Holoprosencephaly is the most common brain malformation in humans and it is a complex genetic disorder. We report on a patient with holoprosencephaly caused by a rare ZIC2 mutation presenting a bifid nose associated with a nasal fistula and an epidermal cyst, besides hypernatremia. The patient was a 1 year and 4 months old girl that developed an important neuropsychomotor delay. Currently, she uses a wheelchair to move around and only emits sounds. Computed tomography (CT) scan revealed a semilobar holoprosencephaly and a Dandy-Walker variant. Head magnetic resonance imaging also disclosed cor…

OBJECTIVE: To evaluate the relationship between enteral water infusion for hypernatremia and significant intestinal morbidity in infants <or=1000 grams.
STUDY DESIGN: This is a retrospective study of 321 infants <or=1000 g birth weight. Infants were grouped by the highest serum sodium (mmol/l) during the first 14 days of life as follows: <150 (normal control), >or=150 (high sodium control), >or=150 and treated with sterile water (study group). Significant intestinal morbidity was defined as probable or proven necrotizing enterocolitis or spontaneous intestinal perforation. Statistical analysi…

An infant with E.E.C. Syndrome (a genetic condition characterized by: Ectrodactyly, Ectodermal dysplasia, Cleft Lip-Palate) and delayed psychomotor development was found to have chronic hypernatremia and absence of thirst. Computerized brain tomography revealed abnormal lateral ventricles, agenesis of corpus callosum, calcified cortex. His plasma Anti Diuretic Hormone (ADH) levels were inappropriately low relative to his high values of plasma osmolality, whereas ADH responsiveness to nonosmotic stimuli was normal. E.E.C. Syndrome in this baby is characterized by: ectrodactyly of the feet, synd…

Hypernatremia was found as a cause of recurrent fever episodes in a 18 month old boy with Mb, Pierre-Robin and agenesia of the corpus callosum. We found values of serum-sodium until 160 mVal/l. Examinations of the osmotic regulation showed a so called "essential hypernatremia" without polyuria and polydipsi. Therefore must be responsible a defect of the osmotic center and thirst center in the hypothalamic area. As therapy a diet of 1000 ml = 1000 kcal = 100 mg Sodium/d. was successful.

CASE PRESENTATIONS: We describe two cases of children who were significantly impacted by incorrectly prepared, hyperosmolar formula delivered via enteral tubes. The first case is of a 5-month-old post-pancreatectomy patient with complex needs who received hyperosmolar formula while in a pediatric intensive care unit (PICU), whereas the second is of a 1-year-old with spinal muscular atrophy who received hyperosmolar formula at home - resulting in a PICU admission.
DISCUSSION: Hypovolemic hypernatremia is an important but poorly understood complication that can occur if overly concentrated form…

In exclusively breastfed newborns, hypernatremic dehydration is associated with a free water deficit secondary to insufficient fluid intake. Failure of newborns to regain their birth weight by the 10th day of life should be investigated urgently. In this report, we present a case of a 2 -week-old girl who presented to our institution for 30% weight loss and was found to have severe hypernatremic dehydration associated with acute renal failure (creatinine 4 mg/dL). Upon further investigation, the breast milk sodium content was found to be extremely elevated (90 mEq/L). To our knowledge, the fol

Salt poisoning is a rare cause of severe hypernatremia in children resulting from the ingestion of toxic amounts of sodium chloride, either from accidental or intentional administration of salted solutions. We present the case of a newborn admitted to a pediatric emergency department for lethargy and reduced oral intake; his laboratory evaluation showed severe hypernatremia ([Na+] of 174 mmol/L). The infant developed convulsive status epilepticus during treatment. Neuroimaging showed a tetraventricular hemorrhage, a large right-sided parenchymal hemorrhage with midline shift, and several left …

Profound acidemia impairs cellular and organ function and consequently should be associated with an increased risk of mortality in critically ill humans and animals. Neonatal diarrhea in calves can result in potentially serious metabolic derangements including profound acidemia due to strong ion (metabolic) acidosis, hyper-D-lactatemia, hyper-L-lactatemia, azotemia, hypoglycemia, hyperkalemia and hyponatremia. The aim of this retrospective study was to assess the prognostic relevance of clinical and laboratory findings in 1,400 critically ill neonatal calves with diarrhea admitted to a veterin

Plasma and urinary arginine vasopressin (AVP) in normal subjects and in patients with various water metabolism disorders was measured using … athogenesis of hyponatremia. Variable levels of plasma AVP were observed in patients with essential hypernatremia, which in turn suggested that osmoreceptors may be selectively damaged in some patients, and that ADH-secreting neurons are also involved in others. Our radioimmunoassay facility made it possible for us to measure plasma and urinary DDAVP in the tre …

Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes characterized by hyperglycemia, high-anion-gap metabolic acidosis, and ketonemia. Hypernatremia in DKA is uncommon and typically reflects disproportionate free water loss. Arginine vasopressin resistance (AVP-R), previously known as nephrogenic diabetes insipidus, may occur in the setting of hypokalemia, leading to polyuria, hypotonic urine, and worsening hypernatremia. We report the case of a 14-year-old previously healthy female who presented with new-onset severe DKA complicated by profound acidosis, hyperglycemia, h…

PURPOSE: Discharge is a critical time point in the care pathway of geriatric hospital patients, and post-acute care facilities often have less monitoring possibilities. Active medical issues such as electrolyte disturbances should be treated before transfer. We studied the impact of in-hospital hypernatremia of older hip fracture patients to mortality at 90 days.
METHODS: A retrospective study population of 2240 hip fracture patients from 2015 to 2019 was collected from the Hospital District of Southwest Finland data pool. In the present study we included patients aged ≥65 years who were tran…

We report a case of thromboembolism in a patient with hypernatremia resulting from lithium-induced nephrogenic diabetes insipidus (NDI). A 49-year-old female patient on chronic lithium therapy due to bipolar disorder was transferred to the emergency department with signs of dehydration, altered mental status, and increased oxygen demand. She was admitted to a local psychiatric clinic first because of an exacerbation of a manic episode. When she was transferred to our clinic, her blood pressure was 130/80 mmHg, she was tachycardic (110 beats/min), had tachypnea (24 breaths/min), normal body tem…

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is characterized by impaired renal responsiveness to arginine vasopressin, causing defective urinary concentration and large volumes of dilute urine. Long-term lithium exposure is an important acquired cause. Perioperative fasting, restricted free-water intake, and ongoing fluid losses may unmask compensated lithium-associated urinary concentrating dysfunction and precipitate severe hypernatremia.

A major goal in treatment of calves with diarrhea is to restore hydration and to correct metabolic acidosis. This can be achieved by the adm … ing OES than in calves receiving CON (4.2 d vs. 2.1 d, respectively). The OES treatment resulted in hypernatremia (serum Na+ >145 mmol/L) within 48 h after the first OES administration. Hypernatremia was linked with higher serum Cl- and urea concentrations and thus higher serum osmolarity in OES calves compared with CON calves. Administered under these conditio …
