Postoperative Blepharoptosis After Modern Phacoemulsification Procedure.

PURPOSE: To investigate the frequency of transient (1 month) and persistent (at least 6 months) postoperative ptosis following clear corneal sutureless phacoemulsification and to analyze the factors affecting them. DESIGN: Cohort study. METHODS: Patients who underwent phacoemulsification cataract surgery from October 2016 to June 2018 in a tertiary center were enrolled. Margin reflex distance 1 (MRD1), MRD2, and levator function were measured and facial photography was taken before, 1 month, and at least 6 months after the surgery. Clinical ptosis was defined as any postoperative drop of MRD

Blepharoptosis and HAART related mitochondrial myopathy.

PURPOSE: To report a case of blepharoptosis in a patient with human immunodeficiency virus (HIV) on highly active antiretroviral therapy (HAART) with biopsy confirmed mitochondrial deletions consistent with HAART related myopathy. METHODS: A 51-year-old man with HIV demonstrated visually significant ptosis after being on HAART therapy for over 20 years. RESULTS: Muscle tissue biopsy following blepharoplasty was analyzed and found to have significant mitochondrial deletions. CONCLUSIONS: This patient represents a case of isolated ptosis consistent with acquired myopathy secondary to mitochon…

Genetic study of blepharoptosis among Egyptians.

OBJECTIVES: This study was aimed to develop an etiological classification of congenital blepharoptosis among Egyptian children and also to differentiate between congenital blepharoptosis as an isolated anomaly or part of a syndrome. BACKGROUND: Blepharoptosis refers to dropping of the upper eyelid. The difference in the height of the fissures with the eyes in primary position signifies the amount of ptosis. Ptosis has traditionally been divided into congenital and acquired types. METHODS: Thirty six patients presenting with blepharoptosis (22 males and 14 females) were examined. All patients…

Acquired blepharoptosis secondary to essential blepharospasm.

We treated four patients with essential blepharospasm, receiving botulinum A toxin, in whom, although they had no preexisting blepharoptosis, a concurrent bilateral acquired blepharoptosis developed. Since the blepharoptosis did not improve after the period of time during which the effects of botulinum A toxin would have been expected to resolve (2 to 10 weeks), we judged that its development was unrelated to the toxin. We propose, rather, that the stretching, attenuation, disinsertion, or dehiscence of the upper eyelid levator muscle caused by the blepharospasm were at least partly responsibl…

Unilateral Blepharoptosis from Renal Cell Carcinoma.

Pyoderma gangrenosum is a noninfectious neutrophilic dermatosis that usually starts with sterile pustules which rapidly progress to painful ulcers of variable depth and size with undermined violaceous borders. In 17 to 74% of cases, pyoderma gangrenosum is associated with an underlying disease, most commonly inflammatory bowel disease, rheumatological or hematological disease or malignancy. Diagnosis of pyoderma gangrenosum is based on a history of an underlying disease, typical clinical presentation and histopathology, and exclusion of other diseases that would lead to a similar appearance. Blepharoptosis due to Kearns-Sayre syndrome.

A 16-year-old patient suffering from Kearns-Sayre syndrome presented with severe blepharoptosis. A levator advancement procedure was performed. It seems that in conjunction with medical treatment, blepharoptosis surgery offers good and hopefully long lasting results to patients suffering from this rare syndrome.

[Acute blepharoptosis caused by fibrous dysplasia].

We report a case of fibrous dysplasia revealed by acute ptosis in a 38-year-old patient. Ophthalmologic examination disclosed a moderate right ptosis. The rest of the results were strictly normal except for a known facial asymmetry. Visual acuity was 20/20 in both eyes, there was no extrinsic or intrinsic oculomotor disorder, and funduscopy was normal. Given this acute presentation, emergency cerebral imagery was carried out, providing a diagnosis of fibrous dysplasia with orbital and cavernous repercussions, with no sign of cerebral disorder or obvious vascular lesion. Progression was marked

Aetiology of acquired blepharoptosis in young adults.

PURPOSE: To identify the causes of blepharoptosis in young adults, and explore cases that do not fit into current diagnostic categories. METHODS: A retrospective cohort study of all patients aged 18-40 years ("young adults") with acquired blepharoptosis that presented to two specialist ocular plastics practices and a paediatric ophthalmologist over a period of up to 25 years. Each patient was classified according to diagnosis. Where the diagnosis was uncertain, the files were examined in detail to try and further establish a cause. RESULTS: A total of 266 young adult patients were included. …

Tarsoaponeurectomy as an alternative in difficult blepharoptosis cases.

