[Health cooperation under the "Lancang-Mekong Cooperation 2.0" framework: a decade of retrospection and prospective outlook].

Since the launch of the Lancang-Mekong Cooperation in 2015, a core platform for regional health governance has been established. This paper systematically deconstructs 28 multilateral official documents issued between 2015 and 2024 by using policy text analysis to trace the logical trajectory of Lancang-Mekong health cooperation's development from project-driven initiatives to institutionalized governance. The study concludes that major public health emergencies within the region is the key factor to improve the institutional development and resource allocation of the system. This has expanded

Charting the path to the implementation of universal health coverage policy in Nigeria through the lens of Delphi methodology.

BACKGROUND: Expanding access to equitable health insurance is an important lever towards the overall strategy for achieving universal health coverage. In Nigeria, health insurance coverage is low with a renewed government action on increasing access to and coverage of high-quality healthcare services to citizens, particularly for the vulnerable and poor population. Therefore, our study co-creates the priorities for expanding health insurance in Nigeria, focusing on key policy reforms, public advocacy, and innovative financing strategies to ensure broader and more equitable coverage for the pop

Designing care at home as an integrated continuum: insights from Singapore and application to U.S. health systems.

Many U.S. hospital-at-home programs operate primarily as substitutes for inpatient admission. Singapore has instead pursued a nationally coordinated care-at-home framework: Healthier SG re-anchors chronic disease management in primary care, while MIC@Home mainstreams acute hospital care at home. We examine the governance, financing, workforce, and digital infrastructure designed to connect these programs, and draw design insights for U.S. health systems building integrated care-at-home continuums.

Sustainable surgical financing through philanthropy: an analysis of contributions to low- and middle-income countries (LMICs).

BACKGROUND: Surgery is an indispensable component of a health system, yet financing its delivery faces significant financing challenges. This study seeks to address the research question: What is the financial contribution of philanthropy to the delivery of surgical care in low and middle-income countries (LMICs)? METHODOLOGY: This study combines a qualitative and a quantitative component to examine the magnitude of philanthropic contributions in surgical care. The qualitative analysis is composed of a narrative review of peer-reviewed and gray literature that was conducted and synthesised th

Estimating the government public economic benefits attributed to investing in assisted reproductive technology: a South African case study.

Limited resources and high treatment costs are arguments often used in many public health systems in low- and middle-income countries to justify providing limited treatments for people with infertility. In this analysis, we apply a government public economic perspective to evaluate public subsidy for in-vitro fertilization (IVF) in South Africa. A fiscal model was developed that considered lifetime direct and indirect taxes paid and government transfers received by a child conceived by IVF. The model was constructed from public data sources and was adjusted for mortality, age-specific educatio

A review of tensions and risks in naturopathic education and training in Australia: a need for regulation.

OBJECTIVES: In line with increasing complementary medicine (CAM) use, the Australian government has committed considerable resources to the training of CAM practitioners. However, it has generally failed to complement this support with regulation or accountability measures. This is particularly true in Australia's largest CAM profession (naturopaths), which remains entirely unregulated but attracts approximately AUD$40 million each year in government funding for its education sector. This article explores the consequences of such unfettered support on professional outcomes. DESIGN: Data on Au

Beyond dependency: Ethical health sovereignty and global justice after the pandemic treaty.

The inequities observed during the COVID-19 pandemic were not an accident. They reflect deeper patterns of injustice already embedded in rules, financing models and power arrangements in global health governance. This paper argues that the paradigm of ethical health sovereignty offers a moral countermodel to the dependency shaping global health relations. It examines how sovereignty, solidarity and justice are being reinterpreted after the pandemic by drawing on the WHO Pandemic Agreement (A78/10, 2025) and Africa's regional responses through the African Union and Africa Centres for Disease Co

The Kerala State Elderly Commission: preventive governance and the protection of older persons in India.

Population aging is transforming the institutional requirements of elder protection in India. Kerala's establishment of the Kerala State Elderly Commission under the Kerala State Elderly Commission Act, 2025, creates a specialized statutory mechanism for protecting the welfare, rights, and dignity of older persons. This article examines the Commission as an emerging instrument of preventive governance, focusing on its capacities for inquiry, inspection, coordination, legal-aid facilitation, policy guidance, and institutional oversight. Preventive governance is conceptualized through three dime

Can health taxes help sustain Africa's HIV response and research agenda? Potential fiscal space, implementation risks, and equity considerations.

