[ECG changes in aortic valve defects].

ECG is nowadays no longer the dominant way of diagnosing aortic valve diseases. The basis of accurate diagnosis of these diseases is clinical examination, ECHO and catheterization of the heart, which is essential in the great majority of valve diseases, in particular to rule out coronary changes. ECG is however important for the primary evaluation of left ventricular hypertrophy, left ventricular overburdening, for detection of enlargement or overloading of the left atrium. It is irreplaceable for evaluation of impaired rhythm, or impaired conduction of the impulse. Thus we can prove atrial fi

Mechanisms determining the ventricular rate in wolff-parkinson-white arrhythmias.

In patients with WPW syndrome the maximal ventricular rate attained during ectopic rapid supraventricular thythms depended on the type of arrhythmia as well as on the physiological properties of the AP. During reciprocating tachycardias the impulse is almost invariably conducted to the ventricles through the AV node. Therefore, the maxiaml ventricular rate is a function of the AV nodal ERP. On the other hand, when atrial flutter or atrial fibrillation were present the ventricular rate could be moderately elevated (when the ERP of the AP was longer than that of the AV node) or very rapid where

[Echocardiography as a method for the determination of the severity of aortic insufficiency].

4 groups of patients with different degrees of aortic regurgitation (mild n = 7, moderate n = 3, severe n = 21, and aortic regurgitation combined with organic mitral stenosis n = 5) were examined by means of echocardiography and compared with a control group of 10 healthy subjects. The purpose was to determine what information echocardiography may provide in regard to the severity of aortic regurgitation and whether patients with organic mitral stenosis can be distinguished from patients with Austin Flint murmur. Patients with mild or moderate aortic regurgitation could not be clearly distingu

[Arrhythmias during pregnancy].

Cardiovascular emergencies are rare during pregnancy with an incidence of 0,2-4,0%. Emergencies include arrhythmias, acute coronary syndrome … ts. However, life-threatening ventricular tachyarrhythmias (sustained ventricular tachycardia [VT], ventricular flutter [VFlt], ventricular fibrillation [VF]) were observed less frequently. Electrical DC-cardioversion is necessary in all pregnant women who are in a hemodynamically unstable state and have a life-threatening ventricular tachyarrhythmias. In hemodynamic …

Termination of macroreentrant atrial arrhythmias by pacing stimuli without global propagation.

BACKGROUND: Electrical stimulation during ventricular tachycardia resulting in tachycardia termination without global propagation (TWGP) is a well-recognized phenomenon. However, there is a paucity of literature showing a similar phenomenon in atrial arrhythmias. OBJECTIVE: The purpose of the study was to evaluate the significance of TWGP in atrial arrhythmias. METHODS: Electrophysiological studies performed from 2000 to 2019 at Methodist Hospital, Indiana University were reviewed retrospectively. Thirty-four patients were identified in whom stimulation during atrial tachycardia/flutter resu

Clinical relevance of atrial fibrillation/flutter, stroke, pacemaker implant, and heart failure in Emery-Dreifuss muscular dystrophy: a long-term longitudinal study.

BACKGROUND AND PURPOSE: Emery-Dreifuss muscular dystrophy (EDMD) is a rare inherited disorder associated with cardiac involvement. We investigated the spectrum and relevance of the cardiac manifestations of EDMD, focusing on bradyarrhythmias and tachyarrhythmias (including atrial fibrillation/flutter), embolic stroke, and heart failure. METHODS AND RESULTS: Eighteen patients (age 42.8+/-19.6 years) with genetically confirmed X-linked (n=10, including 3 carriers) or autosomal dominant (n=8) EDMD were followed for a period ranging from 1 to 30 years in a research center for neuromuscular diseas

Focal Constrictive Pericarditis, an Unusual Cause for Biventricular Failure.

A 34 year old male who had undergone surgical closure of a ventricular septal defect at the age of 4 years, after a long asymptomatic period, presented with heart failure and was diagnosed with atrial flutter and biventricular dysfunction. Chest X-ray showed a band-like calcification in the atrioventricular groove. Echocardiography revealed dilated atria, focal calcification with constriction at the atrioventricular groove, and biventricular systolic dysfunction. Focal constrictive pericarditis occurring in post-cardiac surgery patients is very rare. Here we report a case of focal variant of c

When Virchow's Triad Goes on Overdrive: Catastrophic Multi-System Thrombosis in the Setting of New-Onset Heart Failure in a Previously Healthy Man.

