Cross-modal distractors modulate oscillatory alpha power: the neural basis of impaired task performance.

Unexpected novel sounds capture one's attention, even when irrelevant to the task pursued (e.g., playing video game). This often comes at a cost to the task (e.g., slower responding). The neural basis for this behavioral distraction effect is not well understood and is subject of this study. Our approach was motivated by findings from cuing paradigms suggesting a link between modulations in oscillatory activity and voluntary attention shifts. The current study tested whether oscillatory activity is also modulated by a task-irrelevant auditory distractor, reflecting a neural signature of an inv

Medication adherence interventions for South Asian populations with cardiovascular disease: A systematic review and narrative synthesis.

South Asian populations experience a disproportionate burden of cardiovascular disease (CVD) globally, with evidence consistently highlighting suboptimal medication adherence across both regional and diaspora settings. While previous research has primarily identified socio-cultural barriers to adherence, there is a critical need to analyse the effectiveness and feasibility of interventional solutions to improve medication adherence in this population. This systematic review synthesised evidence on the reported effectiveness and feasibility of such interventions from 29 studies published betwee

Viral hepatitis elimination challenges in low- and middle-income countries-Uzbekistan Hepatitis Elimination Program (UHEP).

BACKGROUND & AIMS: Chronic infection with hepatitis B and C viruses (HBV & HCV) is a major contributor to liver disease and liver-related mortality in Uzbekistan. There is a need to demonstrate the feasibility of large-scale simplified testing and treatment to implement a national viral hepatitis elimination program. METHODS: Thirteen polyclinics were utilized to screen, conduct follow-up biochemical measures and treat chronic HBV and HCV infection in the general adult population. Task shifting and motivational interviewing training allowed nurses to provide rapid screening and general practi…

Reversal of peripheral nerve injury-induced neuropathic pain and cognitive dysfunction via genetic and tomivosertib targeting of MNK.

Neuropathic pain caused by nerve injury presents with severe spontaneous pain and a variety of comorbidities, including deficits in higher executive functions. None of these clinical problems are adequately treated with current analgesics. Targeting of the mitogen-activated protein kinase-interacting kinase (MNK1/2) and its phosphorylation target, the mRNA cap binding protein eIF4E, attenuates many types of nociceptive plasticity induced by inflammatory mediators and chemotherapeutic drugs but inhibiting this pathway does not alter nerve injury-induced mechanical allodynia. We used genetic man

Quantifying Postural Control during Exergaming Using Multivariate Whole-Body Movement Data: A Self-Organizing Maps Approach.

BACKGROUND: Exergames are becoming an increasingly popular tool for training balance ability, thereby preventing falls in older adults. Automatic, real time, assessment of the user's balance control offers opportunities in terms of providing targeted feedback and dynamically adjusting the gameplay to the individual user, yet algorithms for quantification of balance control remain to be developed. The aim of the present study was to identify movement patterns, and variability therein, of young and older adults playing a custom-made weight-shifting (ice-skating) exergame. METHODS: Twenty older

Functional connectivity of dorsal and ventral frontoparietal seed regions during auditory orienting.

Our ability to refocus auditory attention is vital for even the most routine day-to-day activities. Shifts in auditory attention can be initiated "voluntarily," or triggered "involuntarily" by unexpected novel sound events. Here we employed psychophysiological interaction (PPI) analyses of auditory functional MRI data, to compare functional connectivity patterns of distinct frontoparietal cortex regions during cued voluntary vs. novelty-driven involuntary auditory attention shifting. Overall, our frontoparietal seed regions exhibited significant PPI increases with auditory cortex (AC) areas du

Impaired anatomical connectivity and related executive functions: differentiating vulnerability and disease marker in bipolar disorder.

BACKGROUND: Bipolar 1 disorder (BD1) has been associated with impaired set shifting, increased risk taking, and impaired integrity of frontolimbic white matter. However, it remains unknown to what extent these findings are related to each other and whether these abnormalities represent risk factors or consequences of the illness. METHODS: We addressed the first question by comparing 19 patients with BD1 and 19 healthy control subjects (sample 1) with diffusion tensor imaging, the Intra-Extra Dimensional Set Shift Task, and the Cambridge Gambling Task. The second question we approached by appl

Integrated versus trial specific focus improves decision-making in older preschoolers.

This study explored the effect of integrated-focus (focusing on a depiction of overall gains/losses) versus trial-focus (focusing on gains/losses at each trial) on choice in a preschool variant of the Iowa Gambling task. Participants included 65 preschoolers (M = 47.82, SD = 7.29). Children completed two versions of the Preschool Gambling task, three cool executive function tasks, a moral reasoning task, and an affective perspective taking task. The results indicated that while the integrated-focus condition led to improvement in the awareness of the game, the condition effect was moderated by

The cognitive anchor system: a structural model of self, task selection, and latent intention in human cognition.

INTRODUCTION: Human cognition requires both stability and flexibility: individuals maintain a continuous sense of self while shifting among tasks, intentions, and contexts. Existing theories explain information processing, prediction, attention, and control, but they leave under-specified the structural layer that determines what cognition is currently organized around. METHODS: This theoretical paper develops the Cognitive Anchor System, a structural framework that distinguishes among the self anchor, the task anchor, and the anchor pool. The model is compared with major theories of attentio

The relationship between executive functioning and repetitive negative thinking in youth: A systematic review of the literature.

