Worldwide pacemaker and defibrillator reuse: Systematic review and meta-analysis of contemporary trials.

BACKGROUND: Patients go without pacemaker, defibrillator, and cardiac resynchronization therapies (devices) each year due to the prohibitive costs of devices. OBJECTIVE: We sought to examine data available from studies regarding contemporary risks of reused devices in comparison with new devices. METHODS: We searched online indexing sites to identify recent studies. Peer-reviewed manuscripts reporting infection, malfunction, premature battery depletion, and device-related death with reused devices were included. The primary study outcome was the composite risk of infection, malfunction, prem

Risk Stratification of Future Left Ventricular Dysfunction for Patients with Indications for Right Ventricular Pacing due to Bradycardia.

Although right ventricular (RV) pacing is the only effective treatment for patients with symptomatic bradycardia, it creates left ventricular (LV) dyssynchrony, which can induce LV dysfunction and heart failure. The current criterion for consideration of cardiac resynchronization therapy (CRT) is LV ejection fraction (LVEF) ≤ 35%, but indication for CRT in patients required for RV pacing with LVEF > 35% remains unclear. We studied 40 patients, all LVEF ≥ 35%, who had undergone implantable cardioverter-defibrillator implantation with RV pacing < 5%. Echocardiography was performed at baseline and…

High Incidence of Dilated Cardiomyopathy After Right Ventricular Inlet Pacing in Patients With Congenital Complete Atrioventricular Block.

BACKGROUND: Some patients with congenital complete atrioventricular block (CCAVB) develop dilated cardiomyopathy (DCM) after pacemaker implantation (PMI). We evaluated the relationship between pacing site and DCM incidence. METHODS AND RESULTS: We retrospectively evaluated 38 patients with CCAVB; 8 (25%) of 32 patients who had PMI developed DCM/heart failure death (HFD) after PMI, although none of the 6 patients without PMI showed DCM/HFD. All DCM/HFD occurred within 50 months of PMI. Among the 32 patients with PMI, the DCM/HFD incidence was 55% (6/11) for right ventricular inlet (RVI), 18% (

Effects of biventricular pacing and scar size in a computational model of the failing heart with left bundle branch block.

OBJECTIVES: To study the impact of biventricular pacing (BiV) and scar size on left ventricular (LV) regional and global function using a detailed finite element model of three-dimensional electromechanics in the failing canine heart. BACKGROUND: Cardiac resynchronization therapy (CRT) clinical trials have demonstrated that up to 30% of patients may be classified as non-responders. The presence of a scar appears to contribute to those that do not respond to CRT. A recent study in patients with myocardial scar showed that LV dyssynchrony was the sole independent predictor of reverse remodeling…

Cardiac resynchronization improves coronary blood flow.

Asynchronous ventricular activation, induced by left bundle branch block, is known to have deleterious effects on the systolic and diastolic functions of the left ventricle (LV). Cardiac resynchronization therapy (CRT) has been proposed as a complementary method to improve the LV systolic performance by restoring the synchronized contraction patterns in patients with advanced heart failure and left bundle branch block. However, the effect of CRT on myocardial blood flow is not well established. In the present study, we therefore examined the coronary blood flow in 20 patients with idiopathic d…

Sequential versus simultaneous biventricular resynchronization for severe heart failure: evaluation by tissue Doppler imaging.

BACKGROUND: Cardiac resynchronization therapy (CRT) by means of simultaneous biventricular pacing improves left ventricular systolic performance and synchrony in patients with heart failure and bundle-branch block. We used tissue tracking and 3D echocardiography to evaluate the impact of sequential CRT with individualized interventricular delay programming. METHODS AND RESULTS: Twenty consecutive patients with severe heart failure and left bundle-branch block were included. Tissue tracking and 3D echocardiography were carried out before and on the day after pacemaker implantation. Eleven diff…

Lucky Luke in the Cath lab? Use of a snare catheter to optimize positioning in transcatheter tricuspid valve replacement: a case report.

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) has emerged as a therapeutic option for patients with severe tricuspid regurgitation (TR) who are unsuitable for transcatheter edge-to-edge repair (T-TEER). While procedural outcomes continue to improve, device delivery remains a critical step and may be particularly challenging in complex anatomies. CASE SUMMARY: We report on a 75-year-old patient with heart failure and severe TR of mixed aetiology with a history of cardiac resynchronization therapy (CRT). Surgical repair was deemed prohibitive due to high surgical risk, and transc…

Coronary Sinus Stenting for the Management of Left Ventricular Lead Displacement during Resynchronization Therapy: A Report of Two Cases.

In patients with cardiac resynchronization therapy (CRT), loss of left ventricular (LV) stimulation occurs chiefly because of LV lead dislodgement. The occurrence rate of LV lead dislodgement in different reports is between 2% and 12% of patients. LV lead dislodgement precludes clinical improvement. We describe 2 patients with heart failure, fulfilling the criteria for CRT implantation. In both patients, right ventricular and right atrial leads were implanted via the left subclavian vein in the right ventricular apex and the right atrial appendage, respectively. Repeated LV lead implantation w…

Mechanical dyssynchrony and deformation imaging in patients with functional mitral regurgitation.

Chronic functional mitral regurgitation (FMR) is a frequent finding of ischemic heart disease and dilated cardiomyopathy (DCM), associated with unfavourable prognosis. Several pathophysiologic mechanisms are involved in FMR, such as annular dilatation and dysfunction, left ventricle (LV) remodeling, dysfunction and dyssynchrony, papillary muscles displacement and dyssynchrony. The best therapeutic choice for FMR is still debated. When optimal medical treatment has already been set, a further option for cardiac resynchronization therapy (CRT) and/or surgical correction should be considered. CRT…

[Electrical storm in ICD patients: prevention and treatment].

Medical progress and demographic changes cause a continuous increase in patients with implantable cardioverter-defibrillators (ICD). Up to one third of patients with ICDs for secondary prevention and half of the patients with previous electrical storm (ES) will suffer from (further) ESs. When multiple ICD shocks are reported by patients (ICD storm), appropriate, inappropriate and phantom shocks have to be distinguished. Reported shocks without clinical correlates (phantom) often affect patients suffering from posttraumatic stress syndrome after an ICD storm. Approximately one third of all ICD
