Transcatheter patent ductus arteriosus closure: what have we learned after over 25 years? A single--center experience with 1036 patients.

BACKGROUND: Transcatheter patent ductus arteriosus (PDA) closure has become the first‑choice method of treatment in the majority of patients. However, device selection poses a challenge. AIMS: This study aimed to analyze periprocedural and 1‑year outcomes of PDA transcatheter closure performed with different devices throughout a 25‑year time period in a single center. METHODS: All 1036 patients who underwent transcatheter PDA closure between 1993 and 2020 were included in retrospective analysis. Various devices were used: the Rashkind device (RD; n = 25), coils (n = 469), nitinol duct occlud

Progressive Enlargement of a Coronary Artery Communication With an Atrial Septal Defect Closure Device.

A 70-year-old man underwent coronary angiography 8 years after transcatheter atrial septal defect closure. Although no coronary stenosis was observed, a progressively enlarged coronary artery communication originating from the atrial branch of the left circumflex artery and extending into the device was identified. Serial imaging demonstrated progressive enlargement and elongation of this vascular structure over time. Computed tomography showed contrast opacification within the device and left-to-right leakage, suggesting incomplete neoendothelialization and peridevice leak. These findings may

Percutaneous Retrieval Strategies Following Left Atrial Appendage Occluder and Peri-Device Leak Closure Embolization.

BACKGROUND: Percutaneous LAAO is an established stroke-prevention strategy in atrial fibrillation patients unsuitable for long-term anticoagulation. Device embolization is uncommon but can cause severe complications requiring urgent intervention. ABSTRACT OF THE CASE: We report 2 related cases of device embolization associated with left atrial appendage occlusion and subsequent peri-device leak closure. The first involved early Amulet occluder embolization with mitral inflow obstruction and hemodynamic instability. The second occurred during peri-device leak closure after prior WATCHMAN impla

Occurrence of angiographic femoral artery complications after vascular closure with Mynx and AngioSeal.

INTRODUCTION: There have been recent reports of high vascular complication rates after the use of the Mynx vascular closure device (VCD). At our institution, vascular complications due to these devices have rarely been encountered. A study was undertaken to retrospectively compare angiographic abnormalities seen after femoral artery closure by both the Mynx and AngioSeal VCDs to provide further insight into the risks associated with VCDs. METHODS: All adult patients who underwent deployment of either a Mynx or AngioSeal VCD and subsequently underwent repeat angiography within the next 30 days

Technical Considerations for Wound Closure: A Plastic Surgery Perspective Part I.

Although most interventional radiology (IR) access wounds are small, they frequently occur in physiologically high-risk patients-including those who are anticoagulated, vasculopathic, diabetic, immunosuppressed, or previously irradiated. Even minor wound breakdown in this population can lead to hematoma, infection tracking to grafts or devices, pseudoaneurysm exposure, and reintervention. This invited review discusses reconstructive surgery principles and delves into a practical IR workflow to reduce wound complications. We focus on actionable strategies: hemostasis-first closure, preservation

Sheath Size Up and Down With Single Proglide - A Technique for Achieving Hemostasis With Use of Large Size Delivery System During Endovascular Graft Placement.

BACKGROUND: A total percutaneous approach for management of aortic pathologies with endovascular grafts requires the use of large size delivery-systems. The diameter of these delivery systems usually exceeds the recommended sizes for most of the currently available percutaneous closure devices. A safe, effective and simple vascular access site closure device is desirable for success for percutaneous procedures for aortic pathologies. Hence, we aim to study safety and effectiveness of the use of a single suture based vascular access closure device (ProGlide) using a technique involving serial u

Paravalvular leak closure after TAVI with plugs: A literature review with a specific focus on arterio-arterial loop.

More than mild paravalvular leak (PVL) following transcatheter aortic valve implantation (TAVI) is associated with a twofold increase in all-cause mortality, heart failure hospitalizations, and the need for reintervention. Successfully addressing PVL in TAVIs is more challenging than in surgical valves. The arterial-arterial (A-A) rail technique emerges as a valuable strategy for post-TAVI PVL closure, enhancing success rates by enabling the effective use of lower-profile vascular plug devices. When standard approach is ineffective, generating an A-A loop for post-TAVI PVL closure is probably

Endovascular intervention in the treatment of congenital heart disease in adults.

Over the last years, endovascular intervention have become an important part of treatment in patients with congenital heart disease particularly for residual defects after surgery done in infancy. These transcatheter procedures can be described as dilatation of stenotic sites (angioplasty, endovascular stenting and valvuloplasty) or as a closure of anomalous openings (device closure defects and vascular embolisation). Balloon valvuloplasty, without or with stent, is the procedure of choice in adults with pulmonary valve stenosis, pulmonary arteries stenosis, bicuspid aortic valve stenosis with
