Low Back Pain in Commercial Airline Pilots.

BACKGROUND: In their working life, airline pilots are exposed to particular risk factors that promote nonspecific low back pain (LBP). Because of the varying incidence internationally, we evaluated the point prevalences of acute, subacute, and chronic nonspecific LBP, as well as the current prevalences in German airline pilots. Furthermore, we compared the prevalence to the general German population and to European counterparts.METHODS: An anonymous online survey of 698 participating German airline pilots was evaluated. The impairment between groups was analyzed. Prevalences from our data were

Recurrent Middle Ear Barotrauma in Student Pilots.

INTRODUCTION: The present study evaluated the efficacy of the nine-step inflation-deflation tympanometric test for predicting recurrence of middle ear barotrauma.METHODS: Student pilots who were diagnosed with middle ear barotrauma from October 2010 to April 2011 were enrolled. The grade of barotrauma was assigned using Teed's classification. All subjects underwent tympanometry and nine-step inflation-deflation testing at first visit and after improvement. Recurrence was monitored for 2 wk after they resumed flight. The relationships among the recurrence of middle ear barotrauma, the grade and

Incidence of colorectal neoplasms among male pilots.

AIM: To assess the prevalence of colorectal neoplasms (adenomas, advanced adenomas and colorectal cancers) among Israeli military and commercial airline pilots. METHODS: Initial screening colonoscopy was performed on average-risk (no symptoms and no family history) airline pilots at the Integrated Cancer Prevention Center (ICPC) in the Tel-Aviv Medical Center. Visualized polyps were excised and sent for pathological examination. Advanced adenoma was defined as a lesion >10 mm in diameter, with high-grade dysplasia or villous histology. The results were compared with those of an age- and gende

Elevated bilirubin and visual perception in pilots.

BACKGROUND: A fatal aircraft accident of a Bo-105 helicopter on May 20, 1996 led to the suspicion that the pilot's raised serum bilirubin level could have contributed to the accident. It was suspected that an increase of bilirubin beyond 20.52 micromol x L(-1) could disturb visual perception. METHODS: In 1998 and 1999, 101 pilots with functional hyperbilirubinemia, diagnosed as Gilbert's syndrome, were subjected to perceptual-psychological testing (signal detection test) during their periodic medical examinations at the German Air Force Institute of Aviation Medicine. The results of the psych

Mortality among US commercial pilots and navigators.

The airline industry may be an occupational setting with specific health risks. Two environmental agents to which flight crews are known to be exposed are cosmic radiation and magnetic fields generated by the aircraft's electrical system. Other factors to be considered are circadian disruption and conditions specific to air travel, such as noise, vibration, mild hypoxia, reduced atmospheric pressure, low humidity, and air quality. This study investigated mortality among US commercial pilots and navigators, using proportional mortality ratios for cancer and noncancer end points. Proportional ca

Spondylolithesis in pilots: a follow-up study.

There were 21 pilots followed for 12-131 months in order to determine the natural history of spondylolithesis (SLL). Of these 21, 16 had follow-up X-ray examinations, and only 1 was found with significant progression of the posterior vertebral displacement. Of the 12 pilots with SLL and low back pain (LBP), 4 had recurrent single episodes of acute LBP, but all remained active and continued to fly over the follow-up period. None of the 9 pilots who had SLL discovered on routine X-ray examination developed LBP over the follow-up period. We conclude that pilots with SLL can continue to fly with m

Heat stress exposure of aerial spray pilots.

Heat stress (WBGT index) in the cockpits of agricultural spray pilots, and its physiological and subjective effects, were measured for 9 pilots flying in hot weather for a large Israeli aerial spray company. There was concern that heat exposure may be one of the factors involved in a recent increase in the fatal and non-fatal crash rate among Israeli spray pilots. WBGT index calculations were based on sequential cockpit wet bulb, dry bulb, and globe temperatures read when aircraft landed. The WBGT was always above 25 degrees C and exceeded 26.7 degrees C in 70% of observations. In pilots, a da

Impairment of flying efficiency in anancastic pilots.

Previous work on the effects of stress upon flying efficiency has shown two types of psychological stress to exist; these are usually referred to as cognitive and affective stress, respectively. A refinement in affective stress analysis has enabled two subtypes of stress to be demonstrated--one is characterized by activity of the sympathetic nervous system (SNS) and subjective feelings of rage, fear, anxiety, or frustration; the other by excessive rumination. These two subtypes are found in differing personality configurations. Ten pilots exhibiting the former type of stress, and 6 with the la

Polycythemia Prevalence and Risk Factors in Pilots.

INTRODUCTION: Pilots are frequently exposed to thrombotic risk as a result of immobility from air travel. As hypoxemia is associated with secondary polycythemia, and polycythemia increases the risk of thrombosis, intermittent exposure to high-altitude hypoxic environments could escalate the risk of thrombosis in pilots. Our objectives were to find the prevalence of polycythemia in airplane pilots (primary outcome) and to assess associated risk factors of polycythemia (secondary outcome).METHODS: This study is a cross-sectional descriptive study. Data was collected from paper-based and computer

Chiari-I malformation in two fighter pilots.

