Pathogenesis of acute respiratory illness caused by human parainfluenza viruses.

Human parainfluenza viruses (HPIVs) are a common cause of acute respiratory illness throughout life. Infants, children, and the immunocompromised are the most likely to develop severe disease. HPIV1 and HPIV2 are best known to cause croup while HPIV3 is a common cause of bronchiolitis and pneumonia. HPIVs replicate productively in respiratory epithelial cells and do not spread systemically unless the host is severely immunocompromised. Molecular studies have delineated how HPIVs evade and block cellular innate immune responses to permit efficient replication, local spread, and host-to-host tra…

Pediatric otolaryngologic manifestations of gastroesophageal reflux disease.

In adults, an estimated 4% to 10% of chronic, nonspecific laryngeal disease seen in otolaryngologic clinics is associated with gastroesophageal reflux disease (GERD). Although no such estimates exist in children, many investigators have reported extraesophageal manifestations of GERD, of which the most common is the association of GERD with asthma and chronic cough. A variety of signs and symptoms of otolaryngologic disease also have been attributed to GERD, including hoarseness, laryngitis, chronic rhinitis, sinusitis, globus pharyngeus, recurrent croup, laryngomalacia, stridor, subglottic st…

Congenital tracheal stenosis.

Twenty-one cases of congenital tracheal stenosis seen at the Royal Alexandra Hospital for Children 1971 through 1980 were reviewed with regard to the clinical features, associated anomalies, endoscopic findings and radiological evaluation. Congenital tracheal stenosis was usually a serious problem often associated with other major anomalies of the respiratory tract, esophagus, or skeleton. There was no set pattern of presentation. The presenting features included: persistent wheeze or stridor, atypical "respiratory distress," and atypical "croup" or "bronchiolitis." Documentation of the nature…

Comparative analysis of clinical characteristics of pandemic influenza a/h1n1 and seasonal influenza a infections in hospitalized children.

Pandemic influenza A/H1N1 (piA) caused significant morbidity and mortality worldwide. We compared characteristics of piA (2009/2010) to those of seasonal influenza A (2007/2008/2009) in 134 (piA/seasonal influenza A: N = 55/79) hospitalized patients <18 years of age. Chronic disease was present in 25%/33%. Most common symptoms were fever (87%/94%), cough (78%/86%), rhinitis (76%/76%) and pharyngitis (67%/68%). Croup syndrome (15%/3%), conjunctivitis (31%/10%) and febrile seizures (26%/13%) were more frequent in piA; 64%/53% patients had ≥1 complication, and 5 (3/2) were admitted to intensive c…

Budesonide inhalation suspension: a review of its use in infants, children and adults with inflammatory respiratory disorders.

Budesonide, a topically active corticosteroid, has a broad spectrum of clinically significant local anti-inflammatory effects in patients wi … s as effective as, or more effective than oral prednisolone in improving symptoms. In children with croup, single 2 or 4mg dosages of budesonide inhalation suspension were significantly more effective than placebo and as effective as oral dexamethasone 0.6 mg/kg or nebulised L-epinephrine (adrenaline) 4mg in alleviating croup symptoms and preventing or …

A randomized, controlled trial of vitamin A in children with severe measles.

BACKGROUND: Measles kills about 2 million children annually, and there is no specific therapy for the disease. It has been suggested that vitamin A may be of benefit in the treatment of measles. METHODS: We conducted a randomized, double-blind trial involving 189 children who were hospitalized at a regional center in South Africa because of measles complicated by pneumonia, diarrhea, or croup. The children (median age, 10 months) were assigned to receive either vitamin A (total dose, 400,000 IU of retinyl palmitate, given orally; n = 92) or placebo (n = 97), beginning within five days of the …

Subglottic Stenosis.

Subglottic stenosis refers to narrowing of the airway diameter below the vocal folds and may be congenital or acquired. Typical signs and symptoms range from recurrent croup and exertional stridor to complete airflow obstruction requiring tracheotomy. Management of moderate and severe subglottic stenosis often requires intricate surgical techniques. To optimize the success of these surgeries, a thorough assessment of the child's airway, lungs, reflux, and swallow needs to be evaluated. In order to provide concerted and coordinated care between typically otolaryngology (ENT), pulmonary, gastroe…

Pulmonary hypertension and acute pulmonary edema in a 23-year-old male with a history of an upper respiratory tract infection.

