Serratia marcescens Bloodstream Infections in a Neonatal Intensive Care Unit: A Retrospective Case Series From Rabat, Morocco.

Background Serratia marcescens is an opportunistic Gram-negative bacillus increasingly implicated in neonatal intensive care unit (NICU) inf … le contrast-enhancing brain lesions on magnetic resonance imaging (MRI), consistent with multifocal brain abscess formation. All patients received empirical broad-spectrum antibiotic therapy, subsequently adjusted according to antimicrobial susceptibility testing. Overall mortality was 7/16 (43.8%). Conclusion This single-center case series demonstrates the pot …

Reliability of Frameless Stereotactic Brain Biopsy for Complicated Lesions: Multicenter Experiences.

BACKGROUND: This study aimed to report our experiences with the safety and reliability of frameless stereotactic brain biopsy, including eloquent areas and small-sized lesions. MATERIALS AND METHODS: We reviewed 88 consecutive cases of brain lesions diagnosed with neuronavigation-guided frameless stereotactic biopsy in two different neurosurgical institutes (Turkey and Germany). In addition to a standard technique, magnetic resonance imaging (MRI)-computed tomography (CT) fusion images were used to enhance the accuracy of targeting. To stabilize the navigation system on the target, the deviat

Impact of Focal Muscle Vibration on Flaccid Upper Limb Motor Paralysis following Acute Brain Disease: A Case Study.

Focal muscle vibration (FMV) is increasingly being recognized as a rehabilitative therapy for enhancing motor function in central nervous sy … paralysis immediately after acute CNS diseases. We report the case of a septuagenarian male with a brain abscess in the right parietal lobe, leading to flaccid motor paralysis. Rehabilitation included 28 sessions of occupational and physical therapy that incorporated FMV. Significant improvements were observed in upper extremity function, with moderate to very …

Efficacy of decompressive craniectomy: A retrospective case series study with 321 patients and an update on controversies.

Decompressive craniectomy (DC) is considered a cornerstone in the management of refractory intracranial hypertension. For decades, DC was kn … ients with other neurosurgical entities that underwent DC, such as subarachnoid hemorrhage, tumors, brain abscess and cerebral ventricular sinus thrombosis events. The present study enrolled a total of 321 patients who underwent DC. Group A included 52 out of the 321 (16.1%) patients, group B included 51 (15.8%) patients, group C included 164 (51.0%) patients, …

Cerebral syphilitic Gumma in the modern era: a report of an unusual case and brief review of recent published reports.

BACKGROUND: Cerebral syphilitic gummas are rare. However, numerous case reports on them have been published recently, consistent with the resurgence of syphilis and its accompanying atypical manifestations. We here present a patient with a cerebral syphilitic gumma and an unusual clinical course and review recent case reports. CASE PRESENTATION: A 49-year-old woman had a generalised seizure and was found by computed tomography and magnetic resonance imaging to have a brain mass that mimicked a brain abscess or malignant tumour. Further imaging with magnetic resonance spectroscopy and positron…

Listeria meningoencephalitis in an immunocompetent person.

Listeria monocytogenes is a small, aerobic or facultative anaerobic, non-sporulating gram positive bacillus that can be isolated from soil, vegetation or animal reservoirs. There are six species of Listeria, and only L. monocytogenes is pathogenic for humans. Human disease occurs mainly in immunocompromised people, neonates and in pregnancy, while the cases in immunocompetent people are rare. CNS manifestations of the disease can be in form of meningitis, encephalitis, and also cerebritis and abscess since L. monocytogenes shows tropism for brain and brain stem as well for the meninges. In thi

Ultrasound-based navigation during intracranial burr hole procedures: experience in a series of 100 cases.

BACKGROUND: To establish a rational basis for intraoperative ultrasound guidance in neurosurgical procedures via a single burr hole approach based on the experience of one hundred cases. METHODS: The single burr hole approach is carried out using a bayonet-shaped ultrasound transducer with a tip dimension of 8 x 8 mm. The ultrasound probe with a mounted puncture adapter fits a standard burr hole and allows real-time imaging of the ongoing surgical steps. RESULTS: One hundred cases with five indications have been operated on so far: tapping of the ventricular system (46 patients), tapping of

The Use of POCUS to Identify Subgaleal Fluid Collections and Intracranial Infections.

