Evaluation of breastfeeding and infant feeding attitudes among syrian refugees in Turkey: observations of Syrian healthcare workers.

BACKGROUND: The influx of Syrian refugees into Turkey has highlighted the importance of supporting breastfeeding practices among this vulnerable population. We aimed to evaluate the breastfeeding and infant feeding attitudes of Syrian mothers based on the observations of Syrian healthcare workers (HCWs). METHODS: An online form including 31 questions was prepared in Turkish, Arabic, and English languages and distributed to HCWs, working in refugee health centers via e-mail, WhatsApp, or text message with the help of Ministry of Health in Turkey between January 2020 and March 2020. The questio

Increasing access to family planning services among women receiving medications for opioid use disorder: A pilot randomized trial examining a peer-led navigation intervention.

OBJECTIVE: High rates of unintended pregnancy occur among women with opioid use disorder (OUD). OUD treatment settings may provide an ideal opportunity to address the family planning needs of patients. However, few studies have rigorously evaluated interventions designed to address family planning needs in the OUD treatment setting. This study assessed the feasibility, acceptability, and preliminary efficacy of a peer-led navigation intervention designed to educate and link women receiving medications for OUD to family planning services. METHODS: The study recruited women from four OUD treatm

Integration of trachoma control into primary health care: the Tanzanian experience.

Tanzania was among the first countries to launch a trachoma control program with support from the International Trachoma Initiative (ITI) using surgery, antibiotics, facial cleanliness, and environmental improvement (SAFE) strategy with azithromycin. More than one million children less than 10 years of age in Tanzania have active disease and an estimated 54,000 people have trichiasis. Since 2000, Tanzania has implemented major health sector reform that have been carried out in three phases in 114 districts. A key aspect of the reform process is the policy of developing locally distributed esse

Capacity, Financing, and Structural Strain in Mental Health Care in the United States: A Narrative Review.

The strain on the US mental health care system is structural, evidenced by a high demand for services that outstrips partial treatment coverage and limited behavioral health spending. This deficit has widespread consequences, spilling over and burdening emergency services, correctional facilities, and homelessness support systems. The narrative review examined interactions among capacity, financing, and cross-sector consequences by synthesizing surveys, claims, workforce projections, facility and crisis-service inventories, and homelessness and justice assessments identified through database a

Beyond the Bedside: Addressing Structural Gaps in the Care of Undocumented, Seriously Ill Patients.

Undocumented patients face structural constraints to accessing life-sustaining treatments in the subacute setting due to gaps in health care coverage. We present the case of a 38-year-old Spanish-speaking woman with short bowel syndrome, dependent on prolonged parenteral nutrition after critical illness, who faced additional challenges as an undocumented and uninsured patient. Despite medical stabilization and a consistent desire to pursue life-prolonging care, discharge planning was complicated by a health care system not designed to support individuals without legal status or insurance. Why Safety Planning Matters in School Social Events: Lessons From a Special Education Case.

BACKGROUND: Emergencies during school social activities are often overlooked, as these events are typically assumed to be covered by general safety plans. METHODS: This study examined an emergency during a school-wide social event in a special education school using qualitative interviews. Data were collected from 32 school personnel and analyzed using reflexive thematic analysis. RESULTS: The results highlight four areas: (1) planning gaps, (2) panic response, (3) event conditions, and (4) limited organizational learning. School social activities function as temporary risk ecologies in whic

Progress in family planning in Sierra Leone: a mixed-methods case study.

INTRODUCTION: Expanding access to voluntary family planning is a global health priority, yet progress has been uneven across low-income and middle-income countries. Despite being a fragile state with historically high fertility and limited health resources, Sierra Leone has achieved one of the fastest increases in modern contraceptive prevalence in sub-Saharan Africa. This study explores the drivers of this progress through a mixed-methods case study. METHODS: We analysed national-level data from urban and rural settings, and engaged diverse stakeholders including family planning professional

Causes of death from community settings: insights from verbal autopsy implementation in Tanzania.

BACKGROUND: Approximately 70% of deaths in Tanzania occur outside health facilities and are often unreported or lack cause of death (COD) information. Consequently, health planning relies on data representing only 10%-15% of the population. To address this gap, Tanzania integrated verbal autopsy (VA) into its civil registration and vital statistics (CRVS) to generate more comprehensive COD data for mortality reporting. This manuscript shares Tanzania's VA implementation experience from 2017 to 2020, focusing on its integration into the CRVS system, and assesses its capacity to generate accurat

Nature and the mind: Towards nature-based mental health care and prevention.