This report describes two cases of Chiari Malformation Type I (Chiari-I) in fighter pilots of the Turkish Air Force. Chiari-I is a congenital malformation characterized by herniation of cerebellar tonsils through the foramen magnum. Patients have symptoms and signs related to dysfunction of the brainstem, spinal cord, and cerebellum. They generally are symptomatic in the earlier years of life. However, asymptomatic cases can eventually become symptomatic in later years. Symptoms can be provoked by increasing intracranial pressure (Valsalva or straining). We report on two pilots with Chiari-I m

Behavioral airsickness management program for student pilots.

The management of recurrent airsickness in student pilots has traditionally involved positive reinforcement of motivation and limited use of medication. The Behavioral Airsickness Management (BAM) program seeks to establish an effective behavioral and cognitive intervention format for managing airsickness symptoms. There were 37 student pilots experiencing recurrent airsickness who were exposed to an assessment and rehabilitation program designed to develop diaphragmatic breathing skills while rapidly reducing physiological tension via cue-evoked relaxation strategies. Cognitive modification t

Attention Regulation Among Sleep-Deprived Air-Force Pilots.

Short sleep duration is associated with adverse physical and mental events. However, it is quite challenging to objectively quantify its impact on human cognitive performance. Thus, we aim to examine the effects of sleep deprivation on physiological measures of emotional and attention regulation, in terms of recovery dynamics among sleep-deprived air force pilots. Ninety-one pilots participated in a sleep deprivation workshop held by an aeromedical center. The Auditory Sustained Attention Test (ASAT) and the Psychomotor Vigilance Test were applied to evaluate emotional and attention performanc

Cardiovascular risk profiles in German Air Force pilots.

INTRODUCTION: Cardiovascular disease (CVD) is the leading cause of death in western industrial countries and one of the most frequent causes of sudden incapacitation in flight for pilots. There are limited data available on cardiovascular risk profiles of pilots, and especially military pilots. The aim of this study was to assess the prevalence of cardiovascular risk factors (CVRF) in German military pilots. METHODS: The changing prevalence of CVRF in active military pilots was studied using a cross-sectional survey during two distinct periods, 2007-2009 and 2016-2018. Data collected included

Shared responsibility for managing fatigue: Hearing the pilots.

In commercial aviation, fatigue is defined as a physiological state of reduced mental or physical performance capability resulting from sleep loss, extended wakefulness, circadian phase, and/or workload. The International Civil Aviation Organisation mandates that responsibility for fatigue risk management is shared between airline management, pilots, and support staff. However, to date, the majority of research relating to fatigue mitigations in long range operations has focused on the mitigations required or recommended by regulators and operators. Little research attention has been paid to t

Study of occupational stress among railway engine pilots.

BACKGROUND: Traffic volume and speed is going to be increased in Indian Railways successively, leading to higher stress in staff connected with train operations. The jobs of railway engine pilots come under the category of high-strain jobs, necessitating a need to conduct multicentric study to unfold the factors associated with occupational stress and organizational strategies. MATERIALS AND METHODS: Present study covered 185 railway engine pilots and office clerks working in various railway zones by incidental method. Occupational Stress Index (OSI) test developed by Srivastva and Singh, que

Predictors of skin cancer in commercial airline pilots.

BACKGROUND: Skin cancers among commercial airline pilots have been reported to occur at increased rates in pilot populations worldwide. The reasons for these increases are unclear, but postulated factors include ionizing radiation, circadian disruption and leisure sun exposure. AIMS: To investigate the potential association of these occupational and lifestyle factors, as well as medical history and skin type, with non-melanoma skin cancer in pilots. METHODS: Data were collected using a confidential Internet survey administered in collaboration with the Air Line Pilots Association Internation

Occupational exposure to noise and hypertension in pilots.

The role of occupational exposure to noise as a hypertension risk factor has not been established sufficiently. The aim of the study is to evaluate whether chronic exposure to different levels of noise in two groups of pilots, operating with two types of aircraft, could be a risk for hypertension, what relevance the parameters (intensity, duration and type) of exposure can have and, lastly, whether there are any links between hearing impairment and hypertension. After excluding pilots with confounding factors, a study was made of 77 male pilots of turboprop planes (group A) and 224 male pilots

[Observation of electronystagmogram in pilots under vestibular stimulation].

The relationship between electronystagmogram(ENG) and intensity of vestibular stimulation was observed in 62 pilots. 31 pilots received low intensity vestibular stimulation and the other 31 pilots received higher intensity vestibular stimulation. Electronystagmogram were recorded and analysed. The results showed that there was no relationship between the ENG parameters and stimulation intensity, but both slow phase and fast phase velocities and amplitudes were lower under Coriolis acceleration stimulus, while all of them were higher under angular acceleration and angular stimulations. It was a

[Pilots with diabetes mellitus: cleared for take-off?].

In this issue, we outline the developments that have increasingly enabled people with insulin-dependent diabetes to professionally operate a vehicle. We focus on all professions in passenger transport in the Netherlands, with the pilot as a reference. A protocol has been developed in the UK to enable safe and responsible flying by selected pilots with type 1 diabetes mellitus and insulin dependent type 2 diabetes mellitus. It is used in several countries within and outside Europe, but not yet in the Netherlands. Modern diabetes care, innovative monitoring, good medical supervision and support
