Humeral surface replacement for the sequelae of fractures of the proximal humerus.

Fractures of the proximal humerus can lead to malalignment of the humeral head, necrosis and post-traumatic osteoarthritis. In such cases surface replacement might be a promising option. A total of 28 shoulders with glenohumeral arthritis subsequent to a fracture underwent surface replacement arthroplasty of the humeral head in patients with a mean age of 60 years (35 to 83). On the basis of the inclination of the impacted head, post-traumatic arthritis was divided into three types: type 1, an impacted fracture of the head in an anatomical position (seven cases); type 2, a valgus impacted frac

Intrathoracic fracture-dislocation of the humeral head: a case report.

Fracture-dislocation of the humeral head into the thoracic cavity is a rare injury. We present one such case in a 70-year-old woman. She presented with a 4-part fracture-dislocation of the proximal humerus, with displacement of the humeral head into the thoracic cavity. She had no signs of acute distress or hemodynamic instability. She underwent hemiarthroplasty of the right shoulder, but the humeral head fragment could not be removed. At 27-month follow-up, the patient had limited mobility of her right shoulder due to axillary nerve palsy but no pain or intrathoracic complications. In the abs

Anatomical considerations for percutaneous proximal humeral fracture fixation.

PURPOSE: To investigate the hypothesis that percutaneous wire fixation of proximal humeral fractures is prone to regional neurovascular injury. MATERIALS AND METHODS: 40 cadaveric shoulders had percutaneous wires inserted from anterior and lateral humeral shaft entry points into the humeral head. The shoulders were then dissected to identify the neurovascular structures in this area. RESULTS: The axillary nerve trunk and posterior circumflex humeral artery were found to be within the field of dissection. The axillary nerve was damaged on three occasions by the lateral wires, two of which wer…

Recent advances in the treatment of gunshot fractures of the humeral shaft.

The increase in violent crime has lead to an increase in gunshot-induced fractures in the United States. Injuries to the upper extremity are common. The treatment of gunshot injuries depends on the extent of soft tissue disruption and the type and location of fracture. Most of these injuries are a result of low-energy gunshot wounds that may be treated conservatively under the appropriate conditions. Low-energy fractures that require operative stabilization may be done with predictable results for achieving union with intramedullary fixation or compression plating. Complex open fractures from

Proximal median neuropathy secondary to humeral neck fracture.

Median neuropathies proximal to the wrist are uncommon and usually result from penetrating injuries, fracture dislocation of the distal humerus, or compression by fibrous bands. A 66-year-old man suffered a comminuted fracture of the proximal humerus after a fall. Electrodiagnostic studies revealed a severe proximal median neuropathy and a mild distal radial mononeuropathy. Proximal median neuropathy rarely occurs in humeral neck fracture, mostly because the median nerve is not in close contact with the humerus proximally.

Complications of intramedullary nailing for fractures of the humeral shaft: a review.

OBJECTIVES: The purpose of the present review is to document the complications associated with the use of locking humeral nails and to discuss the prevention and management of these complications. DATA SOURCES: Clinical and research experience supported by a Medline review of the English-language literature from 1985 to 1998. STUDY SELECTION: The majority of (clinical) information available was contained in retrospective reviews; only three prospective or randomized studies were identified (all in abstract form). Anatomical research regarding locking humeral nailing was also identified and i

Axillary artery injury in closed fracture of the humeral neck.

A case of atherosclerotic axillary artery rupture in closed fracture of the proximal part of the humerus is described. It demonstrates a possible serious arterial injury in elderly persons even in case the fracture of the humeral neck is not dislocated. If there are signs of disturbed circulation in these injuries, angiography is indicated irrespective of the type of fracture and mechanism of injury.

Treatment of shoulder dislocation with ipsilateral humeral shaft fracture.

A case presentation of shoulder dislocation with ipsilateral humeral shaft fracture is presented along with a review of the literature regarding nine reported cases. In the current case, closed treatment of the humeral shaft fracture was undertaken with an unsuccessful attempt at closed reduction of the anterior shoulder dislocation, resulting in a radial nerve deficit. Successful treatment required open reduction of the humeral fracture with compression plating followed by closed reduction of the shoulder dislocation. We compare this treatment (with outcome) to other methods of treatment.

[Fractures of the head of the humeral condyle in children and characteristics of their treatment].

The author has examined 165 children with FHCH. A method of surgical treatment of FHCH has been worked out and applied in 37 children; its essence consists in the open reposition of the fractures and the fixation of the fragment of the head with an original construction compression device which provides the possibility of making early active movements preventing rigidity in the elbow joint. The method does not envisage the necessity of fixation of the extremity with a plaster bandage and reduces the period of clinical and general treatment (by 18 and 14 days respectively). The economic damage

[Significance of rotation errors in supracondylar humeral fractures in the child].

