Relationship between monitoring parameters and perinatal outcome in severe, early intrauterine growth restriction.

OBJECTIVE: To investigate whether pathological changes in the umbilical artery (UA), ductus venosus (DV) and short-term fetal heart variation are related to perinatal outcome in severe, early intrauterine growth restriction (IUGR). METHODS: This multicenter, prospective, longitudinal, observational study was carried out in the Departments of Fetal Medicine and Obstetrics in Hamburg, Amsterdam, Utrecht and London. In 70 singleton pregnancies with IUGR fetuses, delivered at 26-33 weeks of gestation because of antepartum fetal distress, short-term variation (STV) of fetal heart rate, pulsatility

Beyond canonical homeostasis: Dynamic, plastic, and intermittently unregulated physiology as the norm.

Homeostasis is often taught as steady-state regulation keeping internal variables within narrow limits-reflecting a mammalian bias. Yet, canonical homeostasis is far from universal. In many animals, including humans, normal physiological states allow some variables to fluctuate cyclically or stochastically while others remain tightly controlled. Indeed, across non-mammalian species, broad physiological variability is frequently normal rather than exceptional. Here we present case studies showing that plastic, variable regulation-sometimes approaching randomness-is both common and adaptive. Exa

Technological advances and clinical application of left ventricular assist devices in the treatment of advanced heart failure.

Left ventricular assist devices (LVADs) have become an important treatment for advanced heart failure, especially given the shortage of heart transplant donors. This article systematically reviews the technological evolution, perioperative assessment, pathophysiological changes, gender differences, functional recovery, and complication management of LVADs, focusing on the CH-VAD, a fully magnetically levitated centrifugal pump independently developed in China. Results show that LVAD technology has evolved from pulsatile flow devices to the third-generation magnetically levitated centrifugal pu…

Hemodynamic and pathological Effects of aortic occlusion during Resuscitation: An ex vivo circulatory platform and in vivo animal study.

Aortic occlusion serves as a critical life-saving intervention by simultaneously controlling hemorrhage and restoring perfusion pressure. However, its clinical efficacy remains debated due to the difficulty of assessing real-time hemodynamic responses during emergencies and the absence of pathological validation. Here, we present an integrated biomechanical-pathological framework combining a biomimetic circulatory platform with in vivo animal validation. The platform, which couples a pulsatile pump-motor system with a 3D-printed aortic model, faithfully replicates physiologic flow and enables

SNR-efficient whole-brain pseudo-continuous arterial spin labeling perfusion imaging at 7 T.

PURPOSE: To optimize pseudo-continuous arterial spin labeling (PCASL) parameters to maximize SNR efficiency for RF power constrained whole brain perfusion imaging at 7 T. METHODS: We used Bloch simulations of pulsatile laminar flow to optimize the PCASL parameters for maximum SNR efficiency, balancing labeling efficiency and total RF power. The optimization included adjusting the inter-RF pulse spacing (TRPCASL), mean B1 + (B1 + ave), slice-selective gradient amplitude (Gmax), and mean gradient amplitude (Gave). In vivo data were acquired from six volunteers at 7 T to validate the optimized p

Case report: a thrombus to remember: a case of warfarin non-compliance in a total cavopulmonary circulation patient.

BACKGROUND: The lack of pulsatile nature of the pulmonary blood flow through total cavopulmonary circulation (TCPC) has shown to prompt endothelial dysfunction thus creating a hypercoagulable state. Patients with a Fontan-type circulation are, therefore, at an increased risk of venous and arterial thromboembolism. In the setting of hypoplastic left heart syndrome following staged surgical repair, subaortic thrombosis is relatively common, but thrombosis of the hypoplastic aortic root is less commonly described. CASE SUMMARY: Here, we present the case of a 25-year-old male with a background of

Symptomatic cerebral hyperperfusion after occipital artery-posterior cerebral artery bypass in a patient with moyamoya disease: illustrative case.