Sub-Saharan Africa's HIV response is under acute financing stress as external assistance becomes less predictable, debt service costs displace public expenditure, and governments face growing demands on health and community systems. Health taxes on tobacco, alcohol, and sugar-sweetened beverages are increasingly proposed as one instrument of domestic resource mobilisation, but their contribution to HIV sustainability is contested, and their design space in African contexts is imperfectly characterised. The present paper asks under what conditions health taxes can contribute to sustaining HIV s

How Is CDC Funded to Respond to Public Health Emergencies? Federal Appropriations and Budget Execution Process for Non-Financial Experts.

The federal budgeting process affects a wide range of people who work in public health, including those who work for government at local, state, and federal levels; those who work with government; those who operate government-funded programs; and those who receive program services. However, many people who are affected by the federal budget are not aware of or do not understand how it is appropriated or executed. This commentary is intended to give non-financial experts an overview of the federal budget process to address public health emergencies. Using CDC as an example, we provide: (1) a br

Cost savings from the provision of specific methods of contraception in a publicly funded program.

OBJECTIVES: We examined the cost-effectiveness of contraceptive methods dispensed in 2003 to 955,000 women in Family PACT (Planning, Access, Care and Treatment), California's publicly funded family planning program. METHODS: We estimated the number of pregnancies averted by each contraceptive method and compared the cost of providing each method with the savings from averted pregnancies. RESULTS: More than half of the 178,000 averted pregnancies were attributable to oral contraceptives, one fifth to injectable methods, and one tenth each to the patch and barrier methods. Government-donor-academia collaboration model in the pharmaceutical regulatory system and considerations for sustainability in low- and middle-income countries.

PURPOSE: This study assesses how collaboration between Lebanon's Ministry of Public Health (MoPH), Agence Française de Développement (AFD) and ESA Business School (ESA) strengthened the pharmaceutical regulatory framework during severe crises. It reviews the design and outcomes of reforms in governance, pharmacovigilance and quality assurance; identifies key enabling and limiting factors and draws lessons for other low- and middle-income countries (LMICs). Family medicine in Iran: the birth of a new specialty.

Since the revolution of 1978-1979, the government of Iran has worked toward development of a primary health care system to improve basic health for its citizens. Although infant mortality and other parameters have improved, increasing urbanization and poor lifestyle choices continue to present major challenges to improving overall health statistics in the country. Generalist physicians, with no training beyond medical school graduation, have not inspired confidence from patients or specialist colleagues. Therefore, many patients prefer to receive care for common health complaints from speciali

The challenges of reshaping disease specific and care oriented community based services towards comprehensive goals: a situation appraisal in the Western Cape Province, South Africa.

BACKGROUND: Similar to other countries in the region, South Africa is currently reorienting a loosely structured and highly diverse community care system that evolved around HIV and TB, into a formalized, comprehensive and integrated primary health care outreach programme, based on community health workers (CHWs). While the difficulties of establishing national CHW programmes are well described, the reshaping of disease specific and care oriented community services, based outside the formal health system, poses particular challenges. This paper is an in-depth case study of the challenges of im

Healthcare financing in Morocco on the path towards universal health coverage: a protocol for a scoping review.

INTRODUCTION: All countries around the world have been encouraged to implement universal health coverage, emphasising its essential role in global health policy. Morocco has long been part of this international movement. Several health reforms have been implemented over the years, significantly improving the health status of the population and service coverage. However, several gaps continue to hinder the expected results. The current health financing model in Morocco is one of the main persistent challenges. It remains largely dependent on out-of-pocket expenditure, with no significant change

Alternative financing mechanisms for ART programs in health facilities in Uganda: a mixed-methods approach.

BACKGROUND: Sub-Saharan Africa is heavily dependent on global health initiatives (GHIs) for funding antiretroviral therapy (ART) scale-up. There are indications that global investments for ART scale-up are flattening. It is unclear what new funding channels can bridge the funding gap for ART service delivery. Many previous studies have focused on domestic government spending and international funding especially from GHIs. The objective of this study was to identify the funding strategies adopted by health facilities in Uganda to sustain ART programs between 2004 and 2014 and to explore variati
