Safety and Feasibility of a Novel Split Design Left Atrial Appendage Occluder: From Preclinical Investigations to First-in-Human Application.

OBJECTIVE: This study was designed to first evaluate the histological compatibility of the split occluder in preclinical animal models, followed by clinical validation of its procedural feasibility and occlusion effectiveness in a first-in-human trial cohort. METHODS: Eight healthy labrador retrievers were selected for this study. A split-type occluder was introduced to occlude their left atrial appendage. The experimental animals were euthanized at 1, 3, and 6 months, respectively. Prior to being euthanized, transesophageal echocardiogram (TEE) and pulmonary vein computed tomography venograp

Deep Learning Image Reconstruction Improves Image Quality in Dual-Low Dose Dual-Energy CT Portal Venography Compared to Adaptive Iterative Image Reconstruction Algorithm-Veo.

BACKGROUND: Deep learning image reconstruction (DLIR) has gained recognition as a promising technique to improve image quality in low-dose CT imaging. However, its performance in dual-energy CT portal venography (DE-CTPV), particularly under reduced contrast medium volume and radiation dose (dual-low dose) conditions, remains underexplored. OBJECTIVE: This study aims to compare the performance of DLIR and adaptive statistical iterative reconstruction (ASIR-V) in DE-CTPV, with a focus on image quality across multiple vascular segments of the portal venous (PV) system under dual-low dose protoc

Predicting Outcomes in Pediatric Intraluminal Pulmonary Vein Stenosis Using a Comprehensive Standardized Catheterization Assessment: A Prospective Study.

BACKGROUND: Pediatric intraluminal pulmonary vein stenosis (PVS) occurs in a heterogeneous patient population but is unified by its cellular composition in affected pulmonary veins. Enhanced delineation of the hemodynamic and physiological vein consequences of PVS and its impact on vein lumen, vein pressure, wall structure, vein compliance, and wall shear stress has not been studied. The objective is to assess PVS characteristics using multimodal catheterization techniques (angiography, pulmonary vein pressure, intravascular ultrasound, balloon compliance testing, and wall shear stress calcula

Detection and Coil Embolisation of Pelvic Vein Incompetence for Chronic Pain in the Female Pelvis: A Development Phase Economic Analysis.

OBJECTIVE: Unexplained chronic pain in the female pelvis (CPFP) affects 7% of people of female sex in the UK. Evidence suggests that pelvic vein incompetence (PVI) could explain CPFP and that coil embolisation could provide relief. The aims of this study were to indicate (1) the cost effectiveness of detecting and treating PVI in people experiencing unexplained CPFP, and (2) the maximum value of further research, suggesting suitable areas. METHODS: A decision tree compared standard care (regular prescribed analgesia) with an intervention comprising transvaginal duplex ultrasound to screen for

A novel 3D multimodal fusion imaging surgical guidance in microvascular decompression for primary trigeminal neuralgia and hemifacial spasm.

BACKGROUND: Neurovascular compression (NVC) is a primary etiology of trigeminal neuralgia (TN) and hemifacial spasm (HFS). Despite Magnetic Resonance Tomographic Angiography (MRTA) being a useful tool for 3D multimodal fusion imaging (MFI) in microvascular decompression (MVD) surgery planning, it may not visualize smaller arterial vessels and veins effectively. We validate a novel computed tomography angiography and venography (CTA/V) - diffusion tensor tractography (DTT) -3D-MFI to enhance the MVD surgical guidance. METHODS: In this prospective study, 80 patients with unilateral primary TN o

Comparison of the rate of concomitant proximal venous stenosis between the upper and lower extremities in patients with secondary lymphedema undergoing lymphaticovenous anastomosis.

BACKGROUND: Concomitant iatrogenic proximal venous stenosis increases venous pressure and can be a risk factor for unfavorable outcomes of lymphaticovenular anastomosis (LVA) in extremities with secondary lymphedema. This study investigated the frequency and relevant factors of venous stenosis in patients diagnosed with secondary lymphedema who underwent LVA. METHODS: Patients who underwent preoperative computed tomographic venography (CTV) and LVA for secondary lymphedema of the extremities from October 2018 to March 2022 were included. The incidence of proximal venous stenosis in the affect

Vascular anatomy in children with univentricular hearts regarding transcatheter bidirectional Glenn anastomosis.

BACKGROUND: Transcatheter stent-secured Glenn anastomosis, aiming to reduce the invasiveness of palliation in patients with univentricular heart defects, has been reported in large experimental animals. The advent of biodegradable stents and tissue-engineered vascular grafts will make this procedure a reality in human patients. However, the relationship between the superior vena cava (SVC) and the right pulmonary artery (RPA) is different in humans. AIM: To characterise vascular anatomy in children with univentricular hearts, regarding technical aspects and device design for this procedure.

Correlation between pelvic congestion syndrome and body mass index.

OBJECTIVE: Previous studies of men suggested that patients with varicocele may be leaner than the normal population. No such work exists in women with pelvic congestion syndrome (PCS). This study evaluated the correlation between body mass index (BMI) and PCS. METHODS: A case-control study at a single institution was performed of women who had the objective diagnosis of PCS. Data were prospectively collected for 100 consecutive patients with PCS. BMI was calculated for every patient and categorized into normal weight (BMI <25), overweight (BMI 25-29.9), and obese (BMI >30). Diagnosis of PCS w

[Remote results of veno-venous bypass operations in post-thrombotic disease].

The present study was aimed at analyzing the remote results of the operation of cross-over autovenous bypass at terms varying from 2 to 28 years in a total of 68 patients presenting with unilateral post-thrombotic occlusive lesions of iliac veins and in 12 patients operated on for obstruction of the femoral vein (saphenopopliteal bypass grafting). The obtained findings showed that a decisive factor of successful cross-over bypass grafting was a sufficient diameter of the autovenous transplant (not less than 7-8 mm). It was determined that in 70.6% of patients the cross-over bypass grafts becom
