Arthroscopy in acute knee injuries.

The diagnostic value of arthroscopy was evaluated in 148 patients with acute hemarthrosis and/or instability of the knee. The treatment planned after clinical examination was compared with the treatment given after arthroscopy. Seventy-nine per cent of the patients had ligamentous injuries; 59 per cent of tears were combined with other injuries, and 71 per cent were complete ruptures. Stability testing under anesthesia was most inaccurate for the anterior cruciate ligament, with 13 per cent false positive and 30 per cent false negative results. The planned treatment was altered as a consequenc

[Allotendinoplasty in knee joint injuries].

The experience of surgical treatment of 51 patients with fresh and old injuries of the knee joint is analyzed. The operation consisted in plastic of the integrity of the injured patella, extension apparatus, cruciate ligaments of the knee joint. Allogenic tendinous transplants of musculus peroneus longus, extensor of the foot toes, flexor of fingers conserved by freezing were used. Good results of the treatment were obtained in 43 patients, satisfactory-in 5 and unsatisfactory-in 3 patients.

Knee ligament injuries in children.

Nine children who were less than fourteen years old and had open physes and ligament injuries of the knee were studied. Despite an initial thorough physical and roentgenographic evaluation, the full extent of the lesion was determined only at operation in seven of the nine patients. The intercondylar eminence of the tibia was avulsed in five patients, four of whom had associated collateral ligament injuries and a positive anterior drawer sign. Despite surgical repair, all nine patients demonstrated some degree of postoperative ligament instability. This was increased in those patients in whom

Management of multiligament knee injuries.

Up to 18% of multiligament knee injuries (MLKI) have an associated vascular injury. All MLKI should be assessed using the ankle brachial pressure index (ABPI) with selective arteriography if ABPI is < 0.9.An ischaemic limb following knee dislocation must be taken to the operating theatre immediately for stabilization and re-vascularization.Partial common peroneal nerve (CPN) injury following MLKI has better recovery than complete palsy.Posterior tibial tendon transfer is offered to patients with complete CPN palsy if there is no recovery at six months.Operative treatment with acute or staged re…

[Imaging strategies for knee injuries].

Injuries of the knees are common. The Ottawa knee rule provides decisional support to determine whether radiographs are indicated or not. With the use of ultrasound it is possible to detect defects of the extensor ligaments and the anterior cruciate ligament. Furthermore, it is possible to detect indirect signs of an intra-articular fracture, e.g. lipohemarthrosis. In complex fractures, e.g. tibial plateau fractures, further diagnostic procedures with multislice computed tomography (CT) are needed for accurate classification and preoperative planning. Multislice CT with CT angiography enables

Overuse injuries of the knee.

Overuse injuries are a common cause of morbidity in athletes. They occur after repetitive microtrauma, abnormal joint alignment, and poor training technique without appropriate time to heal. Overuse injuries are frequent in the knee joint because of the numerous attachment sites for lower limb musculature and tendons surrounding the joint. MR imaging is regarded as the noninvasive technique of choice for detection of internal derangements of the knee. This article describes the characteristic findings on MR of the common overuse injuries in the knee, including patellar tendinopathy, iliotibial

[Knee injuries in general practice].

Careful technique of examination of the knee joint allows to establish the diagnosis of the most frequent knee joint injuries already through simple clinical and radiological evaluation: Recurrent dislocation of the patella with the Apprehension sign; meniscal injuries with the test of McMurray, pain on the joint line and pain on passive hyperextension; the anterior cruciate ligament tear with Lachman's sign and the Pivot Shift test and posterior cruciate injury with the Sag sign. Further imaging techniques allow to establish a definite diagnosis, which has replaced in many cases the classical

[Complex knee trauma--cartilage injuries].

Traumatic cartilage defects of the knee joint occur in an isolated way due to contusions or due to shear forces as a combined injury with ligamentous damage. They must be documented according to localisation, surface involved and depth. In the adult, only the fragment extending into subchondral bone can be refixed. Purely cartilagineous lesions heal spontaneously under partial repair with fibrocartilage; this can equally be surgically stimulated through surgical drillholes or impaction fractures with awls and pins. Since fibrocartilage is known to be mechanically insufficient and is often not

Running Injuries to the Knee.

Approximately one third of serious runners will incur an injury in a given year, and approximately one third of the injuries will involve the knee. Biomechanical studies on running reveal the tremendous cyclic forces to which the knee is subjected. There are several etiologic factors involved, including training errors, anatomic and biomechanical variations, and differences in shoes and running surfaces. Training errors, particularly rapid transitions in training, are responsible for two thirds of injuries. Knee injuries in runners must be approached with a thorough review of the training prog…

Knee dislocations with vascular injuries.

