One hundred patients with a 3rd ventricle width of 12 mm or more were examined for the fifth time in 1976 after an average observation period of 20.8 years. On first admission, a predominant aetiological factor was found in 27 cases. Two patients had air-encephalogrophical findings indicating normal pressure hydrocephalus. One of these had a shunt operation, however, without improvement. Seventy-one patients had died, 7 patients were in need of care and supervision, 11 patients were unable to work, the remaining 11 were able to work to some extent. The group studied had a significantly increas

Idiopathic normal-pressure hydrocephalus (Hakim's disease) is characterized by ventricular enlargement and disproportionately enlarged subarachnoid space hydrocephalus, leading to localized brain deformation. Differentiating regional brain volume changes in Hakim's disease from those in Alzheimer's disease, Hakim's disease with Alzheimer's disease, and mild cognitive impairment provides insights into disease-specific mechanisms. This study aimed to identify disease-specific patterns of brain volume changes in Hakim's disease, Alzheimer's disease, Hakim's disease with Alzheimer's disease, and m

Disorders of cerebrospinal fluid (CSF) dynamics, including idiopathic normal pressure hydrocephalus (iNPH), idiopathic intracranial hypertension (IIH), and spontaneous intracranial hypotension (SIH) frequently present clinical and imaging phenotypes that overlap with other disease processes. Although conventional MRI remains central for identifying characteristic morphologic features, many imaging findings have variable sensitivity, particularly across disease stages. As a result, current diagnostic pathways and treatment selection is still often reliant on invasive, snapshot-based physiologic

Improvement in gait after a cerebrospinal fluid (CSF) tap test is a key indicator for shunt surgery in idiopathic normal pressure hydrocephalus (iNPH) patients. However, quantitative analysis of gait requires sophisticated equipment and specialists that limit practical use. Development of Bluetooth-connected sensors offers affordable way to assess gait. We present a case of iNPH patient in whom gait changes were serially assessed using a smart insole before and after intervention, which helped in clinical decision making. A 68-year-old female who showed the triad of iNPH symptoms (gait disturb

BACKGROUND: The measurement of optic nerve sheath diameter (ONSD) as a non-invasive method of estimating intracranial pressure has been widely reported in the literature. However, few studies have evaluated the accuracy of magnetic resonance imaging (MRI) in assessing ONSD measurements, although it is considered a very reliable method, it is not easily repeatable, expensive and is not readily available bedside. Herein, an assessment of the intra- and inter-rater reliability of ONSD assessment using MRI was conducted.
METHODS: A consecutive, prospective cohort of patients with suspected idiopa

Ventriculoperitoneal shunt (VPS) placement is one of the commoner neurosurgical procedures worldwide. The purpose of this article is to report a case of delayed intraventricular hemorrhage (IVH) following a VPS and to review the literature regarding anatomic factors that could potentially explain this rare complication. A 78-year-old man with normal pressure hydrocephalus, who underwent an uneventful right VPS placement, suffered from a catastrophic isolated IVH five days later. The reported cases of delayed intracerebral hemorrhage (ICH) following VPS are rare and those with IVH are even rare

PURPOSE: A case of schwannoma of the cauda equina in which the initial manifestation was visual loss, due to papilledema, is described.
OBSERVATION: A 56-year-old man presented a history of 1 month of progressively blurred vision. On examination, visual acuity was 20/1,000 due to bilateral papilledema without spinal symptoms or neurological signs. Brain computed tomography demonstrated a nonobstructive hydrocephalus. He underwent a ventriculoperitoneal shunt without improvement. One month later, he complained of disturbance while walking associated with lower back pain. A lumbar puncture reve

We describe our experience with Sophy programmable valve shunts, compared with Codman-Hakim programmable shunts in cases with normal pressure hydrocephalus (NPH) after subarachnoid hemorrhage (SAH). A total of 147 consecutive patients underwent 204 shunt implantations (102 Sophy valves, 51 Codman-Hakim valves, 51 nonprogrammable valves). Of these, 23 Sophy and 25 Codman-Hakim valves respectively were implanted into patients with NPH after SAH. Comparison of reprogramming rate and frequency in cases with NPH after SAH between Sophy and Codman-Hakim valves revealed similar reprogramming rates (6

We have reported a rare case of spontaneous 3rd ventriculostomy with spontaneous arrest of obstructive hydrocephalus. A 41 year old man, who had had an intermittent headache for about a year, was admitted to the department of neurosurgery Kitasato University with chief complaints of sudden onset of severe headache, vomiting and disturbance of consciousness. At the time of admission, 30 minutes after the onset of symptoms, the positive neurological findings were delirious state of consciousness, miotic pupils with sluggish reaction to light, mild hemiparesis on the left site and slight nucnal r

Idiopathic intracranial hypertension (IIH) is a disorder characterized by elevated intracranial pressure (ICP) in the absence of intracranial mass lesions, hydrocephalus, or infection. The condition most commonly presents with headache and papilledema; however, idiopathic intracranial hypertension without papilledema (IIHWOP) is increasingly recognized as a clinically important variant that may lead to diagnostic delay and progressive visual impairment. Established risk factors include obesity, female sex, and hormonal influences, while IIH in people living with HIV remains uncommon. We report

