Europe is not ready for the next pandemic. Nonetheless, several positive developments are cause for optimism. The European Centre for Diseas … s and acceptance for vaccination against the annual influenza epidemics as a means of strengthening pandemic preparedness. The Canadians are also stockpiling antiviral drugs. These drugs are an excellent way to bridge the gap between the appearance of a pandemic virus and the development of an effective vaccine against it. The World Health Organisation (WHO) is driving …

Influenza pandemics in history all caused immeasurable losses of human life and health. With climate change, population ageing, and increased population mobility due to globalization, the preparedness for influenza pandemics faces even greater challenges. To minimize the negative impacts of a pandemic, comprehensive assessments of influenza pandemic preparedness are needed. This paper introduces a series of guidances and assessment tools developed by different countries and international organizations for influenza pandemic preparedness, and compares their strengths and limitations to provide …

INTRODUCTION: Molecular diagnostics focusing on the detection and analysis of nucleic acids are indispensable tools for early pathogen identification, transmission monitoring, and genomic surveillance during pandemics. Recent technological advances have broadened the diagnostic landscape, incorporating PCR-based methods, isothermal amplification, high-CRISPR-based amplification detection, and sequencing. Despite their diagnostic potential, widespread implementation remains limited by high validation costs, time and logistical constraints, the need for specialized professional knowledge, and a

The coronavirus disease 2019 (COVID-19) pandemic demonstrated how rapidly vaccines and monoclonal antibodies (mAbs) could be deployed when the field is prepared to respond to a novel virus, serving as proof of concept that the prototype pathogen approach is feasible. This success was built upon decades of foundational research, including the characterization of protective antigens and coronavirus immunity leading to the development and validation of a generalizable vaccine approach for multiple coronaviruses. For other virus families of pandemic concern, the field is less prepared. The article

The global crisis generated by COVID-19 has heightened awareness of pandemic outbreaks. From a public health preparedness standpoint, it is essential to assess the impact of a pandemic and also the resilience of the affected communities, which is the ability to withstand and recover quickly after a pandemic outbreak. The infection attack rate has been the common metric to assess community response to a pandemic outbreak, while it focuses on the number of infected it does not capture other dimensions such as the recovery time. The aim of this research is to develop community resilience measures

Objectives: The global response to COVID-19 inherited a long history of preparedness features pertaining to various threats, including bioterrorism, (re)-emerging infectious diseases, and pandemics. We describe the evolution of pandemic preparedness frameworks, before and after the COVID-19 pandemic. Methods: We conducted an integrative literature review of publicly available documents, including grey and scientific literature, on pandemic preparedness frameworks. We relied on social science literature as a main source and used search keywords: pandemic preparedness, H1N1, COVID-19, "whole-of-…

While the development of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccines was rapid, time to development and implementation challenges remain that may impact the response to future pandemics. Trained immunity via bacille Calmette-Guerin (BCG) vaccination (an antigen agnostic strategy) offers a potential intervention against future novel pathogens via an existing, safe, and widely distributed vaccine to protect vulnerable populations and preserve health system capacity while targeted vaccines are developed and implemented.

The complexities of planning for and responding to the emergence of novel influenza viruses emphasize the need for systematic frameworks to … igating the impact of an emerging pandemic. Use of this updated framework is anticipated to improve pandemic preparedness and response in the United States. Activities and decisions during a response are event-specific. These intervals serve as a reference for public health decision-making by federal, state, and local health authorities in the United States during an i …

Influenza has not been treated with the degree of medical attention that the disease warrants. As such, there is not an adequate baseline of preparedness in the United States to deal with the potential of pandemic influenza. The National Institute of Allergy and Infectious Diseases (NIAID) has been working to enact measures to deal more effectively with a potential influenza pandemic and also to assist in the management of seasonal influenza. The majority of the NIAID's efforts have been dedicated to basic research aimed ultimately at developing and testing, in clinical trials, countermeasures

OBJECTIVES: The primary objective of this study was to determine the preparedness for pandemic influenza of hospitals, in terms of amount of antiviral drugs on hand and employee vaccination rates, in the Finger Lakes region (FLR) of western New York.
METHODS: A survey of the 17 FLR hospitals was conducted via e-mail during the period of June 2007 to August 2007.
RESULTS: A total of 13 of 17 hospitals responded for a response rate of 76.5%. Only 23.1% of responding hospitals stockpile antiviral drugs. Vaccination rates for personnel with patient contact ranged from 36.8% to 76.1%.

