Hyperbaric oxygen-associated seizure leading to stroke.

Oxygen toxicity seizures are a well-known complication of hyperbaric oxygen treatment (HBOT). Until now, there have not been any reported cases of an acute ischaemic event (stroke) as the result of a HBOT-associated oxygen toxicity seizure. We report an event in which a seizure and stroke occurred together and consider that the stroke may have been caused by seizure-induced demand ischaemia. This challenges the generally held view that oxygen toxicity seizures in the clinical hyperbaric setting are benign. A discussion of the literature on the subject of seizure-induced brain injury is include

Preventive effect of rosiglitazone on liver injury in a mouse model of decompression sickness.

BACKGROUND AND AIMS: Severe decompression sickness (DCS) is a multi-organ injury. This study investigated the preventive effects of rosiglitazone on liver injury following rapid decompression in mice and examined the underlying mechanisms. METHODS: Mice were randomly divided into four groups: a control group, vehicle group, and rosiglitazone (5 and 10 mg·kg⁻¹) groups, the latter three being exposed to a pressure of 911 kPa. Haematoxylin and eosin staining, plasma levels of alanine transaminase (ALT), aspartate transaminase (AST) and lactate dehydrogenase and blood cell counts were used to eva

Tenth European Consensus Conference on Hyperbaric Medicine: recommendations for accepted and non-accepted clinical indications and practice of hyperbaric oxygen treatment.

The tenth European Consensus Conference on Hyperbaric Medicine took place in April 2016, attended by a large delegation of experts from Europe and elsewhere. The focus of the meeting was the revision of the European Committee on Hyperbaric Medicine (ECHM) list of accepted indications for hyperbaric oxygen treatment (HBOT), based on a thorough review of the best available research and evidence-based medicine (EBM). For this scope, the modified GRADE system for evidence analysis, together with the DELPHI system for consensus evaluation, were adopted. The indications for HBOT, including those pro

Proliferative retinopathy during hyperbaric oxygen treatment.

A 43-year-old male with type 2 diabetes mellitus, treated with insulin for 28 years and with an HbA1c of 7.9% six months prior, suffered from bilateral plantar ulcers refractory to specialised wound care. He underwent a planned 40 sessions of hyperbaric oxygen treatment (HBOT) at 243 kPa for 90 minutes. Consent was given for this report. The patient's ophthalmic history included bilateral proliferative diabetic retinopathy (PDR) identified on routine diabetic eye screening three years previously. This was treated with pan-retinal photocoagulation (PRP). Three months before starting HBOT, he un

Identifying and acting on potentially inappropriate care? Inadequacy of current hospital coding for this task.

INTRODUCTION: Recent Australian attempts to facilitate disinvestment in healthcare, by identifying instances of 'inappropriate' care from large Government datasets, are subject to significant methodological flaws. Amongst other criticisms has been the fact that the Government datasets utilized for this purpose correlate poorly with datasets collected by relevant professional bodies. Government data derive from official hospital coding, collected retrospectively by clerical personnel, whilst professional body data derive from unit-specific databases, collected contemporaneously with care by cli

The measurement of Eustachian tube function in a hyperbaric chamber using an ear canal microphone.

OBJECTIVE: The purpose of this study was to further the understanding of the opening of the Eustachian tube in relation to changes in barometric pressure. DESIGN: An ear canal microphone was used to measure the specific sounds related to tube opening and possible eardrum movements. Five subjects with normal tube function were examined in a hyperbaric chamber (up to 304 kPa). All active and passive equalization events were recorded and correlated with the subjectively perceived pressure regulation in the measured ear. RESULTS: The signals recorded were clear and reproducible. The acoustic ana

Poorly designed research does not help clarify the role of hyperbaric oxygen in the treatment of chronic diabetic foot ulcers.

Diabetic foot ulcers (DFUs) are one of the most common indications for hyperbaric oxygen treatment (HBOT). The role of HBOT in DFUs is often debated. Recent evidence based guidelines, while recommending its use, urge further studies to identify the patient subgroups most likely to benefit from HBOT. A recent study in Diabetes Care aimed to assess the efficacy of HBOT in reducing the need for major amputation and improving wound healing in patients with chronic DFUs. In this study, patients with Wagner grade 2-4 diabetic foot lesions were randomly assigned to have HBOT (30 sessions/90 min/244 k

The use of hyperbaric oxygen treatment for sudden sensorineural hearing loss in Europe.

BACKGROUND: The aim of this study was to identify the practice differences in the use of hyperbaric oxygen treatment (HBOT) for sudden sensorineural hearing loss (SSNHL) in Europe. MATERIALS AND METHODS: A questionnaire comprising nine questions was built using the surveymonkey.com website. The medical directors of hyperbaric centres in Europe were invited by e-mail to complete the survey. RESULTS: A total of 192 centres were invited to participate, of which 80 (41.6%) from 25 countries responded. Of these, 70 were using HBOT for SSNHL. The number of patients with SSNHL treated in these cent

Tympanic membrane bleeding complications during hyperbaric oxygen treatment in patients with or without antiplatelet and anticoagulant drug treatment.

INTRODUCTION: Middle ear barotrauma (MEBt) is a frequently occurring complication of hyperbaric oxygen treatment (HBOT). High-grade MEBt may involve tympanic membrane (TM) haemorrhaging. Although many patients undergoing HBOT use antiplatelet or anticoagulant drugs, it is unknown whether these drugs increase the risk of MEBt and particularly TM bleeding complications. METHODS: This multicentre, prospective cohort study investigates the prevalence of MEBt and TM bleeding during HBOT in patients using antiplatelet/anticoagulant drugs, compared with control patients not on such medications. MEBt

Grommets in HBOT patients: GA vs LA, unanswered questions.

