Effects of the Topical Administration of Semaglutide on Retinal Neuroinflammation and Vascular Leakage in Experimental Diabetes.

BACKGROUND: An unexpected increase in the rate of severe diabetic retinopathy was observed in the Semaglutide in Subjects with Type 2 Diabetes (SUSTAIN)-6 clinical trial. Although this effect was attributed to a rapid decrease in blood glucose levels, a direct deleterious effect of semaglutide on the retina could not be ruled out. In order to shed light on this issue, we have performed a study aimed at testing the direct effect of semaglutide administered by eye drops on retinal neuroinflammation and microvascular abnormalities using the db/db mouse model. METHODS: Eye drops containing semagl

Integrating oral semaglutide into clinical practice in primary care: for whom, when, and how?

Oral semaglutide is the first US Food and Drug Administration-approved oral glucagon-like peptide-1 receptor agonist (GLP-1RA) for the treatment of type 2 diabetes (T2D). Prior articles within this supplement reviewed the PIONEER trial program, which demonstrated that oral semaglutide reduced glycated hemoglobin and body weight when given to patients with uncontrolled T2D on various background therapies, and had a safety profile consistent with subcutaneous GLP-1RAs. This article provides guidance on integrating oral semaglutide into clinical practice in primary care. Patient populations with

Access Barriers and Variable Response to Semaglutide in Severe Adolescent Obesity: A Case Series.

Pediatric obesity is a worsening public health crisis, affecting 1 out of 5 youth in the United States. Once-weekly semaglutide has produced clinically meaningful BMI reduction in adolescents in randomized trials. However, trial cohorts and protocols may not reflect real-world care where patients are more diverse and may face access barriers-a factor that disproportionately affects minoritized groups. In this case report, we describe three adolescents of color with severe obesity treated in a multispecialty obesity clinic with lifestyle counseling and semaglutide. We summarize their clinical c

Semaglutide for treatment of intractable diarrhea of different etiologies: a case series of 30 patients.

OBJECTIVES: Diarrhea is common and can exert a profound negative impact on social life due to unpredictable bowel movements and fecal incontinence. Liraglutide has been documented to be effective in patients with bile acid diarrhea (BAD), but documentation for the effect of semaglutide in patients with unspecific diarrhea is lacking. We report experiences with compassionate use of weekly semaglutide in patients with diarrhea of different etiologies. METHODS: Our cases comprised 15 patients with BAD, nine with idiopathic diarrhea, four with short bowel syndrome, two with collagenous colitis an

Metabolite Profiling and Identification of Semaglutide in Liver S9 Across Species and Rat Plasma.

Semaglutide, a glucagon-like peptide-1 receptor agonist, stands as a paradigm of successful peptide drug development. Although absorption, distribution, metabolism and excretion characteristics of semaglutide have been thoroughly studied in both animals and humans, a reliable in vitro screening model for evaluating metabolic behavior of semaglutide and other peptides remains an unmet need in drug development. This study established a novel ultrahigh performance liquid chromatography coupled to high-resolution mass spectrometry method to elucidate the species-specific metabolic pathways of sema

Comparative Effectiveness of SGLT2 Inhibitors and Semaglutide in Diabetic Nephropathy: A Retrospective Observational Study.

Sodium-glucose cotransporter-2 (SGLT2) inhibitors and glucagon-like peptide-1 (GLP-1) receptor agonists have demonstrated renal benefits beyond glycemic control in patients with type 2 diabetes mellitus (T2DM). This study compares renal outcomes and metabolic parameters in 10 patients with early diabetic nephropathy, five treated with SGLT2 inhibitors (empagliflozin or dapagliflozin) and five with semaglutide, a type of GLP-1 receptor agonist. Over a six-month follow-up, both groups showed improvement in albuminuria and estimated glomerular filtration rate (eGFR) stabilization. However, semagl

Cardiovascular outcomes of semaglutide and tirzepatide for patients with type 2 diabetes in clinical practice.

Cardiovascular outcome trials of the incretin-based medicines tirzepatide and semaglutide have shown benefits in populations with varying levels of cardiovascular risk. However, without direct head-to-head comparisons, treatment decisions rely on indirect evidence from heterogeneous trial populations, leaving optimal treatment choices uncertain. Here we conducted five cohort studies to assess the effectiveness of tirzepatide and semaglutide in patients with elevated cardiovascular risk, including obesity and type 2 diabetes, enrolled in insurance programs in the USA between 2018 and 2025. Semaglutide, tirzepatide, and retatrutide attenuate the interoceptive effects of alcohol in male and female rats.

RATIONALE: Alcohol use disorder (AUD) remains a major public health challenge, yet effective pharmacotherapies are limited. As such, there is growing interest in repurposing medications with novel mechanisms of action. Glucagon-like peptide-1 (GLP-1) receptor agonists, originally developed for type 2 diabetes, have emerged as promising candidates due to effects on intake regulation and reward processing. GLP-1 receptor agonists, including semaglutide, reduce alcohol intake and relapse-like behaviors in rodent and non-human primate models, and a recent clinical trial found that semaglutide decr

The impact of semaglutide use for weight loss on transforaminal lumbar interbody fusion outcomes.

INTRODUCTION: While glucagon-like peptides 1 receptor agonists (GLP-1RAs) grow in popularity, their potential for presurgical weight optimization in spine surgery remains unclear. We examined the influence of semaglutide prescription on one- to three-level transforaminal lumbar interbody fusion (TLIF) outcomes. METHODS: Retrospective analysis of obese, non-diabetic patients was conducted from 2018 to 2022. A 1:1 exact match paired semaglutide users with non-users based on age, gender, surgical levels, and comorbidities. The primary outcome were the rates of surgical and medical complications

Semaglutide effects on energy balance are mediated by Adcyap1+ neurons in the dorsal vagal complex.

The use of the GLP-1R agonist semaglutide is revolutionizing the treatment of obesity, yet its mechanistic effects on energy balance remain elusive. Here, we demonstrate that reactivation of semaglutide-responsive dorsal vagal complex neurons mimics the drug's effects of reducing food intake and body weight and promoting fat utilization and conditioned taste aversion. We observe that many of the semaglutide-activated area postrema (AP) and nucleus of the solitary tract (NTS) neurons express Adcyap1 mRNA, and ablation of AP/NTS Adcyap1+ neurons largely reverses semaglutide's effects on energy b