Repetitive negative thinking (RNT) and executive functioning (EF) deficits are each characteristic of many forms of youth psychopathology. Extensive work has examined the relationship between rumination, a form of RNT, and EF in adults. However, less is known about the relationship between RNT more broadly and EF in youth, for whom these constructs are developing and emerging. Here, we systematically and qualitatively reviewed 27 studies on the associations between EF (e.g., shifting, inhibition, working memory) and RNT (e.g., rumination, worry, obsessions) in youth. All forms of RNT were more

Feasibility and acceptability of lay counsellor-delivered single-session therapy: a pragmatic mixed-methods pilot cohort study protocol.

INTRODUCTION: Common mental disorders (CMDs) are endemic globally. The availability of evidence-based mental health interventions has grown rapidly, with many services focused on psychotherapy. Most psychotherapies require multiple sessions, resulting in lengthy waiting times due to limited provider availability. These delays can lead to higher morbidity, poorer adherence and less favourable outcomes.Recent research indicates that single-session therapy (SST) can be as effective as multi-session approaches and offers cost savings. This pragmatic, prospective mixed-methods pilot cohort study as

Nursing tasks and strain development among German home care nurses: results from an observational field study.

Despite the central role of home care in aging societies, little is known about the daily work processes and short-term strain experienced by home care nurses. This observational field study addresses this critical research gap by investigating the nursing tasks and short-term strain reported by home care nurses and nursing assistants in Germany. Using workplace observations and short surveys in combination, this study observed 46 German home care nurses from 25 organizations. Nursing tasks were coded in five-minute intervals, and strain outcomes (fatigue, engagement, monotony, mental satiatio

A Dedicated Population for Reward Coding in the Hippocampus.

The hippocampus plays a critical role in goal-directed navigation. Across different environments, however, hippocampal maps are randomized, making it unclear how goal locations could be encoded consistently. To address this question, we developed a virtual reality task with shifting reward contingencies to distinguish place versus reward encoding. In mice performing the task, large-scale recordings in CA1 and subiculum revealed a small, specialized cell population that was only active near reward yet whose activity could not be explained by sensory cues or stereotyped reward anticipation behav

Do amnesic patients with Korsakoff's syndrome use feedback when making decisions under risky conditions? An experimental investigation with the Game of Dice Task with and without feedback.

We investigated the role of feedback processing in decision making under risk conditions in 50 patients with amnesia in the course of alcoholic Korsakoff's syndrome (KS). Half of the patients were administered the Game of Dice Task (GDT) and the remaining 25 patients were examined with a modified version of the GDT in which no feedback was provided. Patients' results in the GDT and in the modified version were compared with that of 50 healthy subjects of whom 25 subjects performed the original GDT and 25 performed the modified version. While performance on the original GDT was superior to perf

An FMRI meta-analysis of interoception in eating disorders.

Eating Disorders (EDs) are associated with disturbed interoception - the sense of the internal condition of the body. Disturbances in interoception across senses have not yet been comprehensively examined in EDs. To do so, we employed an innovative Bayesian author-topic model approach to fMRI meta-analyses that pools together neural deficits across interoceptive senses and task types in participants with and recovered from EDs. Following PRISMA guidelines, our results combine activation patterns from 1,341 initially screened studies and data from 25 manuscripts that met study criteria that com

Alexithymia, cumulative feedback, and differential response patterns on the Iowa Gambling Task.

Although the role of emotional processing is central to contemporary models of risky decision making, to date the role of trait emotional understanding has not been explored experimentally in this context. The current experiment (N=326) explores the role of alexithymia with respect to performance on the standard Iowa Gambling Task (IGT) and a version where cumulative financial feedback is obscured. Standard learning on the IGT was observed for those low in alexithymia. Those high in alexithymia learned to avoid disadvantageous decks over the first half of the task. However, over the later tria

Are poor set-shifting and central coherence associated with everyday function in anorexia nervosa? A systematic review.

BACKGROUND: There is increasing interest in associations between cognitive impairments and clinical symptoms in Anorexia Nervosa (AN), however, the relationship with everyday function is unclear. The current review synthesizes existing data regarding associations between scores on tests of set-shifting and central coherence and functional outcome measures for individuals with AN. METHOD: A systematic electronic database search yielded 13 studies which included participants with current or lifetime AN where scores on a neuropsychological test of set-shifting or central coherence were directly

Reduced cognitive resources induces risky stepping strategies in older adults.

BACKGROUND: Falls frequently occur during situations that require a transfer or shift of bodyweight (e.g., stepping around an obstacle or turning). One contributor to falls in this scenario is a risky stepping strategy known as 'cross-stepping', whereby the feet cross over during the step. However, contributors to this potentially dangerous stepping strategy are not understood. This study examined whether reduced cognitive resources are a contributor to 'cross-stepping'. METHODS: Fifty-one older adults without neurological, musculoskeletal, or major cognitive impairment participated. Particip