The pathophysiology of upper-airway obstruction (UAO) is complex. Possible causes of UAO that may lead to acute respiratory failure, are as follows: infections like acute epiglottitis and croup, obstructing tumors in the base of the tongue, larynx or hypopharynx, aspirated food or liquid contents, obesity and anatomical variations. Management changes according to the pathogenesis of the disorder. In patients with severe carbon dioxide retention or apnea, emergency endotracheal intubation must be carried out. Hereby, we describe a 23-year-old patient with susceptible upper-airway anatomy and UA…

Acute laryngeal obstruction in children. A fifty-year review.

When dealing with acute laryngeal obstruction, the first important consideration is the differential diagnosis of the cause. The author considers the following six types: acute laryngotracheobronchitis, acute epiglottitis, diphtheria, supraglottic allergic edema (angioedema), subglottic allergic edema (spasmodic croup) and foreign body in the larynx or trachea. He traces the development of the treatments that have in 50 years lowered the mortality rate from 70% to practically zero. High humidity, best supplied by an ultrasonic nebulizer, antibiotics, and corticosteroids in very high dose have …

Airway Obstruction from a Tracheal Mass in a 5-Year-Old: A Case Report.

Stridor is a common presenting symptom for children in emergency departments (EDs) and usually represents an infectious process, such as croup, or aspiration of a foreign body. We present the case of an otherwise healthy 5-year-old girl with episodic increased work of breathing for several months that was initially diagnosed as asthma by her primary care physician. She subsequently presented to the ED with acutely worsening noisy breathing and dyspnea. Patient and parent denied any recent foreign body ingestions or choking episodes. We gave multiple doses of racemic epinephrine in the ED witho…

The relation between recurrent acute subglottic laryngitis and asthma in children.

OBJECTIVE: Acute subglottic laryngitis is caused by some viruses and may recur several times in children. Risk factors underlying the recurr … follow-up period four children showed asthma symptoms and in three of these patients recurrences of croup attacks disappeared after asthma treatment with nebulized budesonide. CONCLUSION: We concluded that recurrent acute subglottic laryngitis may be an early sign of subsequent asthma development.

Management of Respiratory Illness in a Pediatric Patient With Chromosome 16p13.3 Microduplication Syndrome and Potential Seizure Risk.

This case report outlines the management of a 16-month-old girl with chromosome 16p13.3 microduplication syndrome who presented with an acute respiratory illness. For signs and symptoms of croup, pneumonia, and otitis media, this patient required multiple administrations of racemic epinephrine, steroids, and albuterol to reduce inflammation and alleviate respiratory distress. Children with underlying medical conditions and genetic syndromes are likely to have a more complicated clinical course in the setting of acute illnesses. Thus, these patients require more intensive interventions and prol…

Acute respiratory tract infections of children in hospital: a viral and Mycoplasma pneumoniae profile.

Respiratory specimens and blood were collected from all infants and children admitted with acute respiratory illness to a paediatric unit in Christchurch from May to November (late autumn, winter and spring) 1983, to define the viral aetiological agents involved. A virus or Mycoplasma pneumoniae was identified in 160 (50%) of 317 children studied by the rapid indirect immunofluorescence, virus culture and/or serological techniques. Aetiological agents were detected in 71% of children with bronchiolitis, 57% with pneumonia, 53% with bronchitis, 40% with laryngotracheitis (croup), and 45% with u…

Bacterial tracheitis in children: Approach to diagnosis and treatment.

Bacterial tracheitis is due to a secondary bacterial infection of the trachea, resulting in the formation of mucopurulent exudates that may acutely obstruct the upper airway, resulting in a life-threatening condition. Bacterial tracheitis should be considered in the differential diagnosis of any child with acute upper airway obstruction. This diagnosis should also be considered in any child with viral croup that is nonresponsive to conventional therapy. The only definitive way to diagnose bacterial tracheitis is by direct visualization of the trachea via bronchoscopy; however, this may not be …
