Posterior keratoconus (PKC) is a rare, typically non-inflammatory condition that is characterised by an abnormal posterior corneal curvature, which may be accompanied by overlying stromal opacification. It is usually congenital and can be associated with other ocular and systemic abnormalities. PKC remains a clinical diagnosis, although imaging techniques including ultrasound biomicroscopy and anterior segment optical coherence tomography may be useful tools for confirmation and classification. Genetic studies should be considered, although no specific genetic defects have been identified thus…

PURPOSE: To report three patients (four eyes) with hyperopic keratoconus.
METHODS: Patients were evaluated with corneal curvature topography, ultrasonic pachymetry, and rotating Scheimpflug camera.
RESULTS: One patient, without other risk factors, developed unilateral ectasia after LASIK following primary hyperopic ablation in an eye suspicious for keratoconus. Two additional hyperopic patients (three eyes) had curvature and elevation findings compatible with keratoconus.
CONCLUSIONS: Although rare, keratoconus could present in hyperopia. If keratoconus is suspected, we suggest avoidance of…

Keratoconus is a progressive, noninflammatory, nonvascular axial corneal ectasia, with unclear etiology and pathogenesis. It has a prevalence of 50-230 per 100,000 and is a bilateral disorder in up to 90% of cases. The onset of keratoconus is usually at puberty and progresses over 10-20 years producing an irregular myopic astigmatism. The treatment depends on the severity of the irregular astigmatism. Advanced keratoconus, which cannot be treated with contact lenses, requires a surgical procedure--either penetrating keratoplasty, epikeratophakia, or thermokeratoplasty. The results of penetrati…

Keratoconus is one of the most commonly seen corneal dystrophies. It manifests in the early teens, one eye becoming affected before the other. Treatment includes the use of glasses, contact lenses and, finally, penetrating keratoplasty (corneal graft). This care study highlights how an active young man with a future sporting career needed to come to terms with his eye condition.

The keratometer keratoconus index (KKI) is a diagnostic index for the risk of keratoconus calculated from autokeratometer test values. We partially modified the KKI equation and assessed it without limiting the target age and severity of keratoconus. This retrospective study included 179 eyes of 99 patients with keratoconus and 468 eyes from 235 normal controls. In the modified KKI, oblique astigmatism or against-the-rule astigmatism was defined as ≥1D astigmatism. KKI diagnostic power was analyzed in subgroups of <50 and ≥50-year-old patients, and at different keratoconus stages. Although the…

This study investigated corneal epithelial thickness and its distribution in keratoconus (KC) and forme fruste keratoconus (FFK). A total of 35 KC eyes, 25 FFK eyes, and 35 control eyes underwent anterior segment-optical coherence tomography for corneal epithelial evaluation. Sixty KC eyes were included for the assessment of epithelial changes according to KC severity. Epithelial thickness in the central, paracentral, and peripheral areas-each subdivided into eight zones-was statistically analysed. Minimum, maximum, and average epithelial thicknesses were reduced in KC eyes, especially in the …

PURPOSE: To identify the definitions used for the terms sub-clinical keratoconus and forme fruste keratoconus in published articles.
METHODS: This was a prospective, systematic literature review of the electronic database in PubMed, the Cochrane Library, and LILACS Database of all studies using the keywords "subclinical keratoconus" and/or "forme fruste keratoconus" until August 18, 2017. Two independent reviewers analyzed the data. The inclusion criteria for articles were having analyzed subclinical keratoconus or forme fruste keratoconus eyes with a sample size greater than 10 eyes; contain…

Keratoconus (KC) is an eye disease characterized by the progressive thinning and protrusion of the cornea, which results in the loss of visual acuity. This disorder remains poorly understood, although recent studies indicate the involvement of genetic and environmental factors. Recently, we have found that the distribution of the cross-linking enzyme lysyl oxidase (LOX) is markedly decreased in about 63 % of keratoconic specimens. Similarly, LOX activity is significantly reduced by 38 % compared to control tissue. Nearly 70 systemic disorders have been reported in association with KC, most of …

Hypermobility of the joints is a frequent finding in patients who have keratoconus. Twenty-two of 44 patients (50%) presenting in 1973 were found to have hypermobility of joints (mainly Ehlers-Danlos syndrome II or mitis type). The significance of this finding is discussed in relation to biochemical defects recently found in corneas with keratoconus. The hypothesis is presented that keratoconus is often a part of a generalized heritable disorder of connective tissue due to a biochemical defect causing a weakness in its structure.

PURPOSE: To analyze the biomechanical properties of the eye in patients with unilateral keratoconus with normal (forme fruste keratoconus [FFKC]) or abnormal topography (subclinical keratoconus [SKC]).
METHODS: This study included 153 eyes of 153 participants, including 95 eyes of patients with unilateral keratoconus, and 58 eyes of 58 healthy controls. Contralateral eyes with unilateral keratoconus were divided into two groups according to clinical manifestations and global consensus: FFKC (n = 30) and SKC (n = 65). The biomechanical characteristics were analyzed using non-parametric tests; …

PURPOSE: The purpose of this study was to evaluate postoperative Scheimpflug imaging changes during the first 5 years after penetrating keratoplasty (PK) in patients with keratoconus (KC).
METHODS: This retrospective, interventional case series includes 31 eyes of 31 patients who underwent their first PK with a history of KC. Postoperative Scheimpflug imaging was performed 3 months after the removal of the last suture (baseline) and then repeated 3 and 5 years after the PK. Demographic data, donor and host trephination diameter, and Scheimpflug imaging (Pentacam HR, Oculus, Germany) parameter…

Keratoconus (KC) has been traditionally classified as a noninflammatory disease. Barring loss of function, the other classic signs of inflammation (heat, redness, swelling, pain) are not usually obvious or even apparent in KC. This clinical perspective examines the evidence and implications of numerous inflammatory processes that have been recognized in the tears of KC patients as well as some inflammation relevant differences found in the KC cornea. The roles of inflammation in corneal trauma attributed to eye rubbing and/or contact lens wear are examined as is the significance of atopy, alle…

Keratoconus is a bilateral, non-inflammatory corneal ectasia characterized by progressive conical thinning and protrusion of the cornea. Its etiology has long been believed to be multifactorial, with environmental, behavioral, and genetic factors all contributing to the disease process. This review focuses specifically on examining the evidence that supports a genetic basis for keratoconus.