Catastrophic multi-territory thromboembolism is an uncommon but life-threatening presentation that often signals an underlying hypercoagulable state or severe cardiac dysfunction. When all three components of Virchow's triad converge simultaneously in a previously healthy individual, the clinical consequences can be devastating and the diagnostic workup both urgent and complex. We report a 58-year-old man with no prior medical history who presented with progressive dyspnea and was found to have bilateral pulmonary emboli, large mobile thrombi in both the left atrium and left ventricle, and ext

Efficacy and safety of intravenous and oral propafenone in pediatric cardiac dysrhythmias.

Propafenone was administered to 58 patients with a mean age of 3.2 years (range 0.1 to 16). Mean intravenous dose was 1.2 mg/kg body weight (range 0.3 to 1.5 mg). The final mean oral maintenance dose was 308 mg/m2 body surface area (range 200 to 600 mg/m2, 16.8 mg/kg body weight). After intravenous application, propafenone was effective in 21 of 36 patients; atrial flutter was converted in 1 of 5 patients, and reentry supraventricular tachycardia was controlled in 15 of 25 patients. Propafenone was partially or completely effective in 3 of 4 patients with chaotic atrial tachycardia. Junctional

Paroxysmal atrial fibrillation precipitated by amiodarone-induced thyrotoxicosis five months after cessation of therapy.

Amiodarone is currently indicated for the treatment of life-threatening ventricular dysrhythmias. It is also used for the treatment of supraventricular dysrhythmias and as maintenance therapy after successful cardioversion of atrial flutter or atrial fibrillation. Adverse effects related to its expanded use are increasingly common. We describe a case of amiodarone-induced thyrotoxicosis occurring 5 months after cessation of therapy and discuss the pathophysiology and treatment of this disorder.

Atrioventricular accessory pathway unmasked by heart valve replacement.

A 50-year-old male patient with a history of severe valvular regurgitation underwent mitral and aortic valve replacement surgery 3 months ago. Preoperative 12-lead electrocardiogram presented atrial flutter (AFL) and atrial fibrillation. AFL complicated with ventricular pre-excitation was observed on current admission. The potential mechanisms underlying these changes were considered multifaceted, and valve replacement procedure may be a rare incentive factor.

Flecainide-induced ventricular tachycardia and fibrillation in patients treated for atrial fibrillation.

Flecainide acetate has a recognized proarrhythmic effect in patients treated for ventricular tachycardia. Three patients developed severe ventricular arrhythmias while taking flecainide for atrial fibrillation. Patient 1 had normal ventricular function and idiopathic atrial fibrillation. Treadmill exercise tests during digoxin therapy showed no ventricular arrhythmia; however, during flecainide therapy the patient developed ventricular flutter at his peak exercise level that required cardioversion. Patient 2 had normal ventricular function and a prosthetic mitral valve. During therapy with fle…

[Disorders of heart rhythm in hypertrophic cardiomyopathy].

In 64 patients with hypertrophic cardiomyopathy 24-hour Holter monitoring of the ECG was made in order to detect the incidence of individual disorders of the cardiac rhythm. The examination revealed a large number of potentially malignant ventricular arrhythmias (47% of the patients). Most frequently polytopic ventricular extrasystoles were involved (31.3%), attacks of ventricular tachycardia (20.3%), less frequently bigeminy was recorded (10.9%) and early extrasystoles type R on T (3.1%). As to other disorders of the cardiac rhythm, paroxysmal supraventricular tachycardia was frequent (34.4%)

Cor triatriatum: echocardiograhic findings.

The echocardiographic findings in a patient with cor triatriatum are presented. The preoperative echocardiogram demonstrated an unusual structure behind the mitral valve that moved anteriorly during atrial systole. There was also echocardiographic evidence of abnormalities of mitral valve motion, right ventricular and left atrial enlargement, and pulmonary hypertension. Postoperatively, the unusual echo behind the mitral valve had disappeared and the mitral valve appeared more normal. Left atrial and right ventricular dimension had decreased and the pulmonary valve appeared more normal. Fine d
