The optimal angle of the mediolateral episiotomy at crowning of the head during labor.

INTRODUCTION AND HYPOTHESIS: The aim of the mediolateral episiotomy incision is to increase the diameter of the soft tissue of the vaginal outlet to facilitate birth and to prevent vaginal tears. Episiotomy angles that are too narrow and close to the midline increase the risk of obstetric anal sphincter injuries. In order to determine the optimal angle of the episiotomy, we assessed the changes in the angles of episiotomy lines marked during the first stage of labor and measured at the time of crowning of the head. METHODS: Incision lines for mediolateral episiotomy were marked on the perinea

Crown-root Fracture Restoration on a Patient with Autism Spectrum Disorder.

Children with intellectual and physical disabilities including autism are susceptible to dental trauma as a sequela from falls due to poor muscular coordination. In addition, their altered muscle tonus often results in an open bite with labial flaring of the maxillary incisors and lip incompetence, predisposing these teeth to fractures. This case report describes an alternative approach of restoring a fractured maxillary permanent central incisor with a composite strip crown during surgical repositioning of the periodontium on an autistic patient. The prognosis of the incisor is guarded due to

Crowned Dens Syndrome: Report of Three Cases and a Review of the Literature.

BACKGROUND: Patients with crowned dens syndrome (CDS), which is pseudogout of the atlantoaxial junction induced by "crown-like" calcifications around the dens, present with symptoms of severe neck pain, rigidity, and high fever. CDS patients are often misdiagnosed as having meningitis or polymyalgia rheumatica, leading to potentially unnecessary invasive procedures for diagnosis and treatment. CASE REPORT: We report 3 patients with CDS who had characteristic findings on computed tomography (CT), all of whom quickly recovered with nonsteroidal antiinflammatory drug (NSAID) administration. In a

An update on crown lengthening. Part 1: Gingival tissue excess.

This is the first article in a two-part series which aims to provide an overview of the different techniques used to increase clinical crown height. In the first paper, the focus will be on the management of patients who present with gingival tissue excess. The different aetiologies are discussed and illustrated with clinical cases, following which a range of procedures that may be employed in the management of these patients are presented. With an increasingly ageing population, more patients are taking regular medications prescribed from their general medical practitioner, and so having a wo

Crowned dens syndrome diagnosed on ¹⁸F-FDG PET/CT.

An 87-year-old woman with corticosteroid-resistant polymyalgia rheumatica underwent ¹⁸F-FDG PET/CT for suspected giant cell arteritis or neoplastic disease. FDG uptake in the immediate vicinity of the odontoid process, with a crownlike calcification, was identified on the CT scan on the posterior side of the dens, thus confirming the diagnosis of crowned dens syndrome. Because this rare syndrome is frequently misdiagnosed, nuclear physicians should be aware of the signs and symptoms of this condition, which may call for the use of PET/CT imagery.

A rare case of crowned dens syndrome mimicking aseptic meningitis.

BACKGROUND: Crowned dens syndrome (CDS), related to microcrystalline deposition in the periodontoid process, is the main cause of acute or chronic cervical pain. Microcrystal-line deposition most often consists of calcium pyrophosphate dehydrate crystals and/or hydroxyapatite crystals. CASE PRESENTATION: This report describes the case of an 89-year-old woman who presented with sudden onset, high fever, severe occipital headache, and neck stiffness. A laboratory examination revealed a markedly elevated white blood cell count (11,100/µl) and C-reactive protein level (23.8 mg/dl). These clinical

Treatment of atoxoplasmosis in the Blue-crowned Laughing Thrush (Dryonastes courtoisi).

Passerines are frequently parasitized by coccidia, especially species of the genus Isospora, with extra-intestinal stages that can be highly pathogenic causing serious clinical damage in young birds. Whilst there is still no effective treatment to completely clear isosporoid coccidia with extra-intestinal stages from a host species, our results showed that prolonged treatment with toltrazuril (BAYER AG, Leverkusen, Germany) can decrease the oocysts in faeces and thus reduce the extra-intestinal phase of the infection. The toltrazuril treatment is therefore probably indirectly effective against

CPPD crowned dens syndrome with clivus destruction: a case report.

We report a case of CPPD crowned dens syndrome in an 87 year white old male with a known history of pseudogout, with clinical and radiological features characteristic of this syndrome. Interestingly, there was significant mass effect on the clivus, with clivus erosion and destruction, a finding that has not previously been described with this syndrome. The clinical and radiological characteristics of Crowned Dens syndrome, as well as CPPD are reviewed. We suggest that CPPD crowned dens syndrome may be included in the differential diagnosis when clivus destruction or erosion, in association wit

Esthetic crown lengthening: appropriate diagnosis for achieving gingival balance.

