BACKGROUND: 5-Hydroxymethylcytosine (5-hmC) is an epigenetic marker detectable through immunohistochemistry (IHC) that has been shown to distinguish benign nevi from melanoma with high sensitivity and specificity. The purpose of the study was to explore its diagnostic utility in a subset of histologically challenging, heavily pigmented cutaneous melanocytic neoplasms.
METHODS: 5-hmC IHC was performed on 54 heavily pigmented melanocytic tumors. Semi-quantitative analysis of immunoreactivity was correlated with clinical, pathologic and follow-up data.

BACKGROUND: The association of Blaschko lines with genetic mosaicism has lead to the concept that this pattern represents the manifestation of genetically abnormal skin tissue contrasting with the genetically normal skin. Various mosaic defects affecting not only the skin but also extracutaneous tissues have lead to the description of different types of epidermal nevus syndromes. PURPOSE: To describe two patients with blue nevi of the palpebral conjunctiva and to review the existing literature on common and cellular blue nevi of the conjunctiva.
METHODS: Report of two cases and literature review.
RESULTS: We present two cases of blue nevi in the palpebral conjunctiva: an atypical cellular blue nevus of the left upper eyelid and a common blue nevus around the lacrimal punctum of the left lower eyelid. Both patients underwent full thickness eyelid excision with wide margins. There was no tumor recurrence at 11 and 4 months postoperatively.

A case of gallbladder malignant melanoma in a 62-year-old woman is reported. The gallbladder, resected for tumor mass seen on ultrasound examination, revealed neoplastic epithelioid cells with dark granules in the cytoplasm. These cells with pigment were positive immunohistochemically for S-100, HMB45 and vimentin. The patient, affected by dysplastic nevus syndrome, had a history of a melanoma in situ in her left upper arm that was excised 11 years ago. This is only the second case reported to date of primary malignant melanoma in dysplastic nevus syndrome.

Dermal melanocytes are generally most numerous in the sacral, dorsal hand, and dorsal foot. There is also a slight rise that often occurs toward the axial line of the trunk. The practitioner needs to be aware and take necessary measures to properly diagnose the blue nevus from other forms of pigmented skin lesion. The authors believe that surgical excision with proper pathologic follow-up is the preferred treatment.

A 31-year-old patient presented with a new, rapidly growing pigmented skin tumor on her left thigh. Several years previously a level 2 superficial spreading melanoma (SSM) with a tumor thickness of 0.45 mm had been removed from her left thigh. Dermatoscopic examination revealed asymmetric pigmentation of the tumor with branched streaks of variable thickness and irregular pseudopodia. Histological analysis showed it to be a severely dysplastic melanocytic nevus.

Tinea nigra is an asymptomatic superficial fungal infection caused by Phaeoannelomyces werneckii, generally affecting the skin of the palms and characterized by deeply pigmented macular non-scaly patches. These lesions are quite characteristic. However, they can be misdiagnosed as a malignant melanoma or a junctional melanocytic nevus and unnecessary biopsies may be performed. Thus, dermoscopy is a fast, useful, clinical adjunctive tool in differentiating tinea nigra from melanocytic lesion.
