The initiative described here aims to identify quality indicators (QIs) germane to the international practice of primary care providers (PCP) in post-acute and long-term care in order to demonstrate the added value of medical providers in nursing homes (NHs). Primary pulmonary hypertension (PPH) is a rare entity of unknown etiology, manifested as narrowing and obstruction of the pulmonary arteries leading to elevated pulmonary artery pressures. Left untreated, these high pressures can lead to right ventricular dilation, right heart failure, and death within 3 to 5 years. Over the last decade, advances have been made in the diagnosis and treatment of PPH. Advanced practice nurses and other expert nurses can use and interpret diagnostic and therapeutic medical regimens currently available to identify and treat this rare condition.

The main aim of the study was to investigate the effectiveness of multidisciplinary guidelines in promoting the mobility of people with dementia in 20 German nursing homes. The study was based on a semi-experimental design [pre-post design with intervention (IG) and control group]. The statistical analyses revealed a significantly slower decline of the ability to walk among the residents of the IG than among the controls.
