Proboscis lateralis with trans-ethmoidal meningocele and congenital nostril hypoplasia: a case report and literature review.

Proboscis lateralis is an uncommon craniofacial malformation characterized by the presence of a rudimentary tubular nasal appendage originating from the medial canthal region. The degree of severity varies according to the extent of associated structural involvement, which may include ocular and nasal abnormalities, cleft lip and/or palate, and concurrent intracranial anomalies. Multidisciplinary approach is the mainstay of definite management. We report the case of a 4-month-old infant who presented with a 3-cm tubular lesion arising from the right medial canthus. The lesion contained a centr

Surgical Approach in a Patient With Agnathia-Otocephaly Complex: Three-Stage Mandibular Distraction Protocol.

Agnathia-otocephaly complex (AOC) is a rare malformation complex of the first pharyngeal arch that is characterized by agnathia/dysgnathia, microstomia, aglossia/hypoglossia and variable displacement of the ears. Only 11 post-infancy patients with severe AOC have been described in the literature, and the incidence of this malformation complex is estimated to be 1 per 70,000 births. In this brief clinical study, the authors describe the case of an 18-year-old female diagnosed with AOC who underwent a 3-step mandibular distraction protocol with an external distraction device. The surgical protoc

Craniofacial morphological changes of familial bilateral hypodontia of maxillary premolars.

The hypodontia of a permanent tooth from a dental group represents a normal evolution in human dentition morphology. Nevertheless, the hypodontia of two teeth within a dental group is a rare developmental anomaly when not associated to a systemic syndrome. The aim of this study was to report two rare cases of four maxillary premolars hypodontia, not including the third molar, of two white women from the same family. There were presented clinical, radiological and genetic findings. These cases are of interest to practitioners for four aspects: the atypical phenotype of hypodontia, the complexit

Rapp-Hodgkin syndrome.

A 5-year-old boy with a history of a bifid uvula and a submucosal cleft palate presented for evaluation of brittle nails. The physical examination demonstrated cup-shaped ears, a broad nasal root, thin upper lip, mid-facial hypoplasia, coarse hair, and twenty-nail dystrophy. The clinical presentation of ectodermal dysplasia with cleft palate was consistent with Rapp-Hodgkin syndrome, which is one of several allelic diseases associated with mutations in the TP63 gene. The clinical manifestations of Rapp-Hodgkin as well as other ectodermal dysplasias with clefting are discussed.

Stable correction of antimongoloid deformity of the external canthus.

The antimongoloid deformity comprises inferior dystopia of the lateral canthus. It causes a sad looking appearance, inferior scleral show and laxity of the lower eyelid. Many different procedures have been described to correct this deformity. We believe that a stable and lasting correction can be achieved using a coronal-approach subperiosteal mask-lift, combined with DMAS (Deep Musculo-Aponeurotic System) suspension of a temporo-masseteric anchor-flap (instead of a simple canthoplasty). If bony augmentation of the cheek bones is required, we use an autograft of cartilage of the concha of the

Absent lacrimal ducts, distichiasis, dysmorphic features, and brachydactyly: a case report.

We report a male patient presenting with the association of absent lacrimal ducts, distichiasis, dysmorphic facial features and limb abnormalities. Extensive chromosomal studies showed normal chromosomes. We discuss differential diagnoses such as Setleis, Char and Lacrimo-Auriculo-Dento-Digital (LADD) syndromes. This may represent a novel entity for which parental consanguinity would support an autosomal recessive mode of inheritance.

Prevention of etretinate-induced craniofacial malformations by vitamin B6 in the rat.

The preventive effect of vitamin B6 on etretinate-induced malformations in pregnant Sprague-Dawley rats was studied. The etretinate-induced malformation was produced by intraperitoneal administration of 10 mg/kg etretinate at embryonal day 8.5. Vitamin B6 was administered as intramuscular injections at embryonal day 7.5 and 8.5. Vitamin B6 reduced the number and severity of facial clefts, micrognatia, meningocele, microtia and blood vessel anomalies. It is suggested that vitamin B6 induces suppressive effects on etretinate-induced teratogenesis when administered before or at the same time as t

Ritscher-Schinzel syndrome can be characterized as an endosomal recyclinopathy.

