Retrospective comparison of postoperative infection and bone union between late and immediate intramedullary nailing of Gustilo grades I, II, and IIIA open tibial shaft fractures.

BACKGROUND: The optimal method of skeletal stabilization is still controversial. Therefore, we examined the clinical outcomes associated with late (L) versus immediate intramedullary nailing (IMN). METHODS: This was a retrospective comparative study of trauma registry data from an emergency medical care center (university hospital). We examined 85 open tibial shaft fractures (85 patients) treated with L or immediate (I) IMN from January 2004 to December 2010. The L and I groups comprised 37 (33 men, 4 women) and 48 (44 men, 4 women) patients, respectively. The postoperative infection rate, ti

Management of Short Proximal Femur Nail Peri-Implant Fracture of Femur Shaft with Implant Removal and Long Distal Femur Locking Plate and Bone Grafting - A Case Report.

INTRODUCTION: Giant cell tumors of the bone are aggressive and potentially malignant lesions. Juxtaarticular giant cell tumors of the lower end radius are common and is a challenge for reconstruction after tumor excision. Several reconstructive procedures like vascularized and non-vascularized fibular graft, osteoarticular allograft, ceramic prosthesis, and megapros thesis are in use for substitution of the defect in the distal radius following resection. Here, we have analyzed the results of aggressive benign Giant cell tumor of the distal radius treated by en bloc excision and reconstruction

Proximal femoral nailing for intertrochanteric fracture combined with contralateral femoral neck local osteo-enhancement procedure (LOEP) for severe osteoporotic bone loss: An original Italian case series.

Proximal femoral fractures in elderly women are a major cause of morbidity and mortality worldwide and a public health concern. Although pharmacological therapies have shown potential in improving bone mineral density (BMD) and decreasing fracture risk, the current research effort is focused on developing a procedure that can ensure both immediate and long-term efficacy. A minimally-invasive surgical approach, known as AGN1 local osteo-enhancement procedure (LOEP), has been recently developed to promote bone augmentation. The procedure implies the preparation of an enhancement site, a specific

What is the Optimal Nail Length to Treat Osteoporotic Subtrochanteric Fractures? A Finite Element Analysis.

BACKGROUND: Operative management with intramedullary nail fixation remains the definitive treatment of choice for osteoporotic subtrochanteric (ST) fractures; however, there remains no consensus regarding the proper nail length. We aimed to use 3-dimensional finite element (FE) analysis to determine the optimal nail length for the safe fixation of osteoporotic ST fractures. METHODS: Nine modes of FE models were constructed using 9 different lengths of cephalomedullary nails (short nails: 170, 180, and 200 mm; long nails: 280, 300, 320, 340, 360, and 380 mm) from the same company. Commentary on an article by Hannu T. Aro, MD, PhD, et al.: "Recombinant human bone morphogenetic protein-2: a randomized trial in open tibial fractures treated with reamed nail fixation".

This commentary was updated on April 15, 2011, because of a previous error. In the third paragraph, the sentence that had previously read “In the SPRINT, the two groups of open fractures were associated with statistically equivalent reoperation rates, but there was a trend toward better healing in the open fracture group treated with reamed intramedullary nailing.” now reads “In the SPRINT, the two groups of open fractures were associated with statistically equivalent reoperation rates, but there was a trend toward better healing in the open fracture group treated with unreamed intramedullary

Staged limb salvage of a Gustilo-Anderson type IIIC open distal tibial gunshot fracture using cross-leg pedicle flap and delayed intramedullary nailing with bone grafting: a case report.

INTRODUCTION AND IMPORTANCE: Gunshot injuries to the lower limb can cause severe damage to both bone and soft tissues, particularly when the injury occurs at close range. Open fractures of the tibia are especially difficult to manage because of the limited soft-tissue coverage and the high risk of infection, non-union, and other complications. Gustilo-Anderson type IIIC fractures represent the most severe form, as they involve arterial injury that requires urgent repair. Successful limb salvage depends on early debridement, restoration of blood flow, stable fixation, and reliable soft-tissue c

Centromedullary nailing during the second stage of induced membrane (Masquelet) for tibia septic nonunion enables bone union to be achieved without increasing the risk of recurrence of infection, with an earlier return to weight-bearing.