The United Kingdom (UK) Government's recent 10 Year Health Plan for England aims to fix a "broken" and increasingly unaffordable health system through a shift from sickness to prevention, hospital to community, and analogue to digital, with a focus on mental as well as physical health. However, the 171-page strategy for a healthy nation does not contain a single reference to nature or biodiversity. We review the significant benefits of nature and biodiversity to health and outline how these benefits can and should be incorporated into the 10 Year Health Plan. We provide examples of effective a

Equity and Resilience in Higher Education: An Examination of Environmental Injustices Affecting College Students from Marginalized Populations Amid Climate Crises.

As climate crises become more severe, higher education institutions face unprecedented challenges in protecting and supporting their students. This conceptual paper explores the intersection of equity and resilience in higher education, emphasizing how structural inequities increase climate-related vulnerabilities among marginalized student groups, especially low-income, first-generation, and BIPOC students. Using the Social Ecological Model and the Whole Community Approach, this paper suggests that climate resilience in higher education must go beyond traditional emergency management to inclu

Treatment adherence and support for people who inject drugs taking direct-acting antiviral therapy for hepatitis C infection.

A community-based public health facility in Sydney, Australia, the Kirketon Road Centre (KRC), provides health care to people who inject drugs (PWID), homeless and other marginalized people. Since March 2016, KRC has provided treatment for chronic hepatitis C virus (HCV) with direct-acting antivirals (DAAs). We aimed to evaluate treatment adherence amongst clients taking DAAs in a highly marginalized population. All clients who commenced DAA therapy prior to March 2018 at KRC were included in this observational cohort with a subset of clients attending daily or weekly for enhanced adherence su

Breastfeeding, Breastmilk Feeding and Formula Feeding Within the Closed, Temporary Accommodation of Australia's COVID-19 Quarantine.

During 2021-2022, families with infants and young children were among the > 450,000 people who spent 2 weeks in Australia's institutional COVID-19 quarantine. Given pre-existing lack of emergency planning for children, there were concerns about whether parents would be adequately supported in this temporary accommodation. This paper reports on research considering the breastfeeding, breastmilk feeding and formula feeding of infants and young children in Australia's COVID-19 quarantine. Two-hundred and sixteen parents who quarantined with children aged 0-36 months completed an online survey wit

Cervical cancer screening: the complex interplay of medical infrastructure, society, and culture.

Cervical cancer remains a leading cause of cancer death in many low- and middle-income countries. Key issues that must be addressed include the obstacle of poverty, the importance of community representatives, and the credibility of the medical establishment. Better understanding of the complex layers of social ecology and the social determinants of health will allow public health planners to build sustainable programs.

Grading in Gerodontology: A Framework.

INTRODUCTION: Understanding and anticipating oral health deterioration in older adults requires more than describing their current oral and general health. Risk factors arising from oral, systemic and social domains strongly influence disease progression, care complexity and the ability to maintain function over time. We aimed to establish a framework for staging and grading oral health and disease in older adults, with grading systematically assessing and describing determinants of oral health deterioration. The developed framework is intended to support care planning, education, research and

"When you first walk out the gates…where do [you] go?": Barriers and opportunities to achieving continuity of health care at the time of release from a provincial jail in Ontario.

We aimed to explore continuity of health care and health barriers, facilitators, and opportunities for people at the time of release from a provincial correctional facility in Ontario, Canada. We conducted focus groups in community-based organizations in a city in Ontario, Canada: a men's homeless shelter, a mental health service organization, and a social service agency with programs for people with substance use disorders. We included adults who spoke English well enough to participate in the discussion and who had been released from the provincial correctional facility in the previous year.

Pilot implementation of an online program for family and friends supporting the mental health of paramedics in Australia: Lessons learned.

INTRODUCTION: Paramedics experience disproportionately high rates of mental health concerns, often relying on informal support from family and friends. While this support is vital, it can impose significant strain on family and friends, who frequently lack the necessary resources and skills for their role. To address this, a novel online program, Minds Together, was developed specifically for family and friends to enhance their ability to support the mental health of paramedics while prioritizing their own mental health and wellbeing. METHODS: The feasibility and acceptability of the program
