Aphasia following metrizamide myelography has been reported infrequently. During a seven-month period, we examined two patients who developed Broca's aphasia, apraxia of speech, oral-buccal-facial apraxia and neurogenic stuttering after intrathecal metrizamide administration. In each case, focal neurologic deficits were accompanied by clinical, electroencephalographic and radiologic signs of generalized neurologic disease. Myelography is used to visualize the spinal cord and its nerve roots either after intrathecal (IT) injection of contrast media into the spinal subarachnoid space (SAS) or noninvasively using heavily T2-weighted magnetic resonance (MRI) sequences. Four cases of spinal cysticercosis were found in a series of 356 cases of CNS cysticercosis studied neuroradiologically. Cysticercal cysts were seen to migrate within the spinal subarachnoid space on myelographic studies. All four patients presented initially with hydrocephalus, which supports the notion that spinal leptomeningeal cysts have migrated from an intracranial site of origin. Computed tomographic myelography (metrizamide) allowed better localization and definition of the spinal cysticercal lesions.

The radiological and myelographic appearances of 14 patients with severe lumbosacral spondylolisthesis have been reviewed in relation to compression of the cauda equina and their clinical symptomatology. Patients could be separated into two groups as defined by the position of the L5 neural canal. The results of 25 functional lumbar myelographies with iohexol are reported. The image quality was good or excellent in all. The side effects were mostly mild and showed the same frequency as reported in lumbar and thoracic myelographies with metrizamide. A slight increase in the frequency of side effects was found in 13 patients with spinal repuncture 6 or 24 hours after the myelography. Vacuolized monocytes in ultracentrifuged CSF following repuncture was found in 5 patients. No EEG changes or serious complications were found. Iohexol is considered safe in intrathecal use.

Myelography describes the instillation of intrathecal contrast media for the imaging evaluation of spinal canal pathology. Lumbar myelography with iohexol (Omnipaque) was performed in 103 consecutive adult patients with low back pain or sciatica. The patients were observed for 48 h with registration of possible adverse reactions. Mild or moderate transient side effects were recorded in 24 patients. No serious adverse reactions were noted, and EEG recorded in 25 patients showed no changes.

A case with delayed development of obstructive hydrocephalus most probably caused by Pantopaque myelography is reported. At autopsy it was found that the foramina of Magendi and Luschka were occluded by granulation tissue in which the most common findings were multinucleated giant cells containing calcareous and unstained vacuoles, frequently surrounding empty hollows, many lymphocytes and plasma cells. As compared with two other similar cases and several animal investigations this shows that the reaction of the tissue is typical of a Pantopaque reaction.

Myelography was carried out on 30 patients with severe thoracolumbar injuries. In ten cases the myelography gave specific information which could not have been obtained by any other method. Myelography made within 1 month of trauma gave specific information in only one of 16 cases. Late myelography, in nine of 14 cases, provided valuable specific information about the type of cord atrophy, dorsal scar formation and transsection of the cord.

Within 20 months 145 ascending thoracocervical myelographies and 155 lumbar myelographies with the nonionic water-soluble contrast medium iopamidol were performed.
