BACKGROUND: Minimally invasive posterior cervical foraminotomy for radicular symptoms has become more prevalent. The reported experience with microscopic tubular assisted posterior cervical laminoforaminotomy (MTPF) for the treatment of radicular pain is lacking. Tubular assisted techniques have been considered to offer significant benefit, over open procedures, in terms of minimizing tissue damage, operative time, blood loss, analgesic requirements and length of hospital stay. We hypothesized that MTPF reduces post-operative analgesic requirements and length of hospital stay over the traditio

Lateral recess stenosis is a common cause of lumbar radiculopathy in adults. A lumbar nerve root travels in the lateral recess prior to exiting the spinal canal via the neural foramen. In the lateral recess, the traversing nerve root is susceptible to compression by the degenerative hypertrophy of the medial facet in addition to hypertrophied ligamentum flavum and herniated intervertebral disc.1 These degenerative changes are also typically associated with neural foraminal stenosis. Surgical treatment in unilateral cases consists of hemilaminectomy, medial facetectomy, foraminotomy, and, if ap

BACKGROUND: Cervical transforaminal epidural steroid injections (CTFESI) are commonly performed to treat cervical radicular pain and decrease the need of surgery. The literature is mixed regarding the efficacy of CTFESI in obviating the need for cervical spine surgery.
OBJECTIVE: The purpose of this study was to assess the rates of cervical spine surgery and spinal interventions after initial CTFESI for patients with radicular pain.
METHODS: TriNetX, a large national database, was queried from 2004 to 2025 for patients who underwent CTFESI for cervical radiculopathy. The rates of treatment p

PURPOSE: Unilateral hemilaminectomy with bilateral decompression (BDZ) was proposed as an alternative decompressive procedure in cervical spondylotic myelopathy (CSM). Despite promising clinical results, the destabilizing effect is yet unknown. We therefore performed a biomechanical study to investigate whether lateral mass screw fixation should follow BDZ.
METHODS: Six human C2-C7 cervical specimens were tested under various conditions: native, unilateral hemilaminectomy with bilateral decompression without/with fixation (BDZ/BDF), unilateral hemilaminectomy with bilateral decompression and

Lumbar spinal stenosis is typically a degenerative condition that leads to compression of the spinal canal and lateral recess, resulting in leg pain and walking disability. Surgical management is indicated after failure of non-surgical management or rapidly worsening neurological impairment. The traditional approach is a laminectomy with foraminotomy and partial facetectomy but a newer minimally invasive option, unilateral laminectomy for bilateral decompression (ULBD), seems to demonstrate the better postoperative outcomes due to its unilateral exposure. ULBD involves a midline incision, open

STUDY DESIGN: Case report.
OBJECTIVE: To report three cases of Brown-Sequard syndrome (BSS) associated with cervical disc herniation.
METHOD: We describe clinical and radiographic review of three patients who presented with BSS caused by cervical disc herniation. Three patients presented with ipsilateral motor weakness and diminished sensation to pain and temperature on the contralateral side. Magnetic resonance images of the cervical spine in all cases, showed a large paramedian disc herniation at C5-C6, with ipsilateral severe spinal cord compression. Microsurgical removal of the herniated

PURPOSE: To describe endovascular repair of traumatic iliac pseudoaneurysm and arteriovenous fistula (AVF) following spinal surgery.
CASE REPORT: A 48-year-old male underwent minimally invasive L5-S1 hemilaminectomy, foraminotomy, and microdiscectomy spinal surgery for trauma related to a motor-vehicle accident. Postoperative angiogram demonstrated pseudoaneurysm of the right internal iliac artery and AVF at the common iliac bifurcation with the right iliac vein with prompt filling of the iliac vein and vena cava. The second patient, a 25-year-old female, underwent minimally invasive L4-S1 he

STUDY DESIGN: A retrospective cohort study.
OBJECTIVES: While numerous studies have investigated surgical outcomes for cervical spondylosis, limited research has examined the impact of surgical techniques in patients with unilateral upper extremity proximal muscle weakness. This study compares clinical outcomes between anterior cervical decompression and fusion (ACDF) and posterior decompression (PD) with foraminotomy in patients with cervical spondylosis presenting with unilateral proximal muscle weakness.
METHODS: This study included 45 patients diagnosed with either cervical spondylotic a

BACKGROUND: New technologies for sacroiliac joint fusion (SIJF) have demonstrated improvements in pain and function. Sacral radiculopathy is a reported complication. We present a unique case of S1 radiculopathy after lateral transiliac minimally invasive SIJF. The patient provided Health Insurance Portability and Accountability Act (HIPAA)-compliant consent for the inclusion of their clinical information in this report.
CASE REPORT: A 40-year-old woman with SI joint dysfunction underwent right-sided SIJF. She reported resolution of her preoperative symptoms but developed new pain radiating to

Anterior cervical foraminotomy is a motion-preserving and precise decompressing surgery with minimal bone removal. As preserving the motion segment and maximizing decompression are contradictory concepts, proficiency is needed to balance these requirements. Anatomical knowledge and good intraoperative orientation are essential to reach the nerve roots accurately in a narrow surgical field.