INTRODUCTION: Septic nonunion is one of the most feared complications in traumatology. Two-stage management using the induced membrane technique is a validated treatment option, but to date there is no consensus on the ideal type of osteosynthesis for the second stage of surgery. The aim of this study was to compare the results of two-stage treatment of tibial septic nonunion, depending on the type of osteosynthesis used. HYPOTHESIS: Our hypothesis was that internal centromedullary nailing osteosynthesis during the second stage of management of septic tibia nonunion would achieve bone union a

Carbon/PEEK nails: a case-control study of 22 cases.

BACKGROUND: Interest around carbon/PEEK plates and nails has been raising. The elastic modulus close to the bone, the high load-carrying capacity and radiolucency make CFR/PEEK materials a potential breakthrough. In the literature, there are abundant data about CFR/PEEK plates in the treatment of proximal humerus, distal radius and distal fibula fractures. In patients affected by bone metastasis, CFR/PEEK nails were proved effective and safe with 12 months of follow-up. Very little is known about performances of CFR/PEEK nails in patients affected by other pathologies. PURPOSES: The aim of th

Three-dimensional finite element analysis of intramedullary nail with different materials in the treatment of intertrochanteric fractures.

Intramedullary nails are the common treatment options for femoral intertrochanteric fractures. However, aseptic loosening is considered to be one of the primary forms of failure that can be caused by the stress shielding between the bone and implants. The matching in mechanical properties of implant and bone is a key issue to prevent this failure. Polyetheretherketone (PEEK) and Function-graded (FG) materials are widely used in clinical because of their excellent mechanical properties. In this study, to investigate the biomechanical behaviors of intramedullary nails made of Ti-6Al-4V alloy, St

Leg lengthening using intramedullay skeletal kinetic distractor: results of 57 consecutive applications.

BACKGROUND: Mechanically activated intramedullary lengthening nails are advantageous over external fixator. However, difficulties with the control of the distraction rate are the main drawbacks, which may in turn cause insufficient bone regenerate. PATIENTS AND METHODS: A total of 57 lengthening procedures were performed using intramedullary skeletal kinetic distractor (ISKD) nail in 53 patients (femoral = 45 and tibial = 12). Average length gain was 4.3 ± 1.6 cm. The cause of shortening was post-traumatic (n = 33), congenital (n = 20), post-tumour resection (n = 1), cosmetic femoral lengthen

Extraction of Broken Intramedullary Nails Using Olive-Tipped Guidewire and Minifragment Screws.

In nonunion surgery, removing a broken intramedullary nail can be technically challenging and time-consuming, often requiring either creating a bone window or an open approach to retrieve the broken nail. A simple, effective, and minimally traumatic technique is presented for removing the difficult-to-access part of broken canulated intramedullary nails. First, the olive-tipped guidewire is inserted and advanced down the nail until the olive end protrudes out of the broken nail. A minifragment screw is inserted through a locking screw hole in the broken part under fluoroscopy. The guidewire is

Effect of intramedullary nail stiffness on load-sharing in tibiotalocalcaneal arthrodesis: A patient-specific finite element study.

Tibiotalocalcaneal (TTC) arthrodesis is a procedure to treat severe ankle and subtalar arthropathy by providing pain free and stable fusion using IM nails. These nails can be manufactured with multiple materials and some feature the ability to dynamize the arthrodesis construct. However, the impact of IM nail material and nail dynamization on load-sharing and in the setting of bone resorption have not been quantified. This work utilized a patient-specific finite element analysis model of TTC arthrodesis to investigate IM nails with differing material moduli and the impact of nail dynamization

[Fluoride level in human nails and its significance].

Bone tissue investigation provides the best indicator for fluorine saturation of the organism. However, the intravital bone biopsy--as an invasive method, is not applicable in prophylactic studies. Thus, attention was focussed on the nails that constitute an easily accessible material being able to reflect the chronic accumulation of fluorine in the organism. In the actual paper it has been endeavoured to find out the routes, on which fluorine is deposited in the nails, if there is any dependence between the fluorine content in the nails and the age of the studied subjects, and whether the con

Biomechanical and Clinical Comparative Study of the New Elastic Stable Intramedullary Nailing "MJ-FLEX Orthofix".

BACKGROUND: The objectives of this study were first to compare the results of osteosynthesis with standard Metaizeau (MS) nails versus Metaizeau Junior Flex or MJF Orthofix (MJF) in terms of the stability of long-bone fractures in children, and to study the preoperative and postoperative safety of these new nails. We also carried out an analysis by computer modeling of a femur, in order to compare the biomechanical stability of the 2 nails. The interest in the MJF nails lies in the improvements they bring in terms of stability for long-bone fractures in pediatric patients, as well as the simpl
