Aim of this study was to investigate the relationship between the severity of psoriasis and obesity based on the analysis of the visceral fat index and serum levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and resistin. The study included 50 patients with psoriasis and 30 subjects in the control group. A patient with a history of alcoholism and pancreatic calcification, developed subcutaneous fat necrosis and an arthropathy, associated with a pancreatic pseudocyst and accompanied by an elevation of serum lipase and amylase. The illness was complicated by bacteremia and destructive bone lesions. A clinical distinction between osteomyelitis and medullary fat necrosis proved difficult. Infection of bone was demonstrated at one site but did not exclude medullary fat necrosis elsewhere.

Metastatic fat necrosis due to inflammatory or neoplastic pancreatic diseases is rare. This phenomenon is attributed to systemic effects of pancreatic enzymes. Depending on the sites of fat necrosis, a number of different diseases may be mimicked, leading to incorrect diagnosis and therapies. Many case reports describe the phenomenon of skin, joint and bone manifestations of fat necrosis under the acronym PPP (pancreatic, panniculits, polyarthritis) syndrome. The management of "autodigestion" primarily consists of treating the underlying pancreatic disease.

An 8-year-old female Poodle had severe necrotizing skin disease characterized by numerous large red-purple, nonpruritic lesions. The larger lesions had soft, intensely discolored centers that contained sterile viscous blood-tinged material. Biopsies of the lesions revealed necrotizing panniculitis (subcutaneous fat necrosis). The dog did not respond to intensive therapy and euthanasia was performed. Necropsy findings included diffuse fat necrosis and pancreatic nodular hyperplasia. A pathoetiologic relation between pancreatic disease and diffuse fat necrosis in this dog was suggested.

A 46-year old man developed subcutaneous fat necrosis of the right upper thigh in association with pancreatitis. The panniculitis established by biopsy resolved 4 years later. Subcutaneous nodular fat necrosis is a rare dermatological manifestation of pancreatitis or pancreas carcinoma which may be clinically indistinguishable from erythema nodosum. Biopsy of the skin lesion reveals a characteristic histological picture with necrotic fat cells surrounded by lymphocytes, histiocytes and plasmacells.

Intra-abdominal fat is abundantly present in both the peritoneum and retroperitoneum. Fat necrosis or inflammation are common findings in abdominal imaging. The most common pathologies that we encounter are epiploic appendagitis, omental infarction, mesenteric panniculitis, and encapsulated fat necrosis. Less common entities that can occur are pancreatic saponification, heterotopic mesenteric ossification, and pseudolipoma of the capsule of Glisson. These entities can mimic more urgent pathologies such as appendicitis, diverticulitis, or malignancies.
