Improving Decreased Heater-Cooler Efficiency as a Result of Heater-Cooler Infection Control Strategy.

Heater-cooler units (HCUs) play a vital role in temperature management during cardiopulmonary bypass. In recent years, HCUs have been shown to play a significant role in the propagation of bacteria causing patient infection and significant harm. As a result, various institutions across the world have begun moving the HCU either far away or outside of the operative theater entirely. The purpose of this study was to examine the effect that the increased length of HCU water lines have on the ability of the device to heat and cool. We hypothesized that the increase in water line distance leads to

Assessing maintenance of evaporative cooling systems in legionellosis outbreaks.

This study was designed to conduct systematic reviews of existing evaporative cooling system maintenance guidelines and of published Legionnaires' disease outbreaks to determine what, if any, maintenance practices were in place at the time of the disease outbreaks and then to contrast the reported practices with the published guidelines for evaporative cooling systems. For the first review, similarities in the reported recommendations were assessed; in the second review, any reported information about the state of the evaporative cooling system during the outbreak investigation was summarized.

Identification of clinical isolates of nondiphtherial Corynebacterium species and their antibiotic susceptibility patterns.

Starting in 1982, our laboratory has performed species identification of coryneform bacteria isolated from blood cultures, intravenous (i.v.) catheter tips and sites, urines with high colony counts, and other potentially significant cultures, using predefined criteria. Of 283 isolates identified, Corynebacterium jeikeium was the most common (47%), followed by CDC group G2 (12%) and C. minutissimum (8%). Blood cultures and i.v. catheter-related sources were the most frequent sources (58% of total). Certain species or groups, like CDC group G2, were most frequently isolated from blood or i.v. ca

Moisture ingress in a hospital medical air system secondary to dryer failure leading to service disruption.

Medical air is essential for the safe delivery of anaesthesia, critical care and emergency care. We report a hospital-wide medical air contamination event caused by failure of ageing desiccant dryers. Water was detected in the intensive care unit medical air outlets, before moisture contamination was confirmed within the medical air pipeline system and operating theatre pendants. This resulted in shutdown of the central medical air plant, suspension of non-urgent surgery, surgical bypass and a 5-day disruption to normal hospital services. The principal clinical challenges were the loss of reli

Clinical impact of disposable elevator-cap duodenoscopes on post-endoscopic retrograde cholangiopancreatography cholangitis: a prospective cohort study.

BACKGROUND AND AIMS: Disposable elevator-cap (DEC) duodenoscopes were developed to reduce residual microbial contamination after reprocessing and potentially lower infection transmission. We compared clinical outcomes of DEC-equipped and conventional duodenoscopes in routine practice, focusing on post-ERCP cholangitis and multidrug-resistant (MDR) pathogen-associated cholangitis. METHODS: This prospective, single-center observational cohort study included consecutive adults undergoing ERCP with either DEC-equipped or conventional duodenoscopes. The primary outcome was MDR pathogen-associated

Comparative analysis of ampoules and vials in sterile and conventional packaging as to microbial load and sterility test.

OBJECTIVE: To compare sterility and microbial (bacteria and fungi) load in the outer part of hyperbaric bupivacaine (Neocaína®) in ampoule and bupivacaine in vial, in conventional and sterile pack formulations. METHODS: The sterile packs were divided into two groups: G1 (n=16) with ampoules and G2 (n=16) with vials. Conventional formulations were divided into two groups, being G3 (n=16) with ampoules and G4 (n=16) with vials. The ampoules and vials were opened and had their content drawn. The empty bottles were then placed in sterile plastic bags and sent for analysis of microbial load (bacte

Staying proper with your personal protective equipment: How to don and doff.

INTRODUCTION: The global COVID-19 pandemic highlighted the importance of protecting frontline healthcare workers from novel respiratory infections while also exposing the limited instruction that medical students receive on proper donning of personal protective equipment (PPE) and more importantly the safe doffing of contaminated PPE to minimize their risk of nosocomial infection. The best methods of providing this kind of instruction have not yet been determined. METHODS: Anesthesiology interns and CA-1 residents were trained on proper PPE donning and doffing for AGPs using a methodology bas

Dental unit waterlines in Israel: evidence of widespread microbial contamination and a need for national regulation.

BACKGROUND: Dental unit waterlines (DUWLs) are susceptible to microbial contamination because of biofilm formation within their complex water delivery systems. Although the US CDC and several professional organizations recommend maintaining dental treatment water at potable water quality, Israel currently lacks specific national standards and regulatory requirements for monitoring and maintaining DUWLs. This study assessed the prevalence of Legionella and other microbial contaminants in DUWLs across dental clinics in central Israel and examined factors associated with contamination to inform p

Current practice patterns for cleaning tonometry tips and diagnostic or laser lenses.

In an online survey of >1000 ophthalmologists, most reported using alcohol wipes or swabs to clean tonometry tips (73%) and diagnostic (63%) or laser (50%) contact lenses. Use of disposable devices or high-level disinfection (HLD) was uncommon and driven largely by facility policies or regulatory requirements. Only 19% of those using HLD did so because they believed it to be safer. Ocular surface toxicity and device damage were commonly seen with HLD. Reports of infection transmission were exceedingly rare, with 99% indicating they had never encountered a case. More than 75% of respondents wer

Biofilms and device-associated infections.

Microorganisms commonly attach to living and nonliving surfaces, including those of indwelling medical devices, and form biofilms made up of extracellular polymers. In this state, microorganisms are highly resistant to antimicrobial treatment and are tenaciously bound to the surface. To better understand and control biofilms on indwelling medical devices, researchers should develop reliable sampling and measurement techniques, investigate the role of biofilms in antimicrobial drug resistance, and establish the link between biofilm contamination and patient infection.