A gummy smile poses a restorative challenge for dentists attempting to achieve ideal esthetics. Many have advocated the use of a gingivectomy or laser-assisted gingival contouring procedure. However, this simplistic approach can potentially create a mucogingival defect or a biological width violation. To avoid these periodontal-restorative complications, it is important during treatment planning to assess the anatomical relationship that resulted in the gummy smile and choose the appropriate surgical treatment to eliminate this condition.

Replantation of three avulsed permanent incisors with complicated crown fractures.

AIM: To report a rare case of multiple avulsions of permanent teeth associated with complicated crown fractures. SUMMARY: The case describes a 3-year follow-up of the treatment of three avulsed maxillary incisors with complicated crown fractures. The teeth were replanted after an extra-oral time of 90 min (extra-oral dry time 6 min). After root canal treatment, the fractured maxillary incisors were restored with a layered hybrid composite material to re-establish aesthetics and function. Follow-up visits showed signs of replacement resorption affecting the maxillary central incisors but only

Cervicofacial and mediastinal emphysema after crown preparation: a rare complication.

Cervicofacial emphysema is a well-known complication associated with the use of a high-speed air turbine. However, retropharyngeal emphysema or pneumomediastinum in the absence of severe trauma is rare. Immediate recognition of this phenomenon is essential to avoid life-threatening complications such as potential airway obstruction or venous air embolism. This article reports a case of gas diffusion into the mediastinum after dental preparatory treatment for a single-tooth crown. Pneumomediastinum should be considered whenever there are anamnestic data for retrosternal chest pain accompanied b

Variation in crown and root formation and eruption of human deciduous teeth.

The aim of this study was to document variation of deciduous tooth formation and eruption. The material comprises 121 individuals of known or estimated age (using tooth length) from Spitalfields in London, and radiographs of 61 healthy living children aged 2-5 years. Other skeletal material from two medieval Scottish archaeological sites (Whithorn, N=74; Newark Bay, N=59) was also examined. Stages of crown and root formation as well as eruption (alveolar, midway, and occlusal levels) were assessed for each developing maxillary and mandibular tooth from radiographs or direct vision. Age of atta

Consequences of crown fractures with pulpal exposure: histopathological evaluation in dogs.

When a crown fracture involving pulpal exposure is produced, the therapeutic treatment to be applied depends to a great extent on the general histopathological condition of the exposed pulp. Hence, the objective of this study was to evaluate histopathological and bacteriological changes occurring in dental tissue and periradicular tissue of crown-fractured teeth with pulpal exposure. Twenty-four anterior teeth (central and lateral incisors) from the maxillary teeth of four young, adult Mongrel dogs were used. At 48 and 72 h after performing the crown fractures, the animals were sacrificed and

Surgical extrusion of crown-root-fractured teeth: a clinical review.

AIM: In this clinical study combined surgical and endodontic treatment was performed in 20 cases of crown-root fracture and the outcomes reviewed. METHODOLOGY: Surgical treatment involved a conventional extraction and stabilization technique. Root canal treatment using calcium hydroxide was performed. Before root canal obturation, a calcium hydroxide dressing was maintained for 3 months. RESULTS: Follow-up examinations, which varied between 6 and 36 (mean 14.5) months, showed that there were no radiographic and clinical signs of progressive root resorption, marginal bone loss or periapical d

Crown morphology, enamel distribution, and enamel structure in mouse molars.

BACKGROUND: Biomolecular research and genetic manipulations have stressed the importance of thorough knowledge of normal organ morphology. Mouse molar teeth are convenient models for studying basic interactions in organ development and morphogenesis. The aim of the present study was to provide basic information on their morphology. METHODS: Intact and sectioned/ground molars of mice of various ages were observed with SEM. RESULTS: Enamel-free areas (EFA) were present on cusp tips at time of eruption. The dominating structural configuration in enamel was prism decussation in inner enamel and

Attractive magnets for orthodontic extrusion of crown-root fractured teeth.

A subgingival crown-root fracture presents the clinician with a difficult restorative problem, including reaching the fracture line, and is complicated by the need to maintain the periodontal tissues in good health. The treatment options up to now have usually been limited to extrusion of the remaining root with a conventional orthodontic appliance, surgical intraalveolar transplantation of the root or extraction with bridge replacement. In this report, a new method of orthodontic extrusion with attractive magnets is presented. One or two neodymium-iron-boron magnets were attached to the remai
