Two cases of gonococcal urethritis in male children are presented and the records of 45 other children with gonorrhea are reviewed. The presentation, diagnosis and treatment are discussed. Data on the mode of transmission are presented with emphasis on the high incidence of gonorrhea in family members or close contacts of infected children, or a history of sexual abuse.

Gonorrhea and chlamydia rates have risen to record-high levels in the United States over the past decade. Because these infections are often asymptomatic, effective clinical management relies on screening of asymptomatic patients, particularly women younger than 25 years and men who have sex with men. If undetected and untreated, gonorrhea and chlamydia can lead to infertility, ectopic pregnancy, and chronic pelvic pain and can facilitate HIV acquisition and transmission. Primary care providers need to be aware of recent changes in recommended treatments for both infections.

Pregnancy complicated by gonorrhea was studied in 178 patients delivered of their infants over an eight-year period. There was an increased incidence of chorioamnionitis (p less than 0.05) and intrauterine growth retardation (p less than 0.05) in these patients when compared with matched controls.