Point-of-care ultrasound (POCUS) has been useful in describing soft tissue infections, such as cellulitis and abscesses. There has been limited use of ultrasound to describe findings of intracranial infections, such as Pott's puffy tumor, in cases of forehead prominence and signs of infection. In this case series we present POCUS findings in 2 cases of intracranial infections and one case of soft tissue edema without intracranial involvement from a single pediatric tertiary care center. Ultrasound findings revealed subgaleal fluid collections with associated periosteal lifting of the frontal b

Primary pituitary abscess case series and a review of the literature.

OBJECTIVE: Pituitary abscess is a rare condition, with approximately 300 cases reported in the literature. Primary pituitary abscess, the most common type, occurs in previously healthy normal glands. Secondary pituitary abscess is secondary to pre-existing lesions in the pituitary region, such as pituitary adenoma, Rathke's cleft cysts, or craniopharyngioma and infections. MATERIAL AND METHODS: A total of 2281 patients underwent pituitary surgery via endoscopic transsphenoidal approach in Kocaeli University Pituitary Gland Research Center between 1997 and 2018. Among this cohort 9 patients (4

Pituitary abscess: clinical manifestations, diagnosis and treatment of 66 cases from a large pituitary center over 23 years.

PURPOSE: Pituitary abscess (PA) is rare and commonly described in case reports or small case series. This study aimed to better determine salient clinical manifestations related to the diagnosis and appropriate treatment of PA using by far the largest case series of this disease. METHODS: A total of 6361 consecutive patients underwent surgery for pituitary diseases in Peking Union Medical College Hospital between January 1991 and December 2013. Among this cohort, sixty-six patients were diagnosed with PA based on both intraoperative findings and postoperative histopathological evidence. RESU

[Pulmonary nocardiosis with cerebral abscesses mimicking metastatic lung cancer: Three cases and a review of literature].

Nocardiosis is an infectious disease with wide range of clinical features, which can eventually lead to death. The agent responsible belongs to the genus Nocardia that includes about fifty different species. Nocardiosis occurs mainly in immunocompromised hosts. We report here three cases of disseminated nocardiosis misdiagnosed initially as cerebral metastatic lung cancer. These patients, including two immunocompetent hosts, presented with both pulmonary and cerebral lesions. In all three patients, the diagnosis was based on magnetic resonance imaging with diffusion sequence, apparent diffusio

Cerebral abscess caused by Serratia marcescens in a premature neonate.

BACKGROUND: Cerebral abscesses are extremely rare in neonates. Serratia marcescens is an unusual cause of sepsis and neurological spread is especially ominous. PURPOSE: To report the case of a 34-week neonate who developed this rare condition and to discuss diagnostic and therapeutic measures. CASE REPORT: A 34-week male neonate sequentially developed respiratory distress syndrome, early sepsis and necrotizing enterocolitis; later cultures revealed S. marcescens. After deterioration, a cerebral abscess became evident, which revealed S. marcescens. Clinical improvement ensued after high-dose

Percutaneous closure of a persistent left superior vena cava connected to the left atrium.

Cerebral emboli and abscesses are common complications in patients with cyanotic congenital heart disease and right-to-left shunt. Of those, a persistent left superior vena cava (LSVC) draining into the left atrium, directly or through an unroofed coronary sinus, is a rather rare finding. In order to prevent recurrent cerebral emboli and abscesses, correction of this anomaly may be recommended. We report the first case of a patient who underwent a percutaneous closure of a persistent LSVC draining into the left atrium with an Amplatzer ASD occluder.

An unusual case of a compound depressed skull fracture after an assault with a stiletto heel.

Compound depressed skull fractures occur commonly from assault with various sharp and blunt objects. The use of a stiletto heeled shoe as an offensive weapon has not been reported before as a cause of such an injury. However unusual the history of assault, a clear picture of the mechanism of the injury is vital. Patients with scalp lacerations following a direct blow to the head must be evaluated thoroughly. Early investigation and treatment are necessary to reduce the risk of complications, as illustrated by this case.
