Correction of a double spondylolisthesis of the lumbar spine utilizing Chiropractic BioPhysics® technique: a case report with 1 year follow-up.

[Purpose] To report on the reduction of a double lumbar spine spondylolisthesis by use of Chiropractic BioPhysics® technique. [Participant and Methods] A 57 year-old male presented with severe chronic low back pains and sciatica. After playing hockey for 50 years, he was unable to continue and was forced to retire. Lumbar radiography showed an L3 retrolisthesis of -5.3 mm and an L4 anterolisthesis of 5.4 mm. Chiropractic BioPhysics technique including mirror image lumbar spine drop-table adjustments, corrective exercises and a unique pelvic extension traction was performed 50 times over 7-mont

C1 burst fracture.

OBJECTIVE: The presentation of a patient to a chiropractic office with an undiagnosed C1 burst fracture (Jefferson fracture) secondary to head trauma from a motor vehicle accident is introduced. CLINICAL FEATURES: A 67-yr-old real estate manager suffered from neck pain and stiffness following release from a major metropolitan medical center. Anteroposterior open mouth radiographic examination of the cervical spine revealed 10-mm displacement of the lateral masses of C1 relative to the superior articulating facets of C2, indicating the presence of a C1 burst fracture with possible disruption o

Abrupt change in head position and cerebral infarction.

Eight patients are described who developed infarctions in the vertebral-basilar artery distribution following chiropractic neck manipulation or spontaneous head turning. The angiographic and autopsy findings indicate that injury to the intima of the vertebral artery at the atlantoaxial joint forms a nidus for thrombus formation which may propogate or embolize to involve other vessels in the vertebral-basilar system and result in progressive brainstem infarction. The role of anticoagulation in these patients is discussed.

Long-Term Medicare Costs Associated With Opioid Analgesic Therapy vs Spinal Manipulative Therapy for Chronic Low Back Pain in a Cohort of Older Adults.

OBJECTIVES: The purpose of this study was to compare Medicare healthcare expenditures for patients who received long-term treatment of chronic low back pain (cLBP) with either opioid analgesic therapy (OAT) or spinal manipulative therapy (SMT). METHODS: We conducted a retrospective observational study using a cohort design for analysis of Medicare claims data. The study population included Medicare beneficiaries enrolled under Medicare Parts A, B, and D from 2012 through 2016. We assembled cohorts of patients who received long-term management of cLBP with OAT or SMT (such as delivered by chir

A population-based case-series of Ontario patients who develop a vertebrobasilar artery stroke after seeing a chiropractor.

PURPOSE: The current evidence suggests that association between chiropractic care and vertebrobasilar artery (VBA) stroke is not causal. Rather, recent epidemiological studies suggest that it is coincidental and reflects the natural history of the disorder. Because neck pain and headaches are symptoms that commonly precede the onset of a VBA stroke, these patients might seek chiropractic care while their stroke is in evolution. However, very little is known about the characteristics of these patients. In fact, only small clinical case series and physician surveys have described the characteris

Neck pain.

Neck pain is less common than low back pain but still a relatively common reason for seeing a primary care physician. Therefore, it is necessary for the primary care physician to be comfortable with salient points in the history and to be able to perform a basic neurologic examination. Important aspects of the history and physical examination are reviewed. Important clinical syndromes and treatment options are also reviewed.

Lumbar facet syndrome: spinographic assessment of treatment by spinal manipulative therapy.

Spinographic parameters of a group of patients with extension facet subluxations and a group with herniated disc nucleus pulposus of the lumbar spine were evaluated pre- and post-manipulative treatment. A statistically significant decrease in the mean disc angle at the involved level in the extension facet subluxation group was seen with the mean post-treatment disc angle more closely resembling that of the controls. Likewise, a statistically significant increase occurred in the mean disc angle at the involved level in the disc herniation group post-treatment to one more closely resembling tha

Efficacy of manipulation for non-specific neck pain of recent onset: design of a randomised controlled trial.

BACKGROUND: Manipulation is a common treatment for non-specific neck pain. Neck manipulation, unlike gentler forms of manual therapy such as mobilisation, is associated with a small risk of serious neurovascular injury and can result in stroke or death. It is thought however, that neck manipulation provides better results than mobilisation where clinically indicated. There is long standing and vigorous debate both within and between the professions that use neck manipulation as well as the wider scientific community as to whether neck manipulation potentially does more harm than good. The prim

The effectiveness of physiotherapy and manipulation in patients with tension-type headache: a systematic review.

The study design is a systematic review of randomised clinical trials (RCTs). The objectives of the present study are to assess the effectiveness of physiotherapy and (spinal) manipulation in patients with tension-type headache (TTH). No systematic review exists concerning the effectiveness of physiotherapy and (spinal) manipulation primarily focussing on TTH. Literature was searched using a computerised search of MEDLINE, EMBASE and the Cochrane library. Only RCTs including physiotherapy and/or (spinal) manipulation used in the treatment of TTH in adults were selected. Two reviewers independe

Chiropractic complications. Another case report.

We report the case of a 34-year-old man, treated by chiropractic manipulation for tension-type headache. The patient complained of a sharp occipital pain during the first session, followed by vomiting and loss of consciousness, and remained comatose for five days. Neurological examination detected persistence of dysarthria, ataxia, with delayed responses. Neuroradiological findings reveal an ischemic lesion in left PICA region, confirmed by angiography. Clinical and radiological findings suggested complete remission about two months later.

Paraplegia in osteogenesis imperfecta. A case report.

Paraplegia occurred in an adolescent girl with osteogenesis imperfecta after chiropractic manipulation. The child had been able to walk freely out of doors. Complete motor paralysis with sensory sparing resulted due to anterior compression of the cord by spondyloptotic cervical vertebrae. Reconstructed computerised tomography was very helpful in demonstrating the abnormality. Anterior and then posterior decompression relieved the tethered spinal cord and were supplemented with bone grafting. Early diagnosis and surgical treatment will prevent similar neurological accidents.

Collaborative care for a patient with complex low back pain and long-term tobacco use: a case report.

Few examples of interprofessional collaboration by chiropractors and other healthcare professionals are available. This case report describes an older adult with complex low back pain and longstanding tobacco use who received collaborative healthcare while enrolled in a clinical trial. This 65 year-old female retired office worker presented with chronic back pain. Imaging findings included disc extrusion and spinal stenosis. Multiple co-morbidities and the complex nature of this case substantiated the need for multidisciplinary collaboration. A doctor of chiropractic and a doctor of osteopathy

Non-surgical Restoration of L3/L4 Disc Herniation.

We report the case of a 52-year-old Asian man presenting with severe lower back pain and bilateral radiculopathy due to acute L3/L4 disc extrusion, causing severe spinal canal stenosis and bilateral L4 nerve root compression. Following a ten-week conservative management plan consisting of high-velocity, low-amplitude spinal manipulation, adjunctive therapies, and home exercise, the patient experienced significant pain relief, functional improvement, and near resolution of deficits. A six-month follow-up MRI revealed the resolution of the L3/L4 disc extrusion, and the patient remained asymptoma