Nerve root compression by foraminal pathology is challenging for a surgeon to decompress without violating the facet joint, which may necessitate a fusion procedure. One nonfusion approach to foraminal pathology is a combination intracanal approach for a laminotomy/foraminotomy followed by a paraspinal Wiltse approach for far lateral decompression. Unfortunately, even with the combination approach, it continues to be difficult to achieve adequate decompression without violating much of the facet joint overlying the nerve root. Spine endoscopy offers the ability to decompress the foraminal port

STUDY DESIGN: The incidences of postoperative C5 palsy between a group treated by a standardized diagnostic and surgical treatment and a control group treated by a different cervical laminoplastic technique were prospectively compared.
OBJECTIVE: To investigate the cause, risk factors, and prevention of C5 palsy after laminoplasty for cervical myelopathy.
SUMMARY OF BACKGROUND DATA: No one factor could predict postoperative C5 palsy, although postoperative C5 palsy is a clinically significant complication of cervical laminoplasty.
METHODS: One hundred eleven patients who underwent laminopla

Anatomical variations of neural structures in the cervical spine are rare and are not necessarily visible on pre-operative imaging. The authors report an unusual arrangement of neural structures identified during cervical foraminotomy. Anatomical variations of the cervical nerve roots are reviewed and their importance in neurosurgical practice is discussed.

Contralateral radiculopathy is an infrequently reported postoperative complication in patients with degenerative spondylolisthesis (DS) who receive transforaminal lumbar interbody fusion (TLIF). Herein, we report two clinical cases with this complication associated with preoperative facet tropism. Case 1 involved a patient with L4/5 DS presenting with radiating pain in the right lower extremity; new-onset left lower limb weakness and radicular pain occurred immediately after TLIF via the right side. A revision resection of the left L5 superior articular process (SAP) led to marked improvement

Twenty three patients with lumbar spondylolisthesis (6 Grade I, 14 Grade II and 3 Grade III) with low back pain, and radicular pain in 14, were treated surgically when they failed to respond to conservative treatment. Their average age was 33.2 years. Bilateral posterolateral spinal fusions with autologous iliac bone chips, supplemented by a sacrospinalis muscle-pedicle bone graft on the left side, were undertaken after laminectomy and foraminotomy in all except in two adolescents, where laminectomy and foraminotomy were avoided. The period of follow-up varied from 26 to 126 months (average 65

Cervical osteoplastic laminectomy using ceramic or apatite beads as implants was performed in 31 patients with symptomatic cervical bony canal stenosis. Patients with degenerative changes such as osteochondrosis or disc herniation are included in this study but those with spinal cord tumor, arachnoid cyst or ossification of the posterior longitudinal ligament are not. Initially foraminotomies were performed at the C5-8 levels in an attempt at preventing worsening of radicular signs caused by posterior displacement of the cord and resultant tethering effect on the nerve root. Connecting the med

Lumbar foramimal stenosis is one of the important disorders underlying radicular symptoms and unrecognized or recurrent foraminal stenosis may result in failed back surgery. We report a 47-year old man with foraminal stenosis of the lumbar spine associated with a bone spur at the pars interarticularis. He suddenly experienced severe right leg pain, lumbago and gait disturbance. Radiographic studies revealed foraminal stenosis on the right L5/S1associated with a bone spur at the pars interarticularis. There was no dural sac compression by spondylolysis. As his symptoms failed to respond to cons

BACKGROUND: Morphological features of foraminal stenosis in cervical spondylotic radiculopathy and the adequate extent of facet resection in posterior cervical foraminotomy remain uncertain. Herein, we evaluated quantitatively foraminal widths in cervical spondylotic radiculopathy on dynamic flexion-extension computed tomography using a novel three-dimensional analysis method and determined the extent of facet resection in posterior cervical foraminotomy.
METHODS: Seventeen patients undergoing posterior cervical foraminotomy for cervical spondylotic radiculopathy were evaluated. A neuroforame