BACKGROUND AND PURPOSE: To date, no pathophysiological model has sufficiently accounted for all the findings encountered in patients with idiopathic intracranial hypertension (IIH). Intracranial elastance is an index of volume-buffering capacity known to play a role in certain disorders of cerebrospinal fluid (CSF) dynamics, which has not been previously investigated in relation to IIH patients.
METHODS: This was a single-center retrospective cohort study from 1 July 2011 to 1 July 2016. Values for opening pressure (PO ), closing pressure (PC ) and volume (V) of CSF removed were collected, as

BACKGROUND: Overdrainage after cerebrospinal fluid diversion remains a significant morbidity. The hydrostatic, gravitational force in the upright position can aggravate this. Siphon control (SC) mechanisms, as well as programmable and flow regulating devices, were developed to counteract this. However, limited studies have evaluated their safety and efficacy. In this study, direct comparisons of the complication rates between siphon control (SC) and non-SC (NSC), fixed versus programmable, and flow- versus pressure regulating valves are undertaken.
METHODS: A retrospective chart review was pe

Ventriculomegaly is a key neuroimaging feature of normal pressure hydrocephalus (NPH) and other disorders of cerebrospinal fluid (CSF) dynamics. The Evans Index (EI), defined as the ratio of maximal frontal horn width to maximal inner skull diameter, is a widely used marker of ventricular enlargement. However, manual EI measurement is susceptible to inter-observer variability and depends on accurate alignment to the anterior commissure-posterior commissure (AC-PC) plane, limiting reproducibility in large and multi-center studies. We developed a fully automated deep learning-based pipeline for

Several psychiatric, neurologic and neurodegenerative disorders present increased brain ventricles volume, being hydrocephalus the disease with the major manifestation of ventriculomegaly caused by the accumulation of high amounts of cerebrospinal fluid (CSF). The molecules and pathomechanisms underlying cerebral ventricular enlargement are widely unknown. Kinase D interacting substrate of 220 kDa (KIDINS220) gene has been recently associated with schizophrenia and with a novel syndrome characterized by spastic paraplegia, intellectual disability, nystagmus and obesity (SINO syndrome), disease

OBJECTIVES: To determine current epidemiology and clinical characteristics of cerebrospinal fluid (CSF) shunt surgery, including revisions.
METHODS: A retrospective, multicentre, registry-based study was conducted based on 10 years' data from the UK Shunt Registry, including primary and revision shunting procedures reported between 2004 and 2013. Incidence rates of primary shunts, descriptive statistics and shunt revision rates were calculated stratified by age group, geographical region and year of operation.
RESULTS: 41 036 procedures in 26 545 patients were submitted during the study peri

Ventriculoperitoneal (VP) shunt insertion is a common neurosurgical procedure, essentially unchanged in recent years, with high revision rates. We aimed to identify potentially modifiable associations with shunt failure. One hundred and forty patients who underwent insertion of a VP shunt from 2005-2009 were followed for 5-9years. Age at shunt insertion ranged from 0 to 91years (median 44, 26% <18years). The main causes of hydrocephalus were congenital (26%), tumour-related (25%), post-haemorrhagic (24%) or normal pressure hydrocephalus (19%). Fifty-eight (42%) patients required ⩾1 shunt revis

A 59-year-old woman with 45 years history of systemic lupus erythematosus (SLE) and antiphospholipid syndrome was admitted to our hospital with progressive disturbances in consciousness, cognitive impairment and paraparesis following pneumonia and cerebral infarction. Diagnostic workup revealed anti-aquaporin-4 antibody positivity and a longitudinally extensive transverse myelitis lesion, leading to a diagnosis of neuromyelitis optica spectrum disorder (NMOSD). Additionally, ventricular enlargement and cerebrospinal fluid (CSF) findings raised suspicion of normal pressure hydrocephalus (NPH).

Monoclonal immunoglobulin deposition diseases (MIDDs), including light and heavy chain deposition disease (LHCDD), are rare and heterogeneous disorders associated with underlying B-cell clonal disorders. Adrenal involvement is a potential extrarenal manifestation of MIDDs; however, limited data are available regarding its prevalence and clinical presentation. Herein, the present report describes, for the first time, a case of primary adrenal insufficiency that developed twenty-two years after a diagnosis of LHCDD had been made. BACKGROUND: Lumboperitoneal (LP) shunt placement is a good option for treating elderly patients with communicating normal pressure hydrocephalus (NPH) who are also on antiplatelet therapy following endovascular treatment of unruptured bilateral internal carotid artery aneurysms. Here, in an 80-year-old male with an LP shunt, the catheter was "pinched" between adjacent spinous processes, resulting in laceration of the catheter and intrathecal catheter migration.
CASE DESCRIPTION: An 80-year-old male was treated with a LP shunt for NPH 1 year after undergoing endovascular treatment of unrupture