BACKGROUND: Moyamoya disease (MMD) is a progressive, steno-occlusive cerebrovascular disorder. Posterior cerebral artery (PCA) stenosis can develop after successful anterior circulation revascularization. Although symptomatic cerebral hyperperfusion (CHP) is a recognized complication of anterior revascularization, it is rarely reported following occipital artery (OA)-PCA bypass. OBSERVATIONS: The authors report the case of a 50-year-old woman with MMD who developed symptomatic hemodynamic compromise due to progressive right PCA stenosis 10 years after bilateral superficial temporal artery-mid

Effect of Blood Pressure Levels on Sinus Hemodynamics in Relation to Calcification After Bioprosthetic Aortic Valve Replacement.

Coexisting hypertension and aortic stenosis are common. Some studies showed that elevated blood pressures may be associated with progression of calcific aortic valve disease (CAVD) while others showed no correlation. Flow dynamics in the sinuses of Valsalva are considered key factors in the progression of CAVD. While the relationship between hemodynamics and CAVD is not yet fully understood, it has been demonstrated that they are tightly correlated. This study aims to investigate the effect of changing systolic and diastolic blood pressures (SBP and DBP, respectively) on sinus hemodynamics in

Traumatic middle meningeal arteriovenous fistula presenting with long delayed-onset facial nerve palsy without temporal bone fracture: illustrative case.

BACKGROUND: Traumatic facial nerve palsy (FNP) without temporal bone fracture (TBF) has a delayed onset in some cases; however, long delayed-onset FNP in this setting has not been reported. The middle meningeal vein (MMV) is one of the venous drainage routes from the facial nerve. Herein, the authors describe a rare case of traumatic middle meningeal arteriovenous fistula (MMAVF) presenting with the long delayed-onset FNP without TBF. OBSERVATIONS: A 42-year-old man with pulsatile tinnitus and left FNP was admitted to our hospital 4 weeks after head trauma without TBF. Cerebral angiography re

Duality in response of intracranial vessels to nitroglycerin revealed in rats by imaging photoplethysmography.

Among numerous approaches to the study of migraine, the nitroglycerin (NTG) model occupies a prominent place, but there is relatively insufficient information about how NTG affects intracranial vessels. In this study we aim to assess the effects of NTG on blood-flow parameters in meningeal vessels measured by imaging photoplethysmography (iPPG) in animal experiments. An amplitude of the pulsatile component (APC) of iPPG waveform was assessed before and within 2.5 h after the NTG administration in saline (n = 13) or sumatriptan (n = 12) pretreatment anesthetized rats in conditions of a closed c

Fabrication of Pulsatile Polymeric Microparticles Encapsulating Rabies Antigen.

The current guidelines for rabies post-exposure prophylaxis require multiple injections administered over several weeks. This can be disproportionately burdensome to those living in low- and middle-income countries (LMICs), where the majority of deadly exposures to rabies occur. Different drug delivery strategies have been explored to condense vaccine regimens to a single injection by encapsulating antigens into polymeric particles. However, harsh stressors during the encapsulation process can cause denaturation of the encapsulated antigen. This article describes a method for encapsulating the

Transverse-sigmoid Sinus Dural Arteriovenous Fistula Presenting with False Localizing Signs of Cavernous Sinus Dural Arteriovenous Fistula Due to Obstructed Cerebral Venous Outflow into the Bilateral Superior Ophthalmic Veins: A Case Report.

Cavernous sinus (CS) dural arteriovenous fistula (dAVF) presents ocular symptoms and visual disorders due to retrograde drainage from the CS into the superior ophthalmic vein (SOV). Some papers reported non-CS dAVFs with those symptoms. We present a unique case of transverse-sigmoid sinus (TSS) dAVF with localizing signs of CS dAVFs resulting from congested cerebral venous outflow into the CSs and SOVs in an 86-year-old female patient. Right pulsatile tinnitus and chemosis appeared a few years ago. After experiencing progressive bilateral blurred vision and decreased visual acuity with papilla

Mechanical circulatory support devices in advanced heart failure: 2020 and beyond.

Substantial progress in the field of mechanical circulatory support (MCS) has expanded the treatment options for patients with advanced-stage heart failure (HF). Currently available MCS devices can be implanted percutaneously or surgically. They can also be configured to support the left, right, or both ventricles, offering varying levels of circulatory support. Short-term temporary MCS devices are primarily used in high-risk percutaneous coronary intervention, cardiogenic shock, and post-cardiac arrest, while durable left ventricular assist systems (LVAS) are increasingly utilized either as a
