Prevalence, incidence and risk factors for rugby-related injuries: A survey of the Safari Sevens tournament.

BACKGROUND: Rugby-related injuries are a leading cause of dropout from competitive sports, high insurance compensation, disability and socioeconomic marginalisation. The debilitating effect of these injuries on players may deny them the benefits associated with rugby and can lead to premature termination of a rugby career. OBJECTIVES: To determine the prevalence, incidence and risk factors for rugby-related injuries among male Safari Sevens rugby tournament players in Kenya. METHOD: Following ethical approval and clearance, this cross-sectional study was carried out among October 2021 teams …

Nutritional knowledge, attitudes and behaviours in rugby league; influences of age, body composition and ancestry.

INTRODUCTION: Rugby league is a physically demanding sport that necessitates considerable nutritional intake, focusing on quality and type, in order to optimize training and competition demands. However, rugby league athletes are reported to have inadequate nutrition intake to match these demands. Some factors that may determine an athlete's nutrition intake have been reported in other sports, including (but not limited to, knowledge, time, cooking skills, food costs, income, belief in the importance of nutrition, body composition goals, and family/cultural support). However, these potential f…

Player Activity and Load Profiling with Hidden Markov Models: A Novel Application in Rugby League.

Player movement in rugby league is complex, being spatiotemporal and multifaceted. Modeling this complexity to provide robust measures of player activity and load has proved difficult, with important aspects of player movement yet to be considered. These include the influence of time-varying covariates on player activity and the combination of different dimensions of player movement. Few studies have simultaneously categorized player activity into different activity states and investigated factors influencing the transition between states, or compared player activity and load profiles between …

Poor cervical proprioception as a risk factor for concussion in professional male rugby union players.

BACKGROUND: Concussion is one of the most common injuries in male professional Rugby Union ('rugby') and accounts for significant time loss from training and competition. Despite the most recent Concussion in Sport Group consensus statement recommending a focus on the identification of modifiable risk factors, limited evidence for their existence is available. OBJECTIVE: To investigate the association between cervical proprioception and concussion incidence in a group of professional male rugby players over the course of a full season. METHODS: 165 players were assessed at pre-, mid- and end…

Head Injury Assessment in the Elite Level Rugby Union in Japan: Review of 3 Seasons.

Head Injury Assessment (HIA) is the screening tool for head injury during a rugby game. The purpose of this study was to investigate the epidemiology of HIA in the Japan Rugby Top League (JRTL). The incidences of HIA, defined concussion (per 1,000 player-hours) and repeated concussions were evaluated in three seasons (2016-17, 2017-18, 2018-19; total 360 games). The HIA incidence rates were 12.7 (95% confidence interval 9.5-15.9), 20.8 (16.8-24.9), and 25.0 (20.5-29.5) in each season. HIA-1 criteria 2, which is applied for suspected concussion cases, was performed for 46 cases in the 2016-17 s…

Pressure. A Qualitative Analysis of the Perception of Concussion and Injury Risk in Retired Professional Rugby Players.

This study interviewed retired professional rugby union players (≤10 years since retirement) to discuss their careers in the game of rugby union. The primary aim of the study was to document their understanding of concussion knowledge and the analogies they use to describe concussion. In addition, these interviews were used to determine any explicit and implicit pressures of playing professional rugby as described by ex-professional rugby players. Overall, 23 retired professional rugby players were interviewed. The participants had played the game of rugby union (n = 23) at elite professional …

The performance of the World Rugby Head Injury Assessment Screening Tool: a diagnostic accuracy study.

BACKGROUND: Off-field screening tools, such as the Sports Concussion Assessment Tool (SCAT), have been recommended to identify possible concussion following a head impact where the consequences are unclear. However, real-life performance, and diagnostic accuracy of constituent sub-tests, have not been well characterized. METHODS: A retrospective cohort study was performed in elite Rugby Union competitions between September 2015 and June 2018. The study population comprised consecutive players identified with a head impact event undergoing off-field assessments with the World Rugby Head Injury…

Diagnosed concussion is associated with increased risk for lower extremity injury in community rugby players.

