Diagnosis of macroglossia and indications for reduction glossectomy.

Macroglossia can create dentomusculoskeletal deformities, instability of orthodontic and orthognathic surgical procedures, and masticatory, speech, and airway problems. The cause of macroglossia must be clearly defined, and true macroglossia separated from pseudomacroglossia (displacement of the tongue created by anatomic factors other than tongue size alone). This article discusses the signs and symptoms of macroglossia, including the clinical and radiographic features, treatment techniques, as well as previously reported results. Cases are shown to illustrate the applicability of this techni

[Value and results of glossectomy in open bite].

Partial glossectomy is an important part of the treatment of a number of forms of apertognathia, provided it is performed wisely with a rigorous technique (i.e. with sufficient resection and adapted to the lesions produced by the macroglossia). It can obtain favourable and constant results.

Speech outcomes in Cantonese patients after glossectomy.

BACKGROUND: We sought to determine the major factors affecting speech production of Cantonese-speaking glossectomized patients. Error pattern was analyzed. METHODS: Forty-one Cantonese-speaking subjects who had undergone glossectomy > or = 6 months previously were recruited. Speech production evaluation included (1) phonetic error analysis in nonsense syllable; (2) speech intelligibility in sentences evaluated by naive listeners; (3) overall speech intelligibility in conversation evaluated by experienced speech therapists. RESULTS: Patients receiving adjuvant radiotherapy had significantly p…

Speech intelligibility after glossectomy and speech rehabilitation.

BACKGROUND: Oral tumor resections cause articulation deficiencies, depending on the site, extent of resection, type of reconstruction, and tongue stump mobility. OBJECTIVES: To evaluate the speech intelligibility of patients undergoing total, subtotal, or partial glossectomy, before and after speech therapy. PATIENTS AND METHODS: Twenty-seven patients (24 men and 3 women), aged 34 to 77 years (mean age, 56.5 years), underwent glossectomy. Tumor stages were T1 in 3 patients, T2 in 4, T3 in 8, T4 in 11, and TX in 1; node stages, N0 in 15 patients, N1 in 5, N2a-c in 6, and N3 in 1. No patient h…

Major glossectomy: end results of 106 cases.

Advanced cancers of the oral cavity continue to be a therapeutic challenge. Despite significant improvements in radiotherapeutic techniques and adjuvant chemotherapy, patients usually die after a short period. Recent progress in reconstructive techniques has made major glossectomy (subtotal, near total, total or extended total) a reasonable palliative and potentially curative approach. It is the purpose of this study to report a series of 106 patients treated from 1985 to 1994 regarding surgical complications and prognosis. All but 1 patient undergoing major glossectomy had squamous cell carci…

Production of tongue twisters by speakers with partial glossectomy.

Abstract A partial glossectomy can affect speech production. The goal of this study was to investigate the effect of the presence of a tumour as well as the glossectomy surgery on the patients' production of tongue twisters with the sounds [t] and [k]. Fifteen patients with tongue cancer and 10 healthy controls took part in the study. The outcome measures were the patients' speech acceptability, rate of errors, the time needed to produce the tongue twisters, pause duration between item repetitions and the tongue shape during the production of the consonants [t] and [k] before and after surgery…

Simultaneous Glossectomy with Orthognathic Surgery for Mandibular Prognathism.

Macroglossia can create dental and skeletal instability after orthodontic treatment or orthognathic surgery for mandibular prognathism. In relevant literature, partial glossectomy is suggested for a good post-treatment prognosis. Most of the published partial glossectomy cases are two-staged surgery, because of concern about postoperative airway obstruction. As orthognathic surgical techniques and fixation method develop, however, concerns about postoperative airway obstruction have lessened. In this case, mandibular setback surgery and partial glossectomy were performed simultaneously, leadin…

Tongue prosthesis after total glossectomy: a case report.

Squamous cell carcinoma (SCC) is the type of cancer that most frequently affects the oral cavity, mainly in men older than 50 years of age. Treatment for oral SCC often involves surgical excision of the affected margins, resulting in mutilation that affects the patient's quality of life. The objective of this case report is to describe the prosthetic treatment of a 56-year-old man who underwent total glossectomy, resulting in speech and swallowing difficulties. The proposed treatment was a tongue prosthesis retained by orthodontic clasps on the mandibular first molars. After maxillary and mand…

Long-term subjective tongue function after partial glossectomy.

There have been limited studies of subjective tongue function over long-term follow-up in spite of swallowing and articulation disorders are common complications of glossectomy. To assess long-term subjective swallowing and articulation function after partial glossectomy. A total of 63 patients with the mobile tongue cancer who underwent partial glossectomy without reconstruction were interviewed to score their swallowing and articulation function on a 100-point scale. The relation of this subjective scoring to the perioperative data was subjected to multivariate analysis. The mean patient age…

Congenital macroglossia treated by 2-stage partial glossectomy.

Persistent macroglossia affects speech, swallowing, and the dentofacial skeleton. A range of tongue reduction methods have been proposed to preserve the mobility and sensation of the tongue. However, recent reports have shown that speech or esthetic problems still exists, even after a comprehensive glossectomy in early childhood. Because a large volume reduction of the anterior tongue in young growing children might result in a short tongue tip with functional difficulties, it is important to select the appropriate tongue reduction method considering the growth of the tongue. In the current rep…

Reduction glossectomy of congenital macroglossia due to lymphangioma.

Macroglossia is a rare clinical condition defined as an enlarged tongue. Macroglossia can cause structural deformities like diastema and disproportionate mandibular growth and present functional disorders such as dysarthria, dysphonia, and respiratory problems. A 7-year-old boy who had lymphangiomatous macroglossia was treated with a reduction glossectomy by anchor-shaped combination of a U-shape and modified key-hole resection. Postoperatively, the reduced tongue was contained completely within the oral cavity, but open bite remained due to prognathism. Sensory and motor nerves to the tongue …

Indications, reconstructive techniques, and results for total glossectomy.

BACKGROUND: The best treatment for advanced tongue tumors remains controversial. Total glossectomy with laryngeal preservation is considered a feasible option, despite its morbidity. METHODS: A retrospective analysis of 12 total glossectomies with laryngeal preservation was performed in order to assess the available reconstruction techniques and analyze survival and functional outcomes. RESULTS: One-year overall survival (OS) and disease-free survival (DFS) rates were 58.3% and 33.3%, respectively, both dropping to 25% after 3 years. Permanent gastrostomy was necessary in 10 patients, althou…
