We studied 54 patients operated on for combined supraspinatus and infraspinatus rotator-cuff tears. The presence or absence of the dropping and hornblower's clinical signs of impaired external rotation were correlated with Goutallier stage-3 or stage-4 fatty degeneration of infraspinatus and teres minor. These grades of fatty degeneration have previously been correlated with a poorer outcome from reconstructive surgery. We found that hornblower's sign had 100% sensitivity and 93% specificity for irreparable degeneration of teres minor and the dropping sign 100% sensitivity and 100% specificity

Humeral avulsion of the glenohumeral ligament (HAGL) is a lesion that has been recognized as a cause of recurrent shoulder instability. To our knowledge there are no reports of successful return to full function in young, competitive athletes or return to manual labor following nonoperative management of a HAGL lesion. A 26-year-old Navy SEAL was diagnosed with a HAGL injury, and associated traction injury of the axillary nerve as well as a partial tear of the rotator cuff. Operative intervention was recommended; however, due to issues with training and with inability to properly rehab with th

INTRODUCTION: Since traumatic injuries to the scapula and scapulothoracic articulation are rare, they have not been extensively covered in the literature. According to reports, arthrosis, rotator cuff dysfunction, scapulothoracic dyskinesis, and impingement pain all contributed to decreased shoulder function after nonsurgical therapy. In addition, it was noted that surgical intervention for scapular fractures produced better results and a higher functional recovery.
CASE REPORT: A 51-year-old female presented to our emergency department with a history of a fall of a heavy object over her righ

Posterior shoulder dislocation is an uncommon condition, accounting for a minority of shoulder instability cases. It is more frequently encountered in high-level athletes and is often missed at initial presentation, which can lead to chronic instability and recurrent dislocations. Predisposing factors include congenital anatomical variations, capsular laxity, and acquired injuries such as trauma or tetanic muscle contractions during seizures or electrical shocks. Management strategies are broadly divided into conservative and surgical approaches. Nonoperative treatment focuses on strengthening

Platelet-rich plasma (PRP) as a treatment modality has been around for the last four decades, but only truly gained popularity over the last 10 to 15 years in medicine, in a variety of fields ranging from regenerative medicine to infertility treatment. It has gained popularity, especially in treating musculoskeletal conditions where the bulk of research has been performed and published. There is level I evidence available supporting its efficacy in the treatment of osteoarthritis (OA), epicondylitis, bursitis, compressive neuropathy, plantar fasciitis, muscular injuries and osteochondral lesio

This study evaluates the patient-reported functional outcome, clinical functional outcome and frequency of complications of simple oblique and transverse humeral midshaft fractures treated with a retrograde expert humeral nail. A retrospective cohort study of humeral midshaft fractures (AO 12-A2, 12-A3) treated with retrograde nailing between January 2010 and February 2018 in a level II trauma center was performed. Patients' perception of functional outcome was measured using the Disabilities of the Arm, Shoulder and Hand (DASH) scores. Thirteen patients with a median age of 20-years were trea

BACKGROUND AND OBJECTIVES: The physical injuries caused by +Gz include skeletal-muscle overloads. This study has aimed at demonstrating ergonomic conditions of the pilots from the Brazilian Air Force’s Aerial Demonstration Squadron (BAFADS) during flight.
METHODS: All the 13 pilots from the BAFADS were evaluated through an interview. Surface electromyography (SEMG) was employed during the flight manoeuvre simulation (under normal and overload conditions).
RESULTS: The report analyses showed perception of discomfort in the right shoulder related to +Gz. The SEMG showed moderate levels of shou

Ergonomics and prevention of musculoskeletal disorders in obstetric sonography is a subject seldom discussed. However, 80% of sonographers describe pain when performing these examinations. The consequences of these disorders can range from impaired quality of life to the complete cessation of professional activity. Some diseases may even require surgical treatments such as carpal tunnel syndrome and shoulder rotator cuff injuries. After joint disorders mainly affecting the upper limbs and neck, deterioration of visual acuity is the second most commonly reported condition. A literature review c…

Luxatio erecta, or inferior glenohumeral dislocation, is a rare shoulder dislocation usually caused by a hyperabduction injury to the arm. We have reviewed the literature consisting of 80 cases of luxatio erecta and also discuss six additional cases that we have treated. The literature shows that either a fracture of the greater tuberosity or a rotator cuff tear was associated with this injury in 80% of patients; 60% of the patients reviewed sustained some degree of neurologic compromise, most commonly to the axillary nerve. These injuries usually resolved; the time for recovery varied from 2

BACKGROUND: Diagnosis and assessment of functional deltoid recovery following axillary nerve injury is challenging due to numerous other muscular contributions to shoulder movement - particularly supraspinatus, rhomboids and long head of triceps. We present the deltoid abduction lag test (DALT) which offers four clear benefits: 1) it successfully isolates deltoid in the presence of a functioning supraspinatus with intact tendon and attachment; 2) it is safe to perform early following anterior shoulder dislocation (the most common cause of axillary nerve injury); 3) it allows assessment of earl

A consecutive case series from 2 institutions of patients with postoperative wound infections after mini-open rotator cuff repair was reviewed. Between 1991 and 2000, 360 patients underwent mini-open rotator cuff repair after arthroscopic subacromial decompression. Seven patients had postoperative infection develop (1.9%). All patients were men, with a mean age of 55 years (range, 40-64 years). Treatment included serial irrigation and debridement, long-term intravenous antibiotics, and revision rotator cuff repair. Mean follow-up after definitive treatment was 32 months (range, 12-57 months).

