The complexity of the glottic and subglottic region in terms of anatomy and function make this region challenging in evaluation and treatment. A thorough understanding of the complex anatomy is necessary for the management of patients who have dysphonia, vocal fold paralysis, glottic or subglottic stenosis, or complications, which may present after prolonged intubation or surgical interventions in the upper airway and the thorax.

This article examines benign lesions of the glottis and adjacent structures commonly encountered in clinical practice. The goals of management are to understand the cause and the characteristics of these lesions and to provide the most conservative management possible. Nonsurgical management is selected, if possible, with surgical intervention reserved for refractory conditions. Traditional cold steel operative techniques as well as carbon dioxide laser techniques are discussed in this article.

OBJECTIVE: We characterized the normal width of the anterior commissure of the glottis by measuring its dimensions on CT scans obtained in patients who had no laryngeal disorders.
SUBJECTS AND METHODS: CT scans of patients referred for cervical CT myelography were prospectively reviewed by a single board-certified radiologist. Axial images through the anterior commissure (localized by vocal process of the arytenoid cartilage, the vocal muscle, or both) were magnified on the viewing console of the CT scanner. Window and level were chosen to simulate our routine settings for CT scans of the nec…

AIMS AND BACKGROUND: The management of patients with T1 carcinoma of the glottic larynx is controversial, because surgery and radiation therapy are reported to be effective treatments. Several studies have shown radiotherapy to be safe and effective, with a high percentage of voice preservation and minimal complications, but most Italian physicians prefer to surgically treat such patients.
METHODS: From 1980 to 1990, 36 patients with stage I squamous cell glottic carcinoma were treated with radiotherapy alone at the institute of Radiology of University of Rome "La Sapienza". In all patients t

Seven patients with verrucous carcinoma of the glottis treated by hemilaryngectomy are analyzed. The diagnostic criteria for this tumor are reviewed. Close cooperation between the clinician and pathologist is necessary for the correct diagnosis. The controversy regarding radiation therapy is discussed. Hemilaryngectomy is concluded to be a successful surgical modality for limited verrucous carcinoma of the larynx.

A personal modification to Staffieri's technique is presented. This change respects the principles of the operation but is different on the practical performance in order to avoid the incontinence of the neo-glottis. A partial decartilagination of the tracheal or crico-tracheal stump over the definitive tracheostome is performed, so that it can be funnel shaped and idoneus to the end to lateral anastomosis with the mucous membrane of the ipopharinx.