BACKGROUND: The purpose of this study was to evaluate the results of tarsoaponeurectomy in patients with unsuccessful results after repetitive surgery or who developed post-traumatic blepharoptosis. METHODS: The files of 107 patients (136 eyes) on whom surgery was performed between January 2010 and December 2014 due to blepharoptosis were scanned retrospectively. Among these patients, the files and operational notes of eight patients who underwent surgery through the method of tarsoaponeurectomy were examined in detail. The epidemiological data, indication for surgery, previous ptosis and/or …

Blepharoptosis and hypertrophic osteoarthropathy: A case report.

A 52-year-old male patient presented to our hospital with a history of secondary hypertrophic osteoarthropathy (HOA) associated with an abdominal neoplasia and blepharoptosis. He had finger clubbing, hyperhidrosis, and hypertrichosis. He also had a recent history of extensive abdominal surgery with a pathology report of myelolipoma. Routine blood work was unremarkable. Upper eyelid reconstruction with blepharoplasty, upper eyelid wedge resection, and brow suspension was performed to address his eyelid concerns. By this case report, we would like to attract notice that the eyelid involvement ma…

Blepharoptosis surgery in patients with myasthenia gravis.

BACKGROUND/AIMS: To review our approach of cautious surgical correction of blepharoptosis in patients with myasthenia gravis (MG) to minimise risk of exposure complications. METHODS: Retrospective case note review of 30 patients with symptomatic eyelid concerns despite appropriate medical treatment, who underwent eyelid surgery. The mean age at diagnosis was 47 years. 13/30 patients had systemic MG, 14/30 ocular MG and 3/30 congenital MG. The main outcome measures were improvement in eyelid height and/or position, duration of a successful postoperative result, need for further surgical interv…

Topical naphazoline in the treatment of myasthenic blepharoptosis.

INTRODUCTION: When treating ocular myasthenia gravis (MG), the risk/benefit profile of corticosteroids is unclear, and acetylcholinesterase inhibitors are not very effective. We examined the efficacy of topical naphazoline in the treatment of myasthenic blepharoptosis. METHODS: Sixty MG patients with blepharoptosis (32 with ocular symptoms only and 28 with mild generalized symptoms) were enrolled in a multicenter open trial of topical naphazoline. The effects were reported by patients via a questionnaire and were also confirmed for each patient at the clinic. RESULTS: Among 70 eyes of 60 pat…

Unilateral orbital fibrosis with blepharoptosis and enophthalmos.

A 59-year-old woman with congenital orbital fibrosis with ptosis and enophthalmos underwent surgery to widen the palpebral fissures, which resulted in an acceptable cosmetic appearance. This is a rare entity that has been classified as a subtype of congenital fibrosis of the extraocular muscles. However, after a review of literature we believe that congenital orbital fibrosis should be considered a separate entity: it is unilateral, diffusely involves the orbit (not just the extraocular muscles) and does not appear to have a hereditary predisposition.

Downgaze eyelid position in patients with blepharoptosis.

PURPOSE: To prospectively examine the change in upper eyelid position on downgaze in patients with blepharoptosis. METHODS: Upper eyelid margin to corneal light reflex distance (uMRD) was measured in primary gaze and in 30 degrees to 40 degrees downgaze in 50 consecutive patients with acquired ptosis and 10 consecutive patients with congenital ptosis. RESULTS: For the acquired ptosis group, the mean change in uMRD on downgaze was -0.8 mm (range, -2.5 to 1.0 mm), with one patient (2%) showing an increase, 10 (20%) no change, and 39 (78%) a decrease in upper eyelid position. For the congenital…

Histological changes in congenital and acquired blepharoptosis.

Histological specimens of eyelids from patients with congenital genuine ptosis and acquired involutional ptosis are compared. In congenital ptosis there is evidence of true muscular dystrophy of the levator muscle as demonstrated by: loss of cross striations, random decrease in muscle fibre diameter, sarcolemmal retraction, nuclei alignment, and fibrous and fatty tissue displacement of the striated fibres. In involutional ptosis disinsertion of the levator aponeurosis from its normal insertion on the inferior-anterior surface of the tarsus is demonstrated. Correlation between clinical presenta

The Relevance of Blepharoptosis in Diagnostic Suspicion of Myopathies.

Blepharoptosis (ptosis) is classified, based on etiology, into mechanical, cerebral, neurogenic, neuromuscular, myogenic, and due to miscellaneous causes. Primary myopathic diseases are rare causes of blepharoptosis and many patients with myogenic ptosis undergo a series of extensive investigations before a myopathy is being considered. In this study, we report four patients with different myopathic disorders who had blepharoptosis as a presenting symptom of their disease. Moreover, we highlight frequent diagnostic errors and difficulties in patients with myopathies who present blepharoptosis.…