Disease outbreaks are increasing in frequency and complexity, disproportionately affecting children in humanitarian settings. Large numbers of zero-dose children live in conflict-affected contexts, heightening vulnerability to vaccine-preventable and emerging viral diseases. Effective pediatric vaccination and pandemic preparedness require integrated, context-adapted strategies encompassing community engagement, infodemic management, resilient delivery systems, service integration, and reliable surveillance. Innovations such as mobile teams, novel financing, and improved data coordination offe…

The increase in the incidence of avian influenza worldwide in both poultry and humans introduces the potential for another influenza A pandemic that could pose a significant threat to both human health and the global economy. The impact of the next influenza pandemic will be influenced, in part, by how well the medical, government, business, and lay communities are prepared. Despite the additional tools and resources that have become available since prior epidemics, there are limits to the quantity of antiviral drugs that can be manufactured and concerns over the current vaccine production sys

The COVID-19 pandemic revealed a critical weakness in the public health infrastructure for pandemic response: insufficient testing capacity to control the spread of disease. Overwhelmed by surging demand, testing laboratories could not keep up, leading to long turnaround times for results. This paper builds on previous research that identified constraints on laboratory capacity, to propose strategies for enhancing testing capacity with minimal investment. Three key strategies are explored: pooling specimens, flexible allocation of liquid handling machines, and flexible staff scheduling. The st

Introduction. Timely availability of effective influenza vaccine will be critical to mitigate the next influenza pandemic. The mission of Biomedical Advanced Research and Development Authority (BARDA) is to develop medical countermeasures against pandemics, including influenza and other health security threats.Areas covered. Despite considerable gains in pandemic vaccine preparedness since 2009, old and new challenges threaten the pandemic influenza response capabilities of the U.S. Government: insufficient U.S.-based vaccine production, two-dose vaccination regimen, logistically complex adjuv

Public health measures, also known as non-pharmaceutical interventions, are basic actions aimed at slowing the community spread of a communicable disease outbreak. In the event of an influenza pandemic, public health measures and antiviral drugs are the only tools available to mitigate the effects of the pandemic during the months before a vaccine becomes available. The Canadian Pandemic Influenza Preparedness: Planning Guidance for the Health Sector (CPIP) outlines how federal, provincial and territorial governments will work together to ensure a coordinated and consistent health sector appro

BACKGROUND: To identify gaps and define priority actions to strengthen national pandemic preparedness and response plans, we assessed the concordance between national plans for respiratory pathogens against the World Health Organization (WHO) pandemic checklist and the States Parties Annual Report (SPAR) in the Americas.
METHODS: In this retrospective, semiquantitative study, we reviewed the most recent respiratory pandemic plans for 35 Pan American Health Organization (PAHO) member states and assessed their concordance with (1) actionable guidelines in WHO pandemic checklist and (2) Internat…

The next pandemic may demand faster, more flexible responses with better risk-benefit and cost-benefit ratios than current systems can deliver. During COVID-19, R&D funding allocation strongly favored vaccines over antivirals and regulatory approvals of vaccines far exceeded approvals for antivirals. Moreover, early evidence on antiviral effectiveness was often limited or even misleading. Antivirals may have advantages in early outbreak control, high-risk populations, and settings with delayed vaccine uptake. For future pandemics, we propose a dedicated antiviral agenda to coordinate R&D for b