We present the case of a patient with the rare disorder tracheobronchopathia osteochondroplastica who underwent laparoscopic cholecystectomy. After induction of general anaesthesia, we faced difficulties passing the tracheal tube beyond the vocal cords despite bronchoscopic assistance. With a smaller tube, and by using rotating movements, we managed to successfully intubate the trachea. Because of the irregular tracheal surface, however, ventilation was challenging due to a massive cuff leak. Repeated repositioning did not improve this leak. Only cuff overinflation led to adequate ventilation,

The prevalence of asymptomatic shoulder pathology has been shown to be high on both ultrasound and magnetic resonance imaging (MRI). The most common shoulder pathologies identified in asymptomatic, non-athlete individuals include rotator cuff pathology, acromioclavicular (AC) joint pathology, labral tears, subacromial bursitis, and calcific tendinitis. The data in the current literature suggest that asymptomatic rotator cuff tears are diagnosed on ultrasound and MRI at high rates, suggesting that rotator cuff tears may be considered an age-related, normal, degenerative change. However, there a

RATIONALE: We describe a rare case of the isolated tear in the cord-like middle glenohumeral ligament (MGHL) in Buford complex. To the best of our knowledge, this is the first report in the English literature about the isolated tear of cord-like MGHL in Buford complex. The present report describes in detail our experience with the diagnosis of isolated tear of the cord-like MGHL in Buford complex and management.
PATIENT CONCERNS: A 50-year-old female patient visited our hospital with pain and stiffness in the right shoulder that lasted for 9 months.

Certain arthropathies can distort the normal acromiohumeral relationship and make traditional anterolateral access to the proximal humerus for nailing difficult or impossible. This article presents a case of bilateral antegrade humeral nailing in which the Neviaser portal approach was used for humeral shaft fractures in a patient with distorted shoulder anatomy secondary to severe cuff tear arthropathy and rheumatoid arthritis. Based on a literature review, extending the traditional superomedial (Neviaser) portal to the shoulder to perform humeral nailing has never been described clinically. A

This study hypothesizes that full-thickness tearing of the rotator cuff can lead to joint instability and that the degree of instability depends on the size and location of the tear. Twelve cadaveric shoulder specimens were divided into two groups: group 1 had a circular tear centered at the critical area, and group 2 had a circular tear centered at the rotator interval. Each group was tested at 2.5 cm and 5 cm tear sizes. Unloaded, and with the arm in 90 degrees flexion and full internal rotation, the humeral head shifted posteriorly. With loading, a large and more anteriorly located defect h

The conservative management of shoulder instability depends on a well-defined program that emphasizes early diminution in the patient's symptoms, appropriate immobilization, and a well-defined precise rehabilitation program that can be individually tailored to meet the patient's needs. Immobilization should be considered in younger patients less than 20 years of age for between 3 to 4 weeks in order to enhance capsulolabral complex healing. Older patients may be immobilized for a shorter period of time. Analgesic medication and local physical therapeutic modalities may be used to decrease the

Inferior dislocation of the shoulder (luxatio erecta) is a rare type of shoulder dislocation. In this short article we review a case of a 56-year-old man who sustained a subcoracoid inferior dislocation of the right shoulder after accidental axial loading of the right arm. Post-reduction magnetic resonance scan showed supraspinatus tendon rupture. Such type of shoulder dislocation is associated with significant complications (such as rotator cuff tears) and also late complications (including instability, chronic pain and reduced motion). A brief description of presentation, diagnosis and manag

The clinical evaluation of the painful shoulder by an experienced and knowledgeable health care professional remains the most sensitive and clinically useful tool for the assessment of most patients with rotator cuff disease, glenohumeral instability, glenohumeral arthritis, and adhesive capsulitis. The judicious and appropriate use of diagnostic imaging studies requires a thoughtful and thorough evaluation of the data obtained from these studies and their correlation to the patient's symptoms, history, mechanism of injury, and physical findings. The appropriate use of all of this diagnostic i

PURPOSE: Epileptic seizures can cause multiple shoulder injuries, the most common of which are dislocations, recurrent instability, fractures, and isolated lesions of the rotator cuff. Currently, only limited literature exists which describes the frequency and types of lesions in cohorts of epileptic patients and the corresponding treatment outcome. This study aims to document the occurrence of shoulder lesions in patients affected by seizures and to provide detailed information on trauma dynamics, specific lesion characteristics and treatment complications.
METHODS: All patients referring to