Ritscher-Schinzel syndrome (RSS) is a congenital malformation syndrome characterized by cerebellar, cardiac, and craniofacial malformations and phenotypes associated with liver, skeletal, and kidney dysfunction. The genetic cause of RSS remains to be fully defined, and limited information is available regarding the root cause of the multiple tissue phenotypes. Causative mutations in the Commander multiprotein assembly are an emerging feature of this syndrome. Commander organizes the sorting nexin-17 (SNX17)-dependent recycling of hundreds of integral membrane proteins through the endosomal net

Nablus syndrome: Easy to diagnose yet difficult to solve.

Nablus syndrome was first described by the late Ahmad Teebi in 2000, and 13 individuals have been reported to date. Nablus syndrome can be clinically diagnosed based on striking facial features, including tight glistening skin with reduced facial expression, blepharophimosis, telecanthus, bulky nasal tip, abnormal external ear architecture, upswept frontal hairline, and sparse eyebrows. However, the precise genetic etiology for this rare condition remains elusive. Comparative microarray analyses of individuals with Nablus syndrome (including two mother-son pairs) reveal an overlapping 8q22.1 m

The role of cathepsin K in oral and maxillofacial disorders.

Cathepsin K (CTSK) was thought to be a collagenase, specifically expressed by osteoclasts, and played an important role in bone resorption. However, more and more research found that CTSK was expressed in more extensive cells, tissues, and organs. It may not only participate in regulating human physiological activity, but also be closely related to a variety of disease. In this review, we highlight the relationship between CTSK and oral and maxillofacial disorders on the following three aspects: oral and maxillofacial abnormities in patients with pycnodysostosis caused by CTSK mutations, oral

Human facial dysostoses.

The human facial dysostoses can be subdivided into mandibulofacial dysostoses (MFDs) and acrofacial dysostoses (AFDs). The craniofacial phenotypes of the two groups of patients are similar. Both types are thought to be related to abnormal migration of neural crest cells to the pharyngeal arches and the face. The craniofacial anomalies shared by the two groups consist of downslanting palpebral fissures, coloboma of the lower eyelid, from which the eyelashes medial to the defect may be absent, hypoplasia of the zygomatic complex, micrognathia, and microtia, which is often associated with hearing

Pseudoaminopterin syndrome.

Pseudoaminopterin syndrome or aminopterin syndrome-like sine aminopterin (ASSA syndrome--OMIM 600325] is a rare autosomal recessive syndrome defined by characteristic dysmorphic features, skeletal defects, limb anomalies, cryptorchidism, and growth retardation. The syndrome owes its name to the fact that patients resemble the children exposed to aminopterin or to methotrexate, two dihydrofolate reductase inhibitors used for chemotherapy, or as an abortificient in early pregnancy. Ten patients have been described with pseudoaminopterin syndrome. Their phenotype is variable, and differs from the

[Dysgnathia complex, a rare deviation].

Dysgnathia complex is a rare disorder characterized by a hypoplastic or absent mandible (micro- or agnathia), a limited mouth opening (trismus), a small tongue (microglossia) and ear anomalies. Since 1961 only 21 cases of isolated dysgnathia complex are reported in literature. Diagnosis of this disorder is difficult due to the general unfamiliarity of the disorder as well as the less apparent disparity with other known developmental syndromes of the head and neck area. A case of a female recently diagnosed with dysgnathia complex is discussed.

Craniofacial anomalies: from development to molecular pathogenesis.

Advances in developmental biology combined with progress in human genetics are helping us decipher how the craniofacial region develops and how the consequences of misdirected development result in malformation. This review describes the molecular etiology of a number of craniofacial developmental anomalies. The more common craniofacial anomalies cleft lip and palate and craniosynostosis, as well as cleidocranial dysplasia, hemifacial microsomia, holoprosencephaly, enlarged parietal foramina, Treacher Collins syndrome and cherubism are discussed.
