BACKGROUND: Every third person with intellectual disability suffers from additional mental health problems, among others phobic disorders. Yet we do not know whether psychotherapeutic methods that are effective in the normal population are applicable to people with intellectual disabilities.
PATIENTS AND METHODS: We give a survey of the development and the present state of the art of psychotherapy, particularly with regard to phobic disorders in intellectual disability. Therapeutic recommendations described in the literature will be evaluated in a case study of one patient.

Gender identity disorder (GID) is characterized by strong and persistent cross-gender identification and by persistent discomfort with one's sex. This paper reviews reports of GID in which adolescent psychotherapy patients initially desired sex reassignment. In addition, four case studies of adolescents with GID, demonstrating different outcomes of psychotherapy, are presented. It is concluded that great caution must be exercised when treating adolescents with GID, and that sex reassignment must not be started before patients have reached eighteen years of age.

In the process of psychoanalysis or psychoanalytical therapy countertransference plays a significant role: On the one hand still unconscious countertransference reactions of the therapist hinder the progress of the patient, especially when they lead to an acting-out of the countertransference resistance. On the other hand every analyzed countertransference reaction helps directly in understanding the patient and furthers of the process of therapy itself. This also applies to analytical child-therapy, which is demonstrated by means of some case histories.