We read with interest the article on grommet procedures for patients undergoing hyperbaric oxygen therapy (HBOT), and have a number of comments. It appears the authors may have missed a number of cases. In a previous paper from The Townsville Hospital Hyperbaric Unit (TTH HMU), Commons et al presented 14 of 106 patients (13%) who required grommets over the period between June 2009 and May 2010. These patients are included in the Lamprell et al data set. Figure 1 shows an apparent spike in their cases in 2010 (n = 13, part of the period covered in the previous paper) when compared to the remain

Economic outcomes in clinical studies assessing hyperbaric oxygen in the treatment of acute and chronic wounds.

INTRODUCTION: Hyperbaric oxygen treatment (HBOT) is used to treat acute and chronic wounds. This systematic review was conducted to summarise and evaluate existing evidence on the costs associated with HBOT in the treatment of wounds. METHODS: We searched multiple electronic databases in March 2015 for cohort studies and randomised clinical trials (RCTs) that reported on the clinical effectiveness and treatment costs of HBOT in the treatment of acute or chronic wounds. RESULTS: One RCT and three cohort studies reported on economic as well as clinical outcomes. These studies comprised differe

Management of severe spinal cord injury following hyperbaric exposure.

There is an increasing body of evidence that drainage of lumbar cerebrospinal fluid (CSF) improves functional neurological outcome after reperfusion injury to the spinal cord that occasionally follows aortic reconstructive surgery. This beneficial effect is considered owing to lowering of the CSF pressure thereby normalising spinal cord blood flow and reducing the 'secondary' cord injury caused by vascular congestion and cord swelling in the relatively confined spinal canal. Whilst lacking definitive proof, there are convincing randomised controlled trials (RCTs), cohort data and systematic re

The cardiac effects of hyperbaric oxygen at 243 kPa using inchamber echocardiography.

INTRODUCTION: The adverse effects of hyperbaric oxygen (HBO) on cardiac physiology are considered a potential hazard during the treatment of some patients. The haemodynamic effects of HBO are poorly understood and the incompatibility of electrical equipment inside the chamber has made assessment difficult. At Fremantle Hyperbaric Unit, we have modified an ultrasound machine (Logiq™ e) for safe use within the hyperbaric environment. The aim of this study was to evaluate the cardiac changes that occur during HBO using in-chamber transthoracic echocardiography (TTE) in subjects without evidence o

A prospective analysis of independent patient risk factors for middle ear barotrauma in a multiplace hyperbaric chamber.

INTRODUCTION: Middle ear barotrauma (MEBT) is the most common complication of hyperbaric oxygen therapy (HBOT). We wished to determine whether independent risk factors could predict which patients will require tympanostomy tubes in order to continue HBOT. METHODS: Data regarding demographics, medical history and physical examination were collected prospectively over one year. Multivariate logistic regression was used to analyse the data. RESULTS: One hundred and six patients were included. The cumulative risk of MEBT over the first five treatments was 35.8% and that for needing tympanostomy

The effect of intravenous perfluorocarbon emulsions on whole-body oxygenation after severe decompression sickness.

INTRODUCTION: Decompression sickness (DCS) results from a decrease in ambient pressure leading to supersaturation of tissues with inert gas and bubble formation. Perfluorocarbons (PFCs) are able to dissolve vast amounts of non-polar gases. Intravenous (IV) PFC emulsions reduce both morbidity and mortality associated with DCS, but the mechanism of this protective effect has not yet been demonstrated. METHODS: Juvenile Dorper-cross sheep (n = 31) were anaesthetised and instrumented for physiological monitoring, IV fluid administration and blood sampling. Animals were compressed in air in a hype

Hyperbaric oxygen in the treatment of childhood autism: a randomised controlled trial.

BACKGROUND: Promising results with hyperbaric therapy for children with autism have been reported, but most involved the use of only mild pressure with oxygen supplementation. To date, there has been no randomised, blinded trial of 100% oxygen administered at hyperbaric pressure. This study evaluated the efficacy of hyperbaric oxygen therapy (HBOT). METHODS: Sixty Thai children with autism, aged three to nine years, were randomly assigned to receive 20 one-hour sessions of either HBOT at 153 kPa (1.5 ATA) or sham air at 116 kPa (1.15 ATA). Effects on behaviour were measured using the Autism T

Severe hydrogen sulphide poisoning treated with 4-dimethylaminophenol and hyperbaric oxygen.

UNLABELLED: INTRODUCTION Hydrogen sulphide (H2S) is a highly toxic gas which originates mainly during breakdown of organic matter under anaerobic conditions. After inhalation, H2S binds to mitochondrial respiratory enzymes preventing oxidative phosphorylation, thereby causing reversible inhibition of aerobic metabolism and cellular anoxia. The use of hyperbaric oxygen therapy (HBOT) for H2S poisoning remains controversial, but has a similar underlying rationale to that in carbon monoxide poisoning. METHODS: A retrospective review of patients with severe H2S intoxication who presented during 2

Hyperbaric oxygen in the treatment of asphyxia in two newborn infants.

Hypoxic-ischaemic encephalopathy (HIE) is a common cause of brain damage in the neonatal period. Approximately 10% of births involve some degree of asphyxia, and 1% of these are severe. Current treatment has been limited to supportive measures and the recent use of hypothermia. Beneficial effects of hyperbaric oxygen treatment (HBOT) in neonatal asphyxia have been reported in the Chinese literature. We report the use of HBOT to treat two term neonates with moderate HIE according to Sarnat's classification. Clinical improvement occurred following HBOT. A 50% decrease in the total creatine phosp
