Limb lengthening by callus distraction and external fixation has a high rate of complications. We describe our experience using an intramedullary nail (Fitbone) which contains a motorised and programmable sliding mechanism for limb lengthening and bone transport. We performed limb lengthening (callus distraction) with immediate correction of the angulation deformity in 11 patients (16 long-bone segments) in Chiba Children's Hospital. INTRODUCTION: Historically, blocking screws have been used to assist in acute reduction of fractures during intramedullary (IM) nailing. The reverse-rule-of-thumbs (RROT) for blocking screws was introduced to facilitate internal lengthening nail use in deformity correction and limb lengthening. UNLABELLED: Metacarpal lengthening is a useful procedure to address hand deficiencies in children. BACKGROUND: Fractures with related problems including intercalary bone defect, shortening, and malunion are still major problems in orthopedic practice. The Ilizalov's circular external fixator device has several advantages for bone reconstruction but also has certain drawbacks such as its bulkiness and complicated mounting. A 59-year-old woman with bilateral congenital knee dislocation due to Larsen syndrome was treated by arthroplasty. To prevent neurovascular deficiency and major loss of bone substance, a continuous joint distraction was performed by unilateral external fixation. After a period of 25 days and a leg lengthening of 4.5 cm, a constrained total knee endoprosthesis was implanted. Both operations and their postoperative courses were free of complications. A previously planned lengthening of the Achilles tendon was unnecessary.

We report the results and complications of eight consecutive patients who underwent bilateral tibial lengthenings for dwarfism associated with Turner syndrome. Lengthening was performed via distraction osteogenesis with monolateral external fixation. Tibias were lengthened an average distance of 9.2 centimeters or 33 percent of the original tibial length.
