Subcutaneous anchoring device for securement of tunneled hemodialysis catheters: A first retrospective multicenter case series.

BACKGROUND: Subcutaneous Anchor Securement System (SASS) are increasingly used for stabilization of vascular access devices. For many hemodialysis (HD) patients tunneled central venous catheters (t-CVC) are the last vascular access option and their unplanned replacement results in complex procedures and increased costs. Nevertheless, accidental dislodgment of hemodialysis t-CVC is a frequent occurrence in clinical practice. We here describe our first experience with SASS utilization in this unexplored setting. METHODS: Between July 2022 and September 2023, we applied a SASS (SecurAcath®) to n

Paradoxical Embolization of an Intravenous Catheter Fragment through Patent Foramen Ovale: A Rare Case of Acute Limb Ischaemia.

BACKGROUND: Paradoxical embolism occurs when venous materials traverse through a patent foramen ovale (PFO) into arterial circulation. Catheter fragment embolisation represents an unusual cause of systemic arterial occlusion with significant morbidity. CASE PRESENTATION: An 83-year-old female presented with sudden-onset pain, coldness, and cyanosis of the right lower extremity 15 days after pacemaker implantation requiring intravenous catheterisation. Physical examination revealed absent popliteal and distal pulses. Vascular imaging confirmed acute arterial occlusion. During surgical embolect

Safety of long-term esophageal stent placement and a predictive model for restenosis: A double-center retrospective study.

BACKGROUND AND AIMS: Self-expanding metal stents (SEMS) relieve dysphagia but may cause complications. This study assessed the safety of long-term SEMS indwelling, identified risk factors, and developed a Cox model for restenosis prediction. METHODS: A retrospective study was conducted at two centers, including 412 patients who underwent stenting (2015-2025). Complications were compared across indwelling times (< 30, 30-59, ≥ 60 days) and indications (benign, malignant). Logistic regression, Kaplan-Meier, and Cox analyses identified predictors, and a nomogram was constructed. RESULTS: The ov

Inferior Vena Cava Filter Long Term Complications and Retrieval Techniques: A Case Series and Literature Review.

INTRODUCTION AND IMPORTANCE: Long-term mechanical complications after inferior vena cava (IVC) filter placement include embedded or tilted filters, erosion of the vena cava, filter thrombosis. In the setting of caval thrombosis, patients may subsequently develop venous hypertension and post-thrombotic syndrome (PTS). Here we present three unique cases of IVC filter complications and surgical management. CASE PRESENTATION: A 30-year-old female presented with acute abdominal pain, revealing a duodenal perforation caused by an IVC filter eroding into her duodenum. A 42-year-old female with an IV

Classifying Device-Related Complications Associated With Intrathecal Baclofen Pumps: A MAUDE Study.

BACKGROUND: Intrathecal baclofen (ITB) is an effective treatment for spasticity, and therapeutic levels may be optimized using surgically implanted pumps. Though these devices are effective, complications can pose significant challenges to patients, decreasing the therapeutic effect and potentially requiring reoperation. As such, it is critical that complications associated with ITB be effectively characterized. METHODS: We queried the Manufacturer and User Facility Device Experience (MAUDE) database for cases reported during the past 3 years documenting adverse events specifically related to

Porta hepatis abscess and portal vein thrombosis following ingestion of a fishbone.

A man in his late 50s presented to the emergency room with a 1-month history of severe abdominal pain and an endoscopic fishbone retrieval from his rectum. Serial CT scans revealed a fishbone located in the patient's upper abdomen, which had migrated through the stomach wall, into the periportal space, causing a contained gastric perforation, development of a porta hepatis abscess and secondary portal vein thrombosis. Furthermore, the sharp tip of the fishbone lay 5 mm from the patient's hepatic artery. He was transferred to a hepatobiliary centre where he underwent urgent exploratory laparoto

An Extremely Rare Complication: Abdominal Aortic Aneurysm Rupture Caused by Migration of a Zenith Main Body Years After Repair of the Suprarenal Stent Separation.

PURPOSE: To report an unusual case of an abdominal aortic aneurysm (AAA) rupture caused by migration of a Zenith stent-graft main body years after its separation from the suprarenal stent. CASE REPORT: A 72-year-old man underwent endovascular aneurysm repair with a Zenith stent-graft for an infrarenal AAA in year 2000. At that time, a femorofemoral bypass was performed because the left external iliac and common femoral arteries were dissected during treatment. In 2013, follow-up computed tomography (CT) showed disconnection of the uncovered proximal stent, which led to a type Ia endoleak. An

Aspiration Thrombectomy-Assisted Endovascular Retrieval of an Embolized Angio-Seal Device Causing Claudication.

The Angio-Seal vascular closure device is used to reduce time to hemostasis after femoral artery puncture. Although rare, complications associated with Angio-Seal are significant, including infection, pseudoaneurysm formation, symptomatic femoral artery stenosis, and separation and embolization, leading to limb-threatening ischemia. This report describes Angio-Seal polymer anchor embolization to the tibioperoneal trunk successfully retrieved using the 8-French Indigo Aspiration System.

Asymptomatic patients with unsuccessful percutaneous inferior vena cava filter retrieval rarely develop complications despite strut penetrations through the caval wall.

OBJECTIVE: We established a program for retrieval of inferior vena cava (IVC) filters within our hospital system. When percutaneous retrieval fails, we only recommend open surgical removal for symptoms and other complications. We examined our outcomes with conservative management of unsuccessful percutaneous retrieval and open surgical removal for symptomatic/complicated IVC filters. METHODS: All patients with history of IVC filter placement who were referred to us for retrieval between 2010 and 2016 were evaluated. Before retrieval, patients were evaluated for risk of future venous thromboem

Drug-Coated Balloons: Hope or Hot Air: Update on the Role of Coronary DCB.

PURPOSE OF REVIEW: The present manuscript reviews the mechanism of action of drug-coated balloons (DCBs), offering a brief summary of the main clinical evidence on these devices. RECENT FINDINGS: DCBs are regular semi-compliant balloons coated with antiproliferative agents that are rapidly released on contact with the vessel intima, exerting an anti-restenotic effect. This technology may offer some benefits of drug-eluting stents, in particular for the treatment of restenotic lesions, small vessels, and in patients at high-bleeding risk, when the prolonged dual antiplatelet regimen should be

Removal of Polyacrylamide Gel (Aquamid®) from the Lip as a Solution for Late-Onset Complications: Our 8-Year Experience.

BACKGROUND: The polyacrylamide hydrogel Aquamid® has been used as a permanent filler to enhance facial soft tissue volume and correct wrinkles since 2001. Various long-term studies have proved the safety of the product. Nonetheless, if complications such as migration occur, they can be difficult to treat. METHODS: Eleven patients suffering from late-onset complications after taking Aquamid® injections in the lips underwent product removal and subsequent labial reconstruction between 2009 and 2017. The reconstruction was performed using a modified bikini reduction technique combined, in eight

Access From Above-Bailout Solution for Intraoperative Distal Migration of Abdominal Aortic Stent Graft.

The majority of the ruptured abdominal aortic aneurysms today is treated endovascularly. In cases with short aneurysm neck, chimney technique can be used to extend landing zone in emergency setting. Additionally, the repositioning ability of C3 delivery system (Gore & Associates) allows better positioning in cases with challenging anatomy. In our experience, proximal reposition of partially deployed device can be problematic in some patients. We present a case of endovascular repair of ruptured abdominal aortic aneurysm using chimney technique where proximal reposition was achieved by snaring