Any knee that is dislocated or that has both anterior and posterior cruciates and one collateral ligament torn should be carefully evaluated for neurologic and vascular injury. Reduction and immobilization should be performed. If the pulses are anything but normal, arrangements should be made to proceed to the operating room immediately. A one-shot arteriogram can be obtained in the operating room, but proceeding directly with popliteal artery exploration is reasonable if the pulses have not returned to normal. If the pulses have returned to normal, an arteriogram should be obtained. After the

[Open injuries of the knee].

The authors discuss the therapeutic approach to 9 cases with open injuries of the knee which they have encountered during the past 5 years. All patients were hospitalized in a state of serious traumatic shock and all had multiple injuries. All 5 cases were operated on within 2--3 hours of the injury. The authors used the following postulates in treating this type of trauma: prevention of traumatic shock, early meticulous primary surgical treatment of the wound, closure of the joint with the drainage of the synovial cavity and periarticular space, strict immobilization, reinacting the extensor

Neurologic and vascular injuries associated with knee ligament injuries.

BACKGROUND: Multiple ligament knee injuries are commonly associated with neurovascular complications such as popliteal artery, common peroneal nerve, and tibial nerve injuries. HYPOTHESIS: While the importance of identifying these complications in association with bicruciate and triligamentous injuries has been well established, it is important to recognize that any biligamentous and some single ligament knee injuries present with neurological and vascular complications as well. STUDY DESIGN: Literature review. CONCLUSION: Popliteal artery injuries require immediate intervention to help pre…

Knee injuries in the young athlete.

The rate and severity of knee injuries in the pediatric population are on the rise. In addition to more generalized knee injuries, the pediatric knee is at risk for unique injuries secondary to the rapid growth and biomechanical properties of the pediatric skeleton. Given the common nature of knee injuries, the pediatrician must be aware of these injury patterns and possess the skills to effectively evaluate the pediatric knee. This article will discuss several of the most common pediatric knee injuries.

Knee disorders and injuries in adolescents.

The range of knee injuries that adolescent athletes may sustain include minimal injury to the knee, such as contusions and mild sprains, and more serious mechanical disruptions that may lead to permanent disability if not properly treated. The author identifies injury patterns that may be seen by the clinician and offers guidelines on diagnosis and treatment.

Easily missed injuries around the knee.

Most fractures around the knee are easily detected on high-quality radiographs. However, some fractures and musculotendinous and ligamentous injuries have subtle findings and may be difficult to detect even with optimal images; these injuries include tibial plateau fractures, Segond fractures, stress fractures, fibular head fractures and dislocations, injuries to the patella and extensor mechanism, and Salter type fractures. Clinically suspected tibial plateau fractures unseen on standard views may be seen on tangential or tunnel projections. Segond fractures usually have a characteristic appe

Systematic evaluation of acute knee injuries.

The high percentage of significant injury associated with a hemarthrosis correlates with the findings of Noyes, DeHaven, and others. The development of significant knee swelling immediately after injury should be an alarming sign. Our study revealed a significant probability of anterior cruciate ligament tear or meniscal tear in the acute hemarthrosis. Arthroscopy in acute hemarthrosis certainly should be considered and, in many cases, performed. Although not within the scope of this article, acute arthroscopic repair of a torn meniscus, evaluation of the degree of tear of the anterior cruciat

[Nerve injuries of the knee joint].

Injuries of the tibial nerve in lesions of knee joint are rare, gunshot or stab wounds excluded. Injuries of the peroneal nerve happen more often and are found in fractures of the fibular head, in direct traumata and due to intensive extension to enable nailing of fractures of the femoral neck or of the femur. In open nerve injuries immediate suturing can only be done in so called razor-cut lesions and if no dehiscence or infection is present. All other nerve injuries are treated beginning from the 6th week but not later than 6 month after the accident. The same applies to all blunt nerve inju

Paediatric Knee Injuries: A Narrative Review.

The prevalence of paediatric knee injuries is increasing due to higher participation in competitive sports, larger build and more use of imaging in diagnosis. A thorough history of injury (including mechanism and symptoms of pain and instability), together with a comprehensive examination (presence of an effusion, tender areas, range of movement and joint laxity) will dictate the need for radiological investigations. Magnetic Resonance Imaging (MRI) is indicated in most paediatric patients with a history of injury and an acute knee effusion, which indicates damage to an intra-articular structu…