OBJECTIVES: To determine (1) the association between lifetime diagnosed concussion and lower extremity musculoskeletal injury (LE-MSI) among community rugby union players and (2) the sex specific risk of LE-MSI given concussion history among males and females. DESIGN: Retrospective survey. METHODS: 1037 (59.0% male, (612/1037), age: 31.6 ± 11.3 years) rugby players (10.1 ± 8.1 years played) completed an online survey to ascertain injury history. A chi-squared test of association was performed between concussion and LE-MSI; significant outcomes were followed-up with an odds ratio. A binary lo…

The kinematics of 1-on-1 rugby tackling: a study using 3-dimensional motion analysis.

BACKGROUND: Although past studies using video analysis indicated that the arm tackle and head-in-front shoulder tackle are possible risks for shoulder dislocation, the underlying mechanisms of tackling-related shoulder dislocation have not been sufficiently investigated. This study aimed to analyze the kinematic aspects of these tackling motions in 1-on-1 tackles in an experimental setting using a 3-dimensional motion-capture system. METHODS: A total of 65 one-on-one tackles were recorded using a marker-based, automatic, digitizing motion-capture system. A documented tackle was classified int

Determining return-to-sport status with a multi-component assessment strategy: a case study in rugby.

BACKGROUND: The effectiveness of rehabilitation programmes are often distorted by the athlete's desire to return and can result in injury recurrence. Athletic assessments allow for objective and reliable measurements to track rehabilitation progress. This case study used a multi-component assessment strategy to assess a rugby player's lower-extremity strength and symmetry as a primary determinate of their return-to-sport status. CASE DESCRIPTION: A professional rugby league player was assessed for lower-extremity isokinetic strength and sprint kinetics pre- and 10-weeks post-rehabilitation pr…

Superior dislocation of patella in a rugby player: an update on a extremely rare condition and review of literature.

Superior dislocation of patella without patellar ligament injury is a rare clinical condition: hence often brings diagnostic dilemma for Emergency physician. Only few cases have been reported in the literature and all of them had coexistant patellofemoral joint osteoarthritis. We present such dislocation in a 34-years old rugby player after a fall without any preexisting arthrosis in the knee. This case report aims to highlight a rare condition and subtle radiological findings while assessing acute knee pain. Differences of this dislocation with vertical type of intraarticular dislocation have…

Posterior sternoclavicular dislocation in a rugby player as a cause of silent vascular compromise: a case report.

BACKGROUND: Approximately 120 cases of posterior sternoclavicular joint (SCJ) dislocation have been documented in the medical literature since it was first described in 1824 by Sir Astley Cooper, a statistic which underlies its relative rarity. It is associated with high energy trauma, and although it may present innocently enough, it is a potentially life threatening injury. CASE AND RESULTS: We describe a case in which there was no clinical evidence of complication, although CT imaging revealed complete obstruction of the brachiocephalic vein and impingement of the aorta. This required open

[Study of variations in hematologic parameters in rugby players undergoing physical training at a high altitude].

Thirty-three well-trained rugby players, ranging in age from 18 to 30 years, were studied during basal training for one year. Peripheral blood parameters and iron metabolism indices were investigated before, during and at the end of the season. The hematologic status showed no substantial changes with respect to physical activity even if considered by age and team-role. However, a significant reduction (P less than 0.001) in RBC count, hemoglobin, hematocrit, serum iron, plasma transferrin and ferritin, was observed when compared with those obtained from a group of healthy untrained controls. …

Ultrasound-Based Morphofunctional Assessment of the Transversus Abdominis and Urinary Incontinence in Female Rugby Players.

Background: Female rugby players are exposed to repeated contact, impact, acceleration, deceleration, and trunk-stabilization demands that may challenge lumbopelvic control and intra-abdominal pressure regulation. Rehabilitative ultrasound imaging can characterize transversus abdominis (TrA) thickness changes during standardized tasks, although these measures should not be interpreted as direct indicators of neuromuscular activation or sport-specific trunk function. This study aimed to characterize ultrasound-derived TrA thickness and thickness change in nulliparous female rugby players and to…

Nonsteroidal Anti-inflammatory Drugs (NSAIDs) and Collision Sports: The Need for Further Education in Rugby.

Rugby is a collision sport and the fastest growing sport in the United States. Nonsteroidal anti-inflammatory drugs (NSAIDs), which can be acquired without prescription, are used by millions of individuals daily. Athletes in collision sports such as rugby overuse them, and the management of these analgesic anti-inflammatory drugs is gaining importance to healthcare providers in sports because of their potential organ toxicities. Athletes, amateur and elite, may misconstrue the perceived advantages of NSAIDs and seem to be unaware of the serious complications arising from their overuse. This ov…