To get information about the importance of malrotation for posttraumatic varus or valgus deformity a clinical, radiological and CT follow-up examination of 20 children who were treated for supracondylar fracture from 1983 to 1985 was done. Rotational-displacement was measured on computed tomographical images taken from the region where fracture had happened. On the ap follow-up X-ray from both, the fractured and the other side, and the X-ray which was made after reduction, the medial and lateral colonne were measured from a "special point" in the middle of the shaft of the humerus. The length

Fracture of the distal humeral chondroepiphysis in the neonate. A case report.

A neonatal fracture-separation of the distal humeral chondroepiphysis occurred associated with a difficult footling breach delivery. The diagnosis can be predicted when a physical examination of an apparently "dislocated" elbow reveals a normal triangular relationship between the olecranon process and the medial and lateral epicondyles. The injured epiphysis tethered on a wide-based periostium. Acceptable reduction was obtained by alignment of the fracture fragments and avoidance of an abnormal varus or valgus alignment. Healing was rapid, and remodeling as well as range of motion were excelle

Fracture-separation of the distal humeral epiphysis in young children.

Fracture-separation of the distal humeral epiphysis in young children is frequently misdiagnosed as a fracture of the lateral humeral condyle or a dislocation of the elbow. The roentgenographic relationships between the ossification center of the capitulum humeri and the radius are important in distinguishing between the three conditions. Arthrographic studies in our patients showed that this is an extracapsular supracondylar fracture. In six fracture-separations treated either by open reduction and wire fixation or in overhead traction, the results were excellent.

Nerve Injury Severity and Outcomes: An Analysis of Supracondylar Humeral Fractures.

BACKGROUND: Nerve injuries (NI) after pediatric supracondylar humeral fractures (SHF) are commonly reported as neurapraxic, with most studies focusing on motor recovery alone. This study aimed to analyze both sensory and motor outcomes of NI in SHF to identify patterns of recovery and predictors of prolonged nerve recovery. METHODS: A 4-year retrospective cohort study of children 0 to 12 years with NI after SHF was performed. Patterns of nerve recovery and outcomes of spontaneous recovery were analyzed descriptively. Predictors of prolonged nerve recovery (>4 months) were explored using multi…

Vitamin D status in healing of distal humeral fractures: Clinical observations.

BACKGROUND: Fracture healing is a complex biological process that begins soon after a fracture has occurred. Whereas the role of vitamin D status on the maintenance of bone health is well established, the clinical effects of vitamin D deficiency in the healing of distal humeral fractures are less well explored. This study examines the role of vitamin D serum levels in distal humeral fractures (C2, C3) managed with open reduction and double plate fixation, by comparing its concentration in patients with or without radiographic signs of fracture healing and in control subjects. Our hypothesis is…

Long-term outcomes of shoulder hemiarthroplasty for acute proximal humeral fractures.

PURPOSE: Shoulder hemiarthroplasty (HA) is now rarely indicated for complex proximal humeral fractures due to its unpredictable characteristic of the greater tuberosity (GT) healing. Despite the increasing popularity of reverse shoulder arthroplasty (RSA) in fracture treatment, there are still concerns about failure revision and its application in young populations. The complete negation of HA for fracture treatment is still under debate. METHODS: Eighty-seven out of 135 patients with acute proximal humeral fractures treated with HA were enrolled. Clinical and radiographic evaluations were pe…

Percutaneous helical plate fixation in humeral shaft fractures with proximal extension.

BACKGROUND: Humeral shaft fractures with extension to the proximal third are increasingly frequent and technically more demanding. Surgical management of proximal metaphyseal junction humeral fractures is challenging. The aim of this study was to assess the safety, reproducibility, and possibility of early and completed rehabilitation in the percutaneous treatment with helical plates in humeral shaft fractures with proximal extension. PATIENTS AND METHODS: This was a descriptive, retrospective, single-centre cohort study based on consecutive patients with proximal metaphyseal junction humeral…

Infection and Nonunion Following Operative Treatment of Lateral Humeral Condyle Fractures.

BACKGROUND: Lateral humeral condyle fractures (LHCF) are the second most frequent type of pediatric elbow fracture. The purpose of this study is to characterize infections and nonunions of LHCF treated at a single institution. This is the largest series on infections and nonunions of LHCF to date. METHODS: Pediatric patients undergoing surgical treatment for a LHCF from 2012 to 2022 at a single children's hospital were identified for a retrospective review. Patients who were diagnosed with an active infection or nonunion after surgery were included. Data collected included demographics, origi

Radial Nerve Palsy in Paediatric Humeral Shaft Fractures: Incidence and Management.

BACKGROUND: Humeral shaft fractures are relatively rare in the paediatric population. The purpose of our study was to retrospectively evaluate all humeral shaft fractures treated at a children's trauma centre and assess cases involving radial nerve injury. MATERIAL AND METHODS: We retrospectively evaluated a group of 5 skeletally immature patients with radial nerve palsy out of a total of 104 patients with humeral shaft fractures treated in our hospital between January 2011 and December 2021. RESULTS: The study group consisted of four boys and one girl aged 8.6 to 17.2 years (average ag
